Diagnosis and Treatment of Bilateral Chronic Subdural Hematoma

Abstract:Objective To point out the clinical features of bilateral chronic subdural hematoma(CSDH)and sum up the experience of diagnosis and treatment of such patients.Methods The clinical manifestations,neuroradiolog- ical findings,therapeutic methods and outcomes of 30 cases of bilateral CSDH were reviewed retrospectively. Results Among the 30 cases,22 patients were male,and 8 were female with average age of 66 years.Headache, mental disturbance and weakness of bilateral lower limbs were the main manifestations.On CT scanning,CSDH had high density in 4 cases,iso-density in 5,low density in 16,and mixed density in 5.Midline shift was showed in 21 cases and no midline shift in 9 cases.All patients were treated with bilateral single burr-hole irrigation and drainage. The mean time of hospitalization was lldays.On discharge,GOS was 5 in 29 cases and 4 in one case.Among these, 23 cases were followed up more than half-year,and no recurrence was found.Conclusion The majority of bilater- al CSDH frequently occurs in elderly patients,and weakness of bilateral lower limbs is one of the main signs.Good recoverv can be obtained bv single burr-hole irrigation and drainage after early diagnosis.
