Relationship Between Blood Concentration of Mycophenolate Mofetil and Adverse Drug Reaction(adr) in Lupusnephritis

Abstract:Objective:To retrospective assess the relationship between blood concentration of mycophenolate mofetil(MMF)and adverse drug reaction(ADR)in lupus nephritis(LN).Methodology:One hundred and sixty two LN patients(143 female and 19 male)treated with MMF were investigated,with an average age of(28.9±12.6)years old.All patients received methylprednisolone(MP)pulse therapy,oral prednisone and MMF therapy,with an initial MMF dose of 1.0～2.0 g/d.The plasma concentration of total mycophenolic acid(MPA)was detected(HPLC)and MPA-AUC was calculated according to MPA-C0、C0.5 and C2.The relationship between MPA-AUC,C0,C0.5,C2 and ADR were analyzed.Results:(1)MMF dose corrected plasma total MPA-AUC varied significantly among patients 9.18～137 [mean of(33.4±17.6)(mg·h/L·g)].(2)Within the 12 month of MMF therapy,a total of 40(24.7 %)ADR occurred in 32(19.8%)patients.Infection occurred in 26 cases(16.0%),including pneumonia in 10 cases(6.17%)and herpes zoster in 11 cases(6.79%).87.5% of ADR and 80.8% of infection occurred within 3 months of MMF treatment.The initial MPA-AUC(detected 1～2 week after intake of MMF)(52.4±16.6 vs 39.8±14.7,P0.01)and MPA-C0[(2.5±1.5)mg/L vs(1.7±1.0)mg/L,P0.05] was significantly higher in patients with ADR.The incidence of ADR and infection was significantly higher in the initial MPA-AUC40 mg·h/L group(33.8% vs 18.2%,22.1% vs 10.6%,P0.01 respectively),and also markedly elevated in patients with MPA-AUC40 mg·h/L during MMF treatment(18.0% vs 4.0% and 12.4% vs 2.0%,P0.01 respectively).There were 90% of patients before onset of pneumonia whose MPA-AUC level was higher than 40 mgh/L.Furthermore,MPA-C0 at the initial and during MMF treatment were both significantly higher when complicated with ADR and infection,but MPA-C0.5h and MPA-C2 was not differed.Conclusion:MPA-AUC and C0 at the initial and during MMF treatment were related with ADR in LN patients.The incidence of ADR increased when MPA-AUC40 mg·h/L.We suggest that the initial MPA-AUC be less than 40 mg·h/L in LN,but perspective study should needed.
