Diagnosis and Treatment of Severe Pulmonary Infection Accompanied in Patients with Chronic Glomerular Disease on Glucocorticoid and Immunosuppressive Therapy

Abstract:Objective:To observe the clinical characteristics and treatment of severe pulmonary infection during glucocorticoid and immunosuppressive therapy in patients with chronic glomerular disease.Methods:29 patients with chronic glomerular diseases with severe pulmonary infection after glucocorticoid and immunosuppressive therapy from 2004 to 2008 in our department were enrolled.Their clinical datas were retrospectively studied and analyzed.Results:Among the 29 patients,25 were male and 4 female,with age of 23 to 62(42.9±16.3)years.Pulmonary infection occurred after less than 3 months treatment with glucocorticoid and immunosuppressant in 20(69.0%)patients,5(17.2%)patients were from 3 to 6 months,4(13.8%)patients were after 6 months,and the mean duration was 4.47±5.16 months.The clinical symptoms were not specific.Fever was the main symptom at onset of infection(86.2%).Cough was not serious(51%),manifested as mild dry cough.9 patients had chest distress and polypnea,progressed rapidly to respiratory failure and needed mechanical ventilation.On admission,Routine chest X-ray examination showed no serious lesions in all patients,but chest high-resolution CT might show significant lesions.With the progress of disease,Chest X-ray and HRCT of all patients appearanced diffuse lesions.Pathogens were detected in all patients with positive results for 18 times only in 10 patients.7 cases(38.9%)were bacteriums,6 cases(33.3%)were fungouses and 5 cases(27.8%)were viruses.All of the patients stopped immunosuppressive agents or taped,received active anti-infection treatment,strengthened supportive treatment,correctted water-electrolyte acid-base balance disorder and used mechanical ventilation after their infection were confirmed.However,the cure rate was low.13 patients died with death rate of 44.8%.Conclusion:Chronic glomerular diseases with pulmonary infection after glucocorticoid and immunosuppressants therapy mostly occurred in three months.The keys to remedy success were early and combined of anti-bacterial,anti-virus,anti-fungal and anti-mycoplasma therapy,adjustment of immunotherapy in time and mechanical ventilation at the right moment.
