Reverse facial artery-submental artery mandibular osteomuscular flap for reconstruction of maxillary Brown Class 2A defects

Abstract:PURPOSE:To assess the reliability of the reverse facial artery-submental artery mandibular osteomuscular flap for reconstruction of maxillary defects.METHODS:Maxillary defects following benign tumor ablation of eight patients were repaired with reverse facial artery-submental artery mandibular osteomuscular flaps.The patients' age ranged from 16 to 33 years.Five of them were male and the other three were female.Eight cases of benign tumor included three cases of maxillary odontogenic myxoma,two cases of maxillary fibrous dysplasia and ameloblastoma respectively and one case of chondromyxoid fibroma.The defects were classified as Brown Class 2A.RESULTS:Primary reconstruction of the maxilla was carried out using the reverse facial artery-submental artery mandibular osteomuscular flap.No flap necrosis occurred.Dental reconstruction was successful in all patients.Proper esthetics and complete functional restoration were o btained,and there was no donor-site complication.The patients were followed up for 12 to 24 months,with an average of 18.6 months.No recurrence was encountered.CONCLUSION:The reverse facial artery-submental artery mandibular osteomuscular flap is safe,quick,and easy to elevate.The flap can be used reliably for reconstructing maxillary defects.
