Abstract:Objective To analyze the changes of hospitalization expenditure of tertiary hospitals before and after medical price adjustment, so as to evaluate the policies’ effect and provide empirical evidence on policy improvement. Methods Based on the institutional data from 2015 to 2018 and first page of medical records from 2011 to 2018 of 9 tertiary hospitals in Xi’an, structure variation degree analysis method was used to analyze the influence of medical price adjustment on the average hospitalization expenditure per time and its inner structure, and the interrupted time series analysis method was used to analyze the immediate and long-term effect of medical price adjustment on total hospitalization expenditure. Results Hospitalization expenditure per time was reduced after the adjustment（P=0.023）. Medical service items representing the value of technical services of medical personnel had been increased, including sickbed expense, diagnosis expense, treatment expense, operation expense and nursing expense. The proportions of laboratory expense and drug expense were reduced, but the proportions of inspection and sanitary material expense were still increased. Drug expense, sanitary material expense and operation expense significantly changed in structure variation before and after the adjustment. Medical price adjustment had the immediate effect on increasing the total hospitalization expenditure（β=1412.33, P=0.017） and the long-term effect on reducing it（β=-449.59, P＜0.001）. Conclusion Medical price adjustment reduces tertiary hospitals’ hospitalization expenditure and optimizes the expenditure’s structure. However, there is a possibility of irregular use of medical services. It is suggested for the government to play the key role of medical insurance to control expenditure, improve the pricing standard and adjustment mechanism of medical service items, implement the abolishment of medical consumables markup, and strengthen the monitoring of sanitary material and inspection expense proportion.
