Abstract:Tumor necrosis factor alpha (TNF-α) is a homotrimeric protein encoded within the major histocompatibility complex (MHC). TNF-α can bind its receptors in the body to play a role in immune regulation and to participate in the various pathophysiology processes including fever, inflammation, infection, wound healing and tumors necrosis. TNF-α, an important component of inflammatory pathways, is up-regulated in the synovial tissue, synovial fluid and serum of rheumatoid arthritis (RA) patients. In RA patients, TNF-α along with many kinds of tissue factors and matrix proteins together promotes the inflammatory response, abnormal apoptosis of synovial cells, pannus formation and cartilage and bone destruction, which maintain the sustainability of the progress of RA. Thus, TNF-α may be a clinical indicator of RA activity as well as an effective target for RA.

Abstract:Rheumatoid arthritis(RA) is a common chronic,inflammatory,systematic autoimmunity disease.Tumor necrosis factor(TNF-α) and prostaglandin E2(PGE2) are major inflammatory mediators in RA.A lot of compounds are available to treat RA,but most of anti-rheumatic drugs only stay in controlling inflammation symptom and can't modify the progression of the disease,simultaneously accompany with severe side adverse effect.The article reviews the relationship between PGE2,its correlation factors and TNF-α in rheumatoid arthritis and the therapeutic effect of drugs on RA.

Abstract:The pathogenesis mechanisms of diseases like rheumatoid arthritis(RA),Crohn’s disease(CD) are quite similar.Tumor necrosis factor(TNF) is likely to be of fundamental importance on the generation and development and plays a key role on the cytokine immune network.High level of TNF could induce inflammation.There are some TNF inhibitors marketed or in research,and both of them had remarkable efficacy.Along with TNF inhibitors being widely used,it’s more important to concern about the safety profiles.Therefore,this article reviews the relationship between TNF and diseases,and five TNF protein inhibitors available through structures and clinical use,keeping eyes on their possible side effect and providing some new TNF inhibitors with bright future.

Abstract:Background: This study aimed to investigate the molecular mechanism of Radix Paeoniae Alba (white peony, WP) in treating immune inflammatory diseases of rheumatoid arthritis (RA) and tumor necrosis factor-alpha (TNF-α) inhibitors (TNFis) by using network pharmacology and molecular docking. Methods: In this study, the ingredient of WP and the potential inflammatory targets of RA were obtained from the Traditional Chinese Medicine Systematic Pharmacology Database, GeneCard, and OMIM databases, respectively. The establishment of the RA–WP-potential inflammatory target gene interaction network was accomplished using the STRING database. Network maps of the WP–RA-potential inflammatory target gene network were constructed using Cytoscape software. Gene ontology (GO) and the biological pathway (KEGG) enrichment analyses were used to further explore the RA mechanism and therapeutic effects of WP. Molecular docking technology was used to analyze the optimal effective components from WP for docking with TNF-α. Results: Thirteen active ingredients and 71 target genes were screened from WP, and 49 of the target genes intersected with RA target inflammatory genes and were considered potential therapeutic targets. Network pharmacological analysis showed that the WP active ingredients such as mairin, DPHCD, (+)-catechin, beta-sitosterol, paeoniflorin, sitosterol, and kaempferol showed better correlation with RA inflammatory target genes such as PGR, PTGS1, PTGS2, NR3C2, TNFSF15, and CHRM2, respectively. The immune-inflammatory signaling pathways of the active ingredients for the treatment of RA are the TNF-α signaling pathway, Toll-like receptor signaling pathway, cell apoptosis, interleukin-17 signaling pathway, C-type lectin receptor signaling pathway, mitogen-associated protein kinase, etc . Molecular docking results suggested that mairin was the most appropriate natural TNFis. Conclusion: Our findings provide an essential role and basis for further immune-inflammatory studies into the molecular mechanisms of WP and TNFis development in RA.

Abstract:The autoimmune disorder rheumatoid arthritis (RA) causes chronic inflammation and destruction of joints. T cells are a predominant component of the synovial environment in RA, however the functional role of these cells is not yet fully understood. This is in part due to the fact that the balance and importance of the relation of Tregs with T-effector cells in RA is still under investigation. The current treatment regimen for this debilitating disease focuses on controlling symptoms and preventing further joint damage through the use of therapies which affect different areas of the immune system at the synovium. One of the main therapies involves Tumor Necrosis Factor alpha (TNF-α) inhibitors. In the RA immune-environment, TNF-α has been shown to have an influential and extensive but as yet poorly understood effect on Treg function in vivo, and undoubtably an important role in the treatment of RA. Interestingly, the high levels of TNF-α found in RA patients appear to interfere with the mechanisms controlling the suppressive function of Tregs. Relevance for patients: This review focuses on the conflicting literature available regarding the role played by Tregs in RA and the impact of TNF-α and anti-TNF-α therapies on Tregs in this scenario. Individuals suffering from RA can benefit from better insight of the treatment mechanisms of the immunologic processes which occur throughout this disease, as current treatments for RA focus on several different areas of the immune system at the synovial compartment.

Abstract:PURPOSE OF REVIEW: The treatment of rheumatoid arthritis has been revolutionised in recent years with the advent of biologic treatments. The purpose of this review is to outline new treatments that target the inflammatory pathway in rheumatoid arthritis other than tumor necrosis factor-alpha.RECENT FINDINGS: As the use of anti-tumor necrosis factor-alpha treatment has become more widespread, the number of patients in whom this treatment is unsuccessful has also accumulated. Contraindications such as infection and cardiac failure further add to the number of patients who need alternative treatment. A better understanding of the inflammatory pathway in rheumatoid arthritis has led to interest in other therapeutic targets. Promising treatments such as interleukin-6 antagonists (MRA), CTLA4Ig (abatacept), and anti-B cell therapy (rituximab) have already been tested in randomized controlled trials over the past year. Other cytokines have been identified and have been shown to be of benefit in animal models, including interleukin-15, interleukin-17, and interleukin-18, and clinical trials of these agents are currently under way.SUMMARY: For patients with rheumatoid arthritis that does not respond to anti-tumor necrosis factor-alpha treatment, the promising alternatives MRA, abatacept, and rituximab have been tested. It is hoped that these agents will become available shortly.

Abstract:Tumor necrosis factor alpha (TNF-α) was initially recognized as a factor that causes the necrosis of tumors, but it has been recently identified to have additional important functions as a pathological component of autoimmune diseases. TNF-α binds to two different receptors, which initiate signal transduction pathways. These pathways lead to various cellular responses, including cell survival, differentiation, and proliferation. However, the inappropriate or excessive activation of TNF-α signaling is associated with chronic inflammation and can eventually lead to the development of pathological complications such as autoimmune diseases. Understanding of the TNF-α signaling mechanism has been expanded and applied for the treatment of immune diseases, which has resulted in the development of effective therapeutic tools, including TNF-α inhibitors. Currently, clinically approved TNF-α inhibitors have shown noticeable potency in a variety of autoimmune diseases, and novel TNF-α signaling inhibitors are being clinically evaluated. In this review, we briefly introduce the impact of TNF-α signaling on autoimmune diseases and its inhibitors, which are used as therapeutic agents against autoimmune diseases.

Abstract:Objective To investigate the pathogenetic roles of tumor necrosis factor α (TNF α) and vascular endothelium growth factor (VEGF) and the correlation between the two factors in rheumatoid arthritis.Methods CollagenⅡ (CⅡ) and complete Freund′s adjuvant were used to induce experimental arthritis,plasma contents of TNF α and synovium expression of VEGF were assayed simultaneously at different time points,and changes in both new synovium vascularization and arthritis index were observed along the time course.The correlations between those of TNF α and VEGF,arthritis index and those of VEGF and TNF α were studied with linear correlation analysis and rank correlation analysis respectively.Results With the time lasting,synovium new vascular proliferated,synovium thickened,and arthritis index rose gradually as well as those of VEGF and TNF α rose too;Arthritis index correlated with VEGF expression levels ( r =0 535, P 0 05);both arthritis index and TNF α rose,but showed no significant difference ( r =0 371, P 0 05),and those of TNF α and VEGF had a positive correlation ( r =0 893, P 0 01).Conclusion TNF α and VEGF play important roles in RA response and in the cytokine web of new synovium vascularization,both affect and promote those of each other,and may regulate the vicious web circulation.They are the keys of many cytokines that mediate RA pathogenesis,development,matrix invading,and joint deformation.

Abstract:TNF signaling pathway was a valuable target ,and anti-TNF drugs were successfully used to treat autoim-mune and inflammatory diseases .But this therapy abrogate some beneficial TNF signaling ,leading to increased risk of infection and malignancy ,and the onset of new auto-immune diseases .Inhibiting the soluble TNF/TNFR1 axis while saving the benefi-cial transmembrane TNF/TNFR2 signaling untouched was a new approach .Because it inhibited the pathological effects of TNF and reduced the side effects ,and opened the way for the treatment of other diseases in which TNFR2 inhibition was detrimen-tal .The significance of the selective inhibition of TNFR1-mediated signaling pathways for TNF-related diseases was discussed and the mechanism of TNF identification TNFR was clarified ,which might provide new ideas for the design of new drugs .

Abstract:Tumour necrosis factor alpha (TNF-α) is a pleiotropic cytokine synthesised primarily by mononuclear cells; it has a potent pro-inflammatory effect, playing a crucial role in metabolic, immune, and inflammatory diseases. This cytokine has been studied in various biological systems. In bone tissue, TNF-α plays an integral role in skeletal disorders such as osteoporosis, fracture repair and rheumatoid arthritis through its involvement in regulating the balance between osteoblasts and osteoclasts, mediating inflammatory responses, promoting angiogenesis and exacerbating synovial proliferation. The biological effect TNF-α exerts in this context is determined by a combination of the signalling pathway it activates, the type of receptor it binds, and the concentration and duration of exposure. This review summarises the participation and pathophysiological role of TNF-α in osteoporosis, bone damage repair, chronic immunoinflammatory bone disease and spinal cord injury, and discusses its main mechanisms.

Effect of a Recombinant Human Tumor Necrosis Factor Receptor-Fc Fusion Protein on Pro-and Anti-Inflammatory Cytokines in Rheumatoid Arthritis

Abstract:Objective To determine the effect of recombinant human tumor necrosis factor receptor-Fc fusion protein (rhTNFR:Fc) treatment on the level of cytokines in rheumatoid arthritis (RA). Method Thirty patients with RA were treated with rhTNFR:Fc (25 mg, twice a week) as monotherapy. The pro-inflammatory cytokines such as TNF-α, INF-γ, IL-1β, IL-6 and anti-inflammatory cytokines such as IL-10 were measured in RA sera pre-and post-24 weeks of therapy. Results The level of inflammaory cytokines of RA such as IL-6, TNF-α, INF-γ were significantly higher than the healthy controls (P0.01), respectively. The level of IL-1β was higher than the healthy controls, but had no significant difference between RA and control group (P0.05). The level of TNF-α, INF-γ, IL-1β, IL-6 were significantly decreased after treated with rhTNFR:Fc (P0.01). In contrast, IL-10 was remarkably increased (P0.01). The pretreatment TNF-α level correlated to swollen joint counts and swollen joint index. Conclusion rhTNFR:Fc therapy may down-regulate the level of pro-inflammatory cytokines and up-regulate the level of anti-inflammatory cytokines, and there by decrease the Th1/Th2 cytokine ratio.

Abstract:Background Neuropathic pain is a chronic pain state which is usually produced by injured nervous system.Because of its complex mechanisms,there still has been in lack of efficacious drugs to manage.Objective TNF-α is an important pro-inflammatory cytokine which is released early during nerve injury and neuroinflammation. Recently,many studies show that TNF-α which is expressed by activated immune cells in both peripheral nerve and spinal cord plays a critical role in the development of neuropathic pain.Content This article makes a brief review of the role of TNF-α on neuropathic pain,providing a theory basis for anti- TNF-α therapy in alleviating neuropathic pain.Trend As the pathophysiological mechanisms of neuropathic pain are much better understood,TNF-α may be a new target for neuropathic pain therapeutics.
CD40/TNF receptor associated factor 1 expression and NF-κB-dependent proinflammatory gene expression in rheumatoid arthritis

Abstract:Objective To investigatea cellular / molecular mechanism of the CD40 / TRAF1 signalling pathway involved in Rheumatoid arthritis (RA).Methods 16 patients with active RA and 9 patients with Fractures who underwent total knee or hip replacement in The First Affiliated Hospital of Soochow University were included in the study.Synovial tissues (ST) and serum were obtained from each patient.The CD40,TRAF1,NF-κB p65 were detected by ELISA and Immunohistochemistry in serum and tissue respectively.Real time-PCR (RT-PCR) was applied to measure NF-κB-related gene expression.Results CD40 and TRAF1 positive area (％) in RA patients were 28.7±5.4,34.3±4.8 respectively,which were significantly higher (P＜0.05) than Fracture controls (21.2±9.5,21.6±8.7 respectively).The expression of total NF-κB p65,and phospho-NF-κB p65 proteins,as well as NF-κB-related gene expression,including cytokines (TNFα,IL-6),chemokines (MCP-1),and adhesion molecules (ICAM-1) were significantly higher in the ST of RA patients compared to Fracture controls.Conclusion It is thus possible that the CD40/TRAF1 pathway acted as a positive regulator through NF-κB activation and NF-κB-dependent proinflammatory genes in RA.

Abstract:Tumor necrosis factor (TNF) is a central regulator of immunity. Due to its dominant pro-inflammatory effects, drugs that neutralize TNF were developed and are clinically used to treat inflammatory and autoimmune diseases, such as rheumatoid arthritis, inflammatory bowel disease and psoriasis. However, despite their clinical success the use of anti-TNF drugs is limited, in part due to unwanted, severe side effects and in some diseases its use even is contraindicative. With gaining knowledge about the signaling mechanisms of TNF and the differential role of the two TNF receptors (TNFR), alternative therapeutic concepts based on receptor selective intervention have led to the development of novel protein therapeutics targeting TNFR1 with antagonists and TNFR2 with agonists. These antibodies and bio-engineered ligands are currently in preclinical and early clinical stages of development. Preclinical data obtained in different disease models show that selective targeting of TNFRs has therapeutic potential and may be superior to global TNF blockade in several disease indications.
