Abstract:Color Doppler flow imaging (CDFI) was used in the study of 31 patients with 42 focal liver lesions which were smaller than or equal to 3 cm in diameter. The blood flow that entered the lesion's inner and only through its margin was divided into three types: pulsatile flow, constant flow and hepatic vein flow. The result showed that the pulsed Doppler signals were obtained within the lesions from 11 (73.3%) of 15 malignant lesions and from 3 (11.1%) of 27 benign ones. Their differences were significant (P<0.01). When we consider the detection of pulsatile flow signal either within the lesion or at its periphery and the RI more than 0.60 as signs of malignant lesion, the sensitivity, specificity and accuracy were 80.0%, 92.6%, 88.1%, respectively. Our data suggest that CDFI is of important value in differentiating benign from malignant lesions of the liver.

Abstract:PURPOSE:To evaluate micro-flow imaging (MFI) in depicting the vascular architecture of hepatocellular carcinoma (HCC) and other focal liver lesions.MATERIALS AND METHODS:A total of 81 hepatic lesions were enrolled in this study. Each patient underwent CDFI, MFI, and CEUS examinations. The blood flow was first graded into three types (grade 1, 2 and 3) based on its richness with Adeler classification method. The differences in the grade of blood flow in liver tumors were compared between CDFI and MFI. With respect to the presented morphology, the blood flow was further classified into five types (Type I, II, III, IV and V). The morphological differences in blood flow shown by MFI between malignant and benign hepatic tumors were then analyzed.RESULTS:For the total 81 lesions, MFI detected 61 lesion cases (75.31%) with blood flow grade 2 and 3, which obviously outperformed CDFI which detected 28 cases (34.57%) of grade 2 and 3 (χ2 = 35.27, P = 0.000). The MFI also showed that the most common blood flow morphology of HCC is Type-III (21/48, 43.75%) while the hepatic hemangioma (HEM) is mostly presented as Type V (5/10, 50%). Moreover, the grade of blood flow in MFI varied with different pathological subtypes of HCC (χ2 = 5.610, P = 0.018).CONCLUSIONS:Compared with traditional CDFI, MFI reveals more blood vessels in liver lesions with clearer view of blood flow distribution. Besides, MFI technology can demonstrate grade of blood flow for various differentiation stages of malignant liver tumors.

Abstract:To evaluate the value in the diagnosis of focal nodular hyperplasia ( FN H ) by combining micro‐flow imaging ( M FI) and color Doppler flow imaging ( CDFI ) . Methods A total of 32 patients with 32 FNH lesions were enrolled in this study . Each patient underwent CDFI ,M FI ,and contrast‐enhanced ultrasonography ( CEUS ) examinations . T he differences in the grade and characteristic distribution of blood flow in FN H were compared between CDFI and M FI . T he results were further compared with CEUS . Results T here was a significant difference between M FI and CDFI in detecting the blood flow in FN H . T wenty‐six cases ( 81 .3% ) were detected with blood flow of grade Ⅱ and Ⅲ by M FI , however , 18 cases ( 56 .3% ) were detected of grade Ⅱ and Ⅲ by CDFI ( P ＝ 0 .000 ) . A total of 12 ( 37 .5% ) lesions were correctly showed spoke‐w heel arterial flow peculiar by CDFI ,whereas the number increased to 23( 71 .9% ) in combination with M FI ( P ＝ 0 .013 ) . Conclusions In combination with M FI , CDFI can reveal more blood flow and detect more sensitively in FN H ,w hich helps to diagnose FN H . To a certain degree ,it can be applied to reduce the use of CEUS .
Color Doppler Flow Imaging of Liver Cancer by Interventional Treatment with Sonogrphic Guidance

Abstract:Color Doppler sonographic findings on 40 lesions in 35 patients with liver cancer were e-valuated by percutaneous intratumoral injection treatment with sonographic gridance in recently two years.Diagnosis of liver dancer were confirmed by biopsy before treatment.Color Doppler imaging showed flow in 32(80%)of 40 lesions before treatment.In this group,blood flow reduced in 14 pa-tients and disappeared in 1 3 patients respectively after treatment.Reduction or disappearance of the blood flow corresponds to reduction of the tumor increase of echogenicity and decrease of AFP levels,So the changes of the blood flow can be considered as signs of favourable prognosis and vice versa,Histopathologic examination,performed in 9 patients,showed that the tumor was completely necrotic and fibrosis in 8 patients.These 8 necrotic tumors were not visible on color Doppler flow imaging after treatment.We conclude that color Doppler sonography is useful for monitoring blood flow of liver can-cer before and after treatment,and for imageing guidance during treatment.\r\n\r\nKey words: \r\ncolor Doppler flow imaging hepatic neoplasm interventional treatment

Abstract:The purpose of the study was to evaluate the vascular architecture of focal liver lesions with the newly released non-contrast micro-flow imaging (MFI) technique. Eighty patients with 80 hepatic tumors were enrolled in this study. The richness of displayed blood flow was first compared with color Doppler flow imaging using grades according to Adler's method. The results indicated that MFI outperformed color Doppler flow imaging with a statistically significant difference (p <0.0001). With the enhanced position in arterial phase of contrast-enhanced ultrasound imaging as the ground truth, the displayed blood flow distribution within tumors in MFI was further evaluated. The coincidence rate between MFI and contrast-enhanced ultrasound was 73.75% (59/80), which was higher than that of color Doppler flow imaging (52.5%, 42/80) (p < 0.0001). Moreover, for the five cases of focal nodular hyperplasia, MFI clearly revealed the spoke-wheel or radiating intra-tumoral vasculature, a finding specific to the diagnosis of focal nodular hyperplasia.
Diagnosis of Cystic Renal Cell Carcinoma by Color Doppler Flow Imaging

Abstract:Purpose To study the value of diagnosing cystic renal cell carcinoma by color Doppler flow imaging (CDFI). Methods Twenty four cases of cystic renal carcinoma were examined by CDFI and confirmed by operation and pathology. Results Twenty four cases of cystic renal carcinoma were divided into cystic pattern (12), septum pattern (8), nodular pattern(4). Color flow was showed in solid components or on septums in 22 cases and the peak systolic velocity was less than 0.40m/s in 15 patients. Resistance index (RI) ranged from 0.40 to 0.90 in all patients. Conclusion CDFI can play an important role to display intratumoral color flow and increase the diagnostic ability.

Abstract:Objective To investigate the diagnostic value of color Doppler ultrasound for peripheral intrahepatic cholangiocarcinoma (ICC).Methods Ninety- five patients with ICC which were confirmed histopathologically were retrospectively studied.The features on color Doppler ultrasound were analyzed.Results There were 102 tumors which ranged from 20 mm× 20 mm to 130 mm× 100 mm in size.On gray-scale ultrasonic imaging,a large proportion of the tumors presented with inhomogeneous echo (n=75,73.5％), indefinite boundaries (n =70, 68.6％),and irregular shape (n=55,53.9％).On color Doppler ultrasonic imaging,most tumors had flow signals (75,73.5％) mainly in the shape of areatus,short- line and color-dot.Most flow signals had a resistant index greater than 0.6.Of the 102 tumors,thirty-seven (36.3％) were observed to produce streak hyperecho,twentynine (28.4％) slight attenuation in the rear and 22 (21.6％) "unbilical depression".The coincidence rate with ultrasound was 84.2％ (80/95). Conclusions The features of peripheral intrahepatic cholangiocarcinoma on color Doppler ultrasound are characteristic.To be able to appreciate these features is helpful to improve on the accuracy rate of ultrasound.

Abstract:Objective To investigate the contrast enhanced ultrasound (CEUS) filling features of intrahepatic cholangiocarcinoma (ICC)with pathologic basis for increasing further understanding the ultrasonic diagnosis accuracy.Methods The carcinomas in 11 patiens were evaluated using conventional ultrasonography and CEUS.The characterstics of the ultrasound and enhancement duration were recorded.Results Most of all the cases presented hypoechoic in conventional ultrasonography and showed hypovascularity patterns on CDFI.On CEUS,the masses appeared heterogeneous,thick rim-like hyperechoic or feather-like enhancement in the arterial phase.In the portal phase,the contrast agent washed out fast,and then appeared hypoechoic enhancement and maldistribution in the delay phase.Conclusions The CEUS filling pattern is dependent on the patholopy of ICC.
Diagnosis in Focal Hepatic Lesions with Contrast- enhanced Color Doppler Sonography

Abstract:Purpose: To investigate the diagnosis in focal hepatic lesions with contrast-enhanced color Doppler sonography. Methods: One hundred and twelve patients underwent examination with CDFI before and after intravenous injection of Levovist. Results: Conventional CDFI showed intratumoral arterial flow in 15 (44.1%) benign hepatic lesions and 53(67.9%) in malignant ones. After contrast, 19(55.9%) benign lesions and 73 (93.6%) malignant lesions exhibited intratumoral arterial flow. Liver tumors of different pathological types and their own appearence of blood flow on contrast sonography. Time-intensity changes in malignant tumors were stronger than in benign tumors. The accuracy rate of diagnosis was 67.8% (76/112) before contrast and 95.5% (107/112) after contrast with a distinguished improvement. Conclusions: Enhanced CDFI can improve evaluating the vascularity of focal hepatic lesions and aid in differential diagnosis between benign and malignant lesions.

Abstract:ObjectivesTo evaluate the value of simultaneous display of contrast‐enhanced ultrasound and micro‐flow imaging technology (CEUS‐MFI) in intra‐tumoral vessel detection and hepatic tumor diagnosis.MethodsA total of 82 patients with 82 focal liver lesions were enrolled in this study. Each patient received ultrasound exams including color Doppler flow imaging (CDFI), micro‐flow imaging (MFI), contrast‐enhanced ultrasound (CEUS), and CEUS‐MFI with a Philips EPIQ7 ultrasound imaging system. The intra‐tumoral vessels detected by CDFI, MFI, and CEUS‐MFI were compared, respectively. The accuracy and confidence of using CEUS and CEUS‐MFI in diagnosing hepatic tumors were also compared.ResultsCEUS‐MFI was capable of detecting more hepatic intra‐tumoral vessels than MFI (P = .000) and CDFI (P = .000). Compared with CEUS, CEUS‐MFI improved the diagnostic accuracy of hepatic lesions (P = .009). Particularly, among the correctly diagnosed hepatic lesions, the number of cases where radiologists diagnosed with great confidence was increased from 88.4% (61/69) with CEUS only to 92.4% (73/79) with CEUS‐MFI (P = .041).ConclusionsCEUS‐MFI is sensitive in detecting hepatic intra‐tumoral vessels and can improve the accuracy and confidence of radiologists in diagnosing hepatic lesions.
Ultrasound color flow imaging method based on the zero-phase filter

Abstract:To improve the quality of the ultrasound color flow imaging (CFI), the conventional procedure of the CFI is modified and a high performance zero-phase filter is added to low-pass filter ultrasonic echoes so that the noise of velocity estimation can be reduced. This method is validated by computer simulated blood flow signals. Results show that this method can achieve CFI images with more smoothness and less errors than the conventional approach when the axial resolution is relatively high (<0.6 mm). Therefore, the image quality of the CFI is expected to be improved efficiently.

Abstract:Objective To explore the value of color Doppler flow imaging (CDFI) and contrast-enhanced ultrasound (CEUS) in detecting hepatic artery complications in patients after liver transplantation and to compare the usefulness of CDFI and CEUS.Methods A total of 267 CDFI examinations for 158 patients after liver transplantation were studied.Data were collected with attention to the patency,peak velocity,acceleration time and resistance index of intra- and extrahepatic artery,as well as turbulence of extrahepatic artery.Thirty-three patients suspected by CDFI underwent CEUS with SonoVue and Contrast Tuned Imaging (CnTI) to observe hepatic artery and perfusion of liver parenchyma.Results In 19 patients,17 patients were with hepatic artery stenosis (HAS) and two with hepatic artery occlusions (HAO) confirmed by DSA and/or CT angiography (CTA).Vascular parameters of 17 patients with HAS were abnormal with sensitivity of 100%,but CDFI could not show the stenosis position and degree.CDFI did not demonstrate flow signal in the 2 patients with HAO,and could not make a definite diagnosis.CEUS confirmed not only HAO,but also displayed the stenosis position and degree.The accordance rates for HAO,HAS detected by CEUS compared to DSA and/or CTA were 100% (2/2),94.1% (16/17) and 88.2% (15/17),respectively.Conclusions CDFI complements CEUS mutually,which may improve the diagnostic accuracy of hepatic artery complications after liver transplantation.

Abstract:Objective:To investigate the value of Color Doppler Flow Imaging(CDFI) technique in breast neoplasms diagnosis.Methods:The Cross-sectional echocardiography and CDFI scan were performed on 136 breast neoplasms among 114 cases,the high frequency probe of Philips iU22 ultrasonic diagnostic equipment was used.The blood stream parameter was examined and recorded to compare the variability between malignant and benign tumors.Results:Significant differences(P0.05) of susceptibility,specificity and positive predictive value between malignant and benign breast tumor characteristic ultrasonograms had been revealed.Interclass blood stream detection rate and fractionation were contrasted to have a statistical significance(P0.05).The hemodynamic parameters such as PSV,PI,RI were higher in malignant group,the differences were significant(P0.01).Conclusion:The analysis of cross-sectional echocardiography,CDFI blood stream fractionation and hemodynamic(PSV,RI,PI)parameters are of practical and clinical values in breast neoplasm diagnosis.

Abstract:Objective To evaluate the value of color Doppler flow imaging (CDFI) for collateral circulations casued by portal hypertension. Methods Serial collateral circulations and change of hemodynamics of portal flow were detected by CDFI with gastric-filling. Results Various collateral circulations were detected by CDFI in 95 of 430 patients of liver cirrhosis with portal hypertension, including hepatofugal and pulsatile blood flow in the portal vein, reopened umbilical veins, varices in car-diaca and fundus of the stomach, and varices in gastrica left veins, etc. Red and blue blood flows and continuous venous spectrum with low velocity were detected in the collateral veins by CDFI. Conclusions CDFI can detect the blood flow direction、 velocity and property of collateral circulations non-invasively. CDFI is an important method for the diagnosis and management of liver cirrhosis and portal hypertension.

Color doppler ultrasound diagnosis in the axillary lymphatic metastasis of breast cancer

Abstract:Objective To evaluate the diagnostic value of color doppler flow imaging (CDFI) in the axillary lymphatic metastasis of breast cancer. Methods With 2-D and color doppler ultrasonography, the amount, form, size, internal echo and hemodynamic index of axillary metastatic lymph nodes in 66 preoperative patients with breast cancer were observed. Results Conjunctive lymph nodes were the main characteristic of nodal metastases. In the metastasis lymph nodes, more color signals were displayed and most of signals were around the nodes. Conclusions 2-D and color doppler ultrasonography is an effective and simple way for the diagnosis in the axillary lymphatic metastasis of breast cancer.

Abstract:The incidence of breast cancer ranks first among female malignant tumor. With the increase of the sensitivity of color Doppler ultrasound blood flow, the blood flow distribution in and around the tumor can be clearly displayed, and the analysis of hemodynamic parameters is provided, which provides convenience for the study of tumor blood flow characteristics. Studies have shown that tumor cells can secrete a substance called angiogenesis factor, which makes the tumor site form a rich vascular network to promote tumor growth and metastasis. The tumor has many new blood vessels, abnormal structure, thin wall, lack of muscle layer, and is prone to form arteriovenous rash. These characteristics provide a pathological basis for color Doppler flow imaging (CDFI) for the diagnosis of breast cancer. This article discusses the role of two-dimensional sonographic features in the differential diagnosis of benign and malignant breast masses, CDFI was used to study the blood flow distribution and hemodynamic characteristics in benign and malignant breast masses; explore the value of blood flow characteristics and blood flow parameters in the differential diagnosis of breast masses. The experimental results show that the detection rate of blood flow signals and the classification of blood flow signals in the malignant group are higher than those in the benign group, mainly level II and III blood flow, and the irregular branched blood flow is more common, especially when the tumor appears penetrating blood flow supports the diagnosis of malignancy. PSV, RI and PI have a certain differential meaning in the diagnosis of benign and malignant breast masses. PSV, RI and PI of malignant masses are higher than benign masses. For tumors without obvious necrosis, the larger the tumor diameter, the richer the blood flow and the higher the blood flow grade is. The malignant tumors have more blood flow than the benign ones.

Abstract:Objective To discuss the application value of color Doppler flow imaging(CDFI)on evaluating vessel of living donor liver transplantation(LDLT) during the pre-operation,intra-operation and post-operation.Methods Six patients with LDLT were examined with CDFI during pre-operation,intra-operation and post-operation.The findings of CDFI were analyzed retrospectively.Results One of hepatic artery was thinner and one of branch of hepatic vein was bigger on living donor liver with CDFI before operation.No obvious abnormality was found on CDFI in operation.CDFI displayed thrombosis in bridging vessel after operation.Conclusion It is important and useful to assess the vessel and blood supply of donor liver with CDFI during the pre-operation,intra-operation and post-operation.

Abstract:OBJECTIVE:The purpose of this study was to evaluate micro flow imaging (MFI) in depicting the vascular architecture of hepatocellular carcinoma (HCC) and the correlation between pathologic differentiation and the intratumoral vascular architecture pattern.METHODS:Micro flow imaging and contrast harmonic imaging (CHI) were performed in 37 patients with HCC. A sulfur hexafluoride-filled microbubble contrast agent was used. The enhancement level and intratumoral vessels were evaluated on CHI. The vascular architecture of each tumor was evaluated on MFI. Pathologic differentiation of the tumors was classified according to the Edmondson grading system.RESULTS:All 37 HCCs showed hyperenhancement in the arterial phase and hypoenhancement in the portal and late phases on CHI. Intratumoral vessels in the arterial phase were visualized in 20 (54.1%) HCCs. On MFI, the vascular architecture in all lesions was clearly delineated and categorized into 3 patterns: cotton, shrubbery, and deadwood, identified in 12 (32.4%), 22 (59.5%), and 3 (8.1%) of the tumors evaluated, respectively. A chi(2) test showed that pathologic differentiation significantly correlated to the vascular pattern (P = .006). Three (75%) of 4 Edmondson grade I HCCs showed the cotton pattern; 18 (75.0%) of 24 Edmondson grade II HCCs showed the shrubbery pattern; and the deadwood pattern was shown only in Edmondson grade III and IV HCCs.CONCLUSIONS:The MFI technique is more effective in depicting the intratumoral vascular architecture. The vascular architecture pattern correlates with pathologic differentiation of HCC.
