Abstract:Objective: To evaluate the efficacy and safety of surgical resection plus radiofrequency ablation (SR-RFA) for multifocal hepatocellular carcinoma (HCC) with 2 or 3 nodules compared with surgical resection (SR). Method: We retrospectively evaluated 824 consecutive HCC patients (SR, n = 754; SR-RFA, n = 70) from January 2009 to December 2015 and performed propensity score matching (PSM) to adjust for patient imbalances at a ratio of 1:4. Results: At baseline, patients in the SR-RFA group had a smaller tumour size and worse liver function (including more ascites, a higher total bilirubin level, and a longer prothrombin time) than patients in the SR group. However, the two groups had similar overall survival (OS) and recurrence-free survival (RFS) rates (P = 0.209 and P = 0.332). The local recurrence rate of the SR-RFA group was significantly higher than that of the SR group (25.71% and 14.32%, P = 0.011). More patients in the SR-RFA group had postoperative complications (P = 0.003). In the propensity model, there was no intergroup difference in OS or RFS (P = 0.229 and P = 0.311, respectively). Conclusion: SR-RFA provides a similar long-term survival to that on SR in HCC patients with 2 or 3 nodules, and its application should be carefully considered.
Surgical Resection Plus Radiofrequency Ablation for the Treatment of Multifocal Hepatocellular Carcinoma.

Abstract:BackgroundTo compare the efficacy and safety of surgical resection plus radiofrequency ablation (SR-RFA) with transarterial chemoembolization (TACE) in patients with multifocal hepatocellular carcinoma (HCC) beyond the Milan criteria.MethodsFrom 2009 to 2015, a total of 469 consecutive patients with multifocal HCC beyond the Milan criteria (≤5 nodules) treated with SR-RFA (n=59) or TACE (n=410) were analyzed. Their overall survival (OS) rate was compared through propensity score matching (PSM) analysis at a ratio of 1:2, and 141 patients were identified (SR-RFA, n=47; TACE, n=94). Subgroup analysis was conducted according to factors associated with treatment options.ResultsBefore PSM, the 1-, 2- and 3-year OS rates were 81.5%, 68.3% and 64.3% in the SR-RFA group and 58.7%, 35.5% and 24.4% in the TACE group, respectively (all P<0.001). After PSM, the 1-, 2- and 3-year OS rates in the SR-RFA group were 81.8%, 68.7% and 63.4%, whereas those in the TACE group were 59.3%, 36.1% and 19.4%, respectively (all P<0.001). Subgroup analysis also revealed a survival advantage of SR-RFA over TACE in each subgroup. The 30-day mortality rate was 1.22% in the TACE group and nil in the SR-RFA group (P=0.861).ConclusionsSR-RFA provides better a long-term survival rate than TACE for patients with multifocal HCC beyond the Milan criteria. SR-RFA may serve as an alternative treatment for patients with multifocal HCC in a selected patient population.

Abstract:Radiofrequency ablation (RFA) has been playing an important role in the treatment of hepatocellular carcinoma (HCC). It is minimally invasive, easily repeatable and potentially curative. Many centers now accept it to be the best therapeutic choice for patients with early-stage HCC when liver transplantation or surgical resection is not suitable. In this article, we reviewed the indications, techniques , and clinical results of RFA in the treatment of HCC.Suitable patient selection, good pre-treatment planning,complete ablation, and careful post-treatment follow up are important. Techniques, such as artificial pleural effusion and ascites, contrast-enhanced ultrasonography-guided RFA,laparoscopic or open surgical approaches have reduced complications and expanded the indications for RFA.
A randomized controlled trial of radiofrequency ablation and surgical resection in the treatment of small hepatocellular carcinoma.

Abstract:The aim of this study was to compare the efficacy of radiofrequency ablation (RFA) with surgical resection (RES) in the treatment of small hepatocellular carcinoma (HCC).A total of 168 patients with small HCC with nodular diameters of less than 4 cm and up to two nodules were randomly divided into RES (n=84) and RFA groups (n=84). Outcomes were carefully monitored and evaluated during the 3-year follow-up period.The 1-, 2-, and 3-year survival rates for the RES and RFA groups were 96.0%, 87.6%, 74.8% and 93.1%, 83.1%, 67.2%, respectively. The corresponding recurrence-free survival rates for the two groups were 90.6%, 76.7%, 61.1% and 86.2%, 66.6%, 49.6%, respectively. There were no statistically significant differences between the two groups in overall survival rate (p=0.342) or recurrence-free survival rate (p=0.122). Multivariate analysis demonstrated that the independent risk factors associated with survival were multiple occurrences of tumors at different hepatic locations (relative risk of 2.696; 95% CI: 1.189-6.117; p=0.018) and preoperative indocyanine green retention rate at 15 min (ICG-15) (relative risk of 3.853; 95% CI: 1.647-9.015; p=0.002).In patients with small hepatocellular carcinomas, percutaneous RFA may provide therapeutic effects similar to those of RES. However, percutaneous RFA is more likely to be incomplete for the treatment of small HCCs located at specific sites of the liver, and open or laparoscopic surgery may be the better choice.

Abstract:The treatment of hepatocellular carcinoma (HCC) is a significant challenge. Although radiofrequency ablation (RFA) has emerged as a popular therapeutic option for patients with resectable HCC, whether it can achieve comparable survival outcomes compared with surgical resection (RES) remains unclear. The aim of the present study was to conduct a meta‑analysis to assess the survival outcomes of RFA vs. RES in patients with early resectable HCC tumors. A Medline, Embase, and Cochrane Library search was performed for data published between January 2000 and February 2018. A meta‑analysis of the efficacy of RFA compared with RES for HCC was subsequently performed, with particular emphasis on overall survival and disease‑free survival (DFS) rates. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated using the random‑effects model. In the present study, a total of 13,147 patients with HCC were included; of which, 6,727 were treated with RFA and 6,420 were treated with RES. The overall survival rates (OR1‑year, 0.757, 95% CI, 0.578‑0.989; OR3‑year, 0.530, 95% CI, 0.401‑0.700; OR5‑year, 0.566, 95% CI, 0.423‑0.758) and the DRS rates (OR1‑year, 0.569, 95% CI, 0.456‑0.711; OR3‑year, 0.418, 95% CI, 0.267‑0.653; OR5‑year, 0.374, 95% CI, 0.231‑0.606) of RES were significantly higher than those of RFA. The results indicate that RES is superior to RFA for promoting the survival of selected patients with resectable HCC. However, future randomized controlled trials are required to investigate the specific relevance of these modalities in the treatment of HCC.
A comparison of efficacy between radiofrequency ablation and surgical resection in treatment of small primary hepatocellular carcinoma

Abstract:Objective To investigate the efficacy between radiofrequency ablation and surgical resection in treatment of small primary hepatocellular carcinoma(HCC).Methods Sixty patients with small primary HCC were divided into two groups with 30 cases each.The patients in group A were treated with local radiofrequency ablation and those in group B with surgical resection.Conventional chemothrapy was used in both groups after the operation.The 5-year recurrence rate of HCC and survival rate were compared.Results The 5-year recurrence rate and survival rate were 13.3%% and 93.3% respectively,in group A,which were 16.7% and 90.0% in group B.There was no significant difference in recurrence and survival rates betwenn two groups(P0.05).Conclusion The local radiofrequency ablation therapy has the same efficacy as surgical resection and is a theraputic method with less trauma in treating small primary HCC.

Abstract:The study by me and my colleagues our study in the May 2015 issue of Journal of Surgical Research compared the clinical efficacy and safety of radiofrequency ablation (RFA) versus surgical resection (SR) for intrahepatic hepatocellular carcinoma (HCC) recurrence using meta-analysis techniques [1]. We found that the 1-, 3-, 5-year overall survival rate and procedure-related mortality rate were similar in patients treated with RFA or SR. Meanwhile, SR was associated with significantly higher 1-, 3-, 5-year recurrencefree survival rate and procedure-related morbidity rate compared with RFA. Then, we concluded that RFA seems to be as effective as SR for the treatment of intrahepatic HCC recurrence owing to comparable overall survival benefits though RFA was associated with lower recurrence-free survival, and the advantages of being less invasive, highly target-selective and repeatable may render RFA a preferred treatment option for selected patients [2, 3]. Herein, we would like to raise the following comments: In our study, most of the included studies were performed in Asian countries where there is high prevalence of hepatitis B virus (HBV) related HCC; only one study was performed in non-Asian county country. It has been suggested that early HCC recurrence is generally considered as the dissemination of initial HCCs which were undetected by imaging modalities [2, 3]. Also, recent studies have demonstrated that high HBV load and HBV mutation promote the growth and metastasis of HCC, and the hepatitis B virus X protein promotes the invasive and metastatic potential of HCC [4, 5]. These viral factors may actually accelerate the recurrence of HCC after RFA rather than after surgical resection as due to the RFA’s insufficient elimination of the first recurrent HCC ablation feature of RFA in certain conditions compared to surgical resection. Therefore, whether the results, especially the recurrence-free survival outcomes, could be applicable to non-Asian populations remains to be determined. In addition, RFA can be performed via percutaneous, laparoscopic or open approaches, and percutaneous RFA is the most less least invasive approach and significantly improves the rate of censored patients compared to other approaches. However, in our study, we did not compare each of these three approaches with surgical resection for intrahepatic hepatocellular carcinoma recurrence. Therefore, the clinical efficacy and safety of different approaches of RFA versus surgical resection, especially RFA via percutaneous versus surgical resection, should be further researched.

Abstract:Background: To compare the clinical efficacy and safety of radiofrequency ablation (RFA) versus surgical resection (SR) for intrahepatic hepatocellular carcinoma (HCC) recurrence by meta-analytical techniques.Methods: Literature documenting a comparison of RFA and SR for intrahepatic HCC recurrence was identified by searching PubMed, Embase, Cochrane Library, and Web of Science databases, for those from inception to July 2014 with no limits. The heterogeneity was tested by the Cochrane Q statistic; the pooled estimates were measured using either fixed or random effect model. Furthermore, subgroup and sensitivity analyses were conducted to explore heterogeneity between studies and to assess the efficacy of different studies.Results: Seven studies were included with a total of 718 patients (359 treated with RFA and 359 treated with SR). Our meta-analysis showed that the 1-, 3-, and 5-y overall survival rate and procedure-related mortality rate were similar in patients treated with RFA or SR. Meanwhile, SR was associated with significantly higher 1-, 3-, and 5-y re-recurrence-free survival rate and procedure-related morbidity rate compared with RFA. In the subgroup analysis of patients in China, the results concerning overall and re-recurrence-free survival were similar to the outcomes of the meta-analysis without regional restriction. In the subgroup analysis of intrahepatic recurrent HCC <= 3 cm, the 1-, 3-, and 5-y overall survival rate did not differ significantly in the comparison of RFA and SR.Conclusions: Although RFA was associated with lower re-recurrence-free survival, it seems to be as effective as SR for the treatment of intrahepatic HCC recurrence owing to comparable overall survival benefits. Abstract:Background and AimTo determine and compare the adverse events and long-term effectiveness for patients with small hepatocellular carcinoma (HCC) (3cm) treated by percutaneous radiofrequency ablation (RFA) or hepatectomy.MethodsSmall HCC from 120 patients were randomized into either percutaneous RFA therapy or hepatectomy group, and the effectiveness and complications of two treatment modalities were analyzed. The complications of post-RFA or hepatectomy, the complete treatment rate, treatment-related mortality, and disease-free and overall survival rate were followed up and conducted.ResultsIn patients with small HCC, complete remission rates were achieved in 95% and 96.7% in the percutaneus RFA and hepatectomy groups, respectively (P>0.05). Hepatic function at day-7 status post-treatment, including albumin and bilirubin levels, were significantly worse in the hepatectomy group (P<0.01). Compared with the RFA group, the incidence of postoperative complications (27.5% vs 5.0%) and hospital stay (11.83.1 vs 4.3 +/- 1.5) were significantly higher in the hepatectomy group (P<0.01). After a mean follow-up of 40 months, 22 patients (36.6%) in the RFA group and 21 patients (35.0%) in the hepatectomy group developed a recurrence (P>0.05). There was no significant difference of the disease-free and overall survival rates at 1, 2, and 3 years between the RFA group and the surgical hepatectomy group (P=0.443 and P=0.207, respectively).ConclusionsIn patients with small HCC, percutaneous RFA showed similar local control and long-term survival compared with hepatectomy. Importantly, percutaneous RFA are accompanied with a lower complication rate and shorter hospital stay day.

Abstract:AIM:To evaluate survival and recurrence after radiofrequency ablation (RFA) for the treatment of small hepatocellular carcinoma (HCC) using a meta-analysis. METHODS:Literature on RFA vs surgical resection for the treatment of small HCC published between January 1990 and December 2008 was retrieved.A metaanalysis was conducted to estimate pooled survival and recurrence ratios.A fixed or random effect model was established to collect the data. RESULTS:The differences in overall survival at 1-year, 3-years and at end of follow-up were not statistically significant between the RFA and surgery groups (P > 0.05).There were no differences in 1-year and 3-year recurrences between the RFA and surgery groups (P > 0.05).However, recurrence in the RFA group was lower than that in the surgery group up to the end of follow-up (P = 0.03).Survival was not significantly different.There was a significant difference in recurrences at the end of follow-up after RFA compared with surgical resection.CONCLUSION: RFA did not decrease the number of overall recurrences, and had no effect on survival when compared with surgical resection in a selected group of patients.

Abstract:ABSTRACT:Stereotactic body radiotherapy (SBRT) is a novel noninvasive treatment for unresectable hepatocellular carcinoma (HCC). Whether its efficacy is comparable to radiofrequency ablation (RFA), a recommended therapy for unresectable HCC, is unknown. The present study aims to compare the clinical outcome between SBRT and RFA for patients with unresectable HCC.The clinical data of 60 patients with unresectable HCC from January 2018 to January 2021 were retrospectively reviewed. There were 22 cases treated by SBRT and 38 cases by RFA. The short-term and long-term clinical outcomes were compared.There was no significant difference in the baseline demographic characteristics between two groups. The complete remission rate at 3 months was comparable between SBRT group (81.8%) and RFA group (89.4%). Local tumor control rate was also similar between two groups (90.9% vs. 94.7%). There was no severe complication (grade IIIa or above) in both groups. The 1-year and 2-year overall survival rates were 88.2% and 85.7% in SBRT group and 100% and 75% in RFA group, respectively. There was no statistical significant difference between groups (P = .576).SBRT can achieve similar short and long-term clinical outcome as RFA for unresectable HCC. Future prospective clinical study is needed to justify its role in patients with HCC.

Abstract:Local ablation technologies, such as radiofrequency ablation (RFA), microwave ablation (MWA) and cryoablation, have become a standard treatment option for hepatocellular carcinoma (HCC) less than 5 cm in size, particularly in individuals who are not candidates for hepatectomy. Except for equivalent prognosis and efficiency, RFA has various advantages over surgical excision, including a lower rate of complications, a cheaper cost, more normal tissue preservation, and a shorter hospital stay. However, the rate of tumor recurrence and/or distant metastasis after RFA therapy is still high. RFA has been widely employed in multiple cancers, large cancer, and lesion identified at “high-risk” sites in recent years, with the advancement of ablation types and operating techniques, particularly the combined use of many technologies. The real value of RFA technology has been more fully reflected. We will examine the status, progress, and problems of RFA in the treatment of HCC in this review.
Radiofrequency ablation versus reresection in treating recurrent hepatocellular carcinoma: a meta-analysis.

Abstract:Treatment for recurrent hepatocellular carcinoma (RHCC) remains controversial. This study tried to compare survival benefits between radiofrequency ablation (RFA) and reresection for RHCC patients following curative surgical treatments.Databases were searched for comparative studies published from 2008 to 2014 on RFA versus reresection in treating RHCC. Meta-analysis was performed using a random or fixed-effect model to compare the overall survivals (OSs) and disease-free survivals (DFSs) between RFA and reresection. Begg funnel plot and Egger test were performed to assess the publication bias.Six retrospective comparative studies fulfilled our criteria and were included. For patients with RHCC, RFA was equivalent to reresection in 1-year OSs (odds ratio [OR] 0.86; 95% confidence interval [CI], 0.50-1.49; P = 0.587), 3-year OSs (OR 0.91; 95% CI, 0.64-1.28; P = 0.581), and 5-year OSs (OR 0.97; 95% CI, 0.69-1.36; P = 0.846). However, reresection was superior to RFA in 3-year DFSs (OR 2.25; 95% CI, 1.37-3.68; P = 0.001) and 5-year DFSs (OR 3.70; 95% CI, 1.98-6.93; P = 0.000). The outcome of 1-year DFSs was unstable with statistical heterogeneity among studies included in meta-analysis (I = 77.4%). No evidence of publication bias was found. RFA was considered as a less invasive modality for RHCC patients.RFA achieves comparable OSs as reresection in the treatment of RHCC, with lower postoperative complications.

Patient characteristics, overall survival (OS), and recurrence-free survival (RFS) were retrospectively examined, and comparisons were made between the two groups and through subgroup analyses.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The 1-year, 3-year, 5-year, and 7-year OS outcomes were comparable between the surgical resection group and the RFA group (P = 0.193), but the surgical resection group had significantly higher 1-year, 3-year, 5-year, 7-year, and 10-year RFS than the RFA group (P = 0.018). Multivariate analysis revealed that patients with lower age, Child–Turcotte-Pugh score, or albumin–bilirubin score before treatment had better OS, and patients with an HCV infection or receiving RFA treatment had higher HCC recurrence rates.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The liver reserve determined the long-term OS of patients with an HCC tumor ≦ 2 cm, and surgical resection offered better RFS than RFA (<a href="http://ClinicalTrials.gov">ClinicalTrials.gov</a> number, NCT04525833.).</p>

Transarterial Chemoembolization versus Radiofrequency Ablation for Recurrent Hepatocellular Carcinoma after Resection within Barcelona Clinic Liver Cancer Stage 0/A: A Retrospective Comparative Study.

Abstract:PURPOSE:To compare outcomes of transarterial chemoembolization with radiofrequency (RF) ablation in treatment of recurrent hepatocellular carcinoma (HCC) after resection within Barcelona Clinic Liver Cancer (BCLC) stage 0/A. MATERIALS AND METHODS:From January 2007 to December 2011, 110 consecutive patients with recurrent HCC meeting BCLC stage 0/A criteria underwent transarterial chemoembolization (n = 78; mean tumor size, 1.9 cm ± 1.0) or RF ablation (n = 32; mean tumor size, 1.9 cm ± 0.6) as initial treatment. The primary outcome was overall survival (OS). Kaplan-Meier method was used to construct survival curves, which were compared by log-rank test. Prognostic factors for OS were analyzed using univariate and multivariate Cox proportional hazard models. RESULTS:No significant differences between baseline clinical characteristics of the 2 treatment groups were identified. The 1-, 3-, and 5-year OS rates were 89.7%, 61.0%, and 36.6% for the transarterial chemoembolization group and 90.1%, 72.8%, and 60.0% for the RF ablation group. There was no significant difference in OS rates between the groups (P = .159). Subgroup analysis indicated that RF ablation achieved better survival than transarterial chemoembolization among patients ≤ 55 years old and patients with BCLC stage 0 (P = .036 and P = .045). Multivariate analysis revealed that serum albumin (≤ 35 g/L) (hazard ratio = 2.797; 95% confidence interval, 1.366-2.726; P = .005) and α-fetoprotein (> 400 ng/mL) (HR = 2.336; 95% CI, 1.210-4.508; P = .011) levels before treatment were 2 significant risk factors for poor prognosis. CONCLUSIONS:Transarterial chemoembolization might provide a similar OS as RF ablation in patients with recurrent BCLC stage A HCC. However, RF ablation could provide better OS in patients with recurrent BCLC stage 0 HCC.
Effectiveness and Safety of Radiofrequency Ablation versus Liver Resection in the Treatment of Early-stage Hepatocellular Carcinoma: A Systematic Review and Meta-analysis

Abstract:Objective: To systematically evaluate the efficacy and safety of radiofrequency ablation and liver resection in the therapeutic management of early-stage hepatocellular carcinoma. Method: We conducted a comprehensive search of domestic and foreign databases including PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure (CNKI), and Wanfang to retrieve literature on radiofrequency ablation and liver resection for the treatment of early hepatocellular carcinoma. The retrieved literature underwent thorough screening, and relevant data were extracted. Following the evaluation of the literature's quality, Meta-analysis was performed using RevMan 5.4 software. Results: In this study, a total of 11 documents were selected, comprising 1334 patients with hepatocellular carcinoma. Meta-analysis results indicated that there was no statistically significant difference in the 1-year overall survival rate [Relative risk (RR) = 1.01, 95% confidence intervals (CI) (0.98; 1.04)] and the 3-year overall survival rate [RR = 0.95, 95% CI (0.90; 1.01)] between the radiofrequency ablation and liver resection groups (p > 0.05). Similarly, there was no statistically significant difference in the 1-year disease-free survival rate [RR = 0.94, 95% CI (0.87; 1.01)] between the two groups. However, the 3-year disease-free survival rate [RR = 0.84, 95% CI (0.74; 0.96)] of patients in the radiofrequency ablation group was significantly lower than that in the hepatectomy group (p < 0.05). Notably, the incidence of complications [RR = 0.42, 95% CI (0.33; 0.55)] was significantly lower in the radiofrequency ablation group compared to the hepatectomy group. Conversely, the local recurrence rate [RR = 1.45, 95% CI (1.22; 1.73)] was significantly higher in the radiofrequency ablation group compared to the hepatectomy group (p < 0.05). Conclusion: During the treatment of hepatocellular carcinoma, hepatectomy demonstrates superior clinical efficacy compared to radiofrequency ablation, particularly in its ability to control tumor recurrence. However, radiofrequency ablation presents with fewer complications and a higher level of safety. These findings can serve as a valuable foundation for clinicians when selecting the most suitable treatment approaches for liver cancer.

Abstract:Objective To evaluate the method and effect of cool-tip radio-frequency ablation(RFA)in treatment of hepatocarcinoma.Methods Thirty five patients with hepatocarcinoma were treated with percueaneous and ultrasound-quided cool-tip RFA in our hospital.After treatment,the main clinical symptoms,AFP level and liver function were observed.Meanwhile,ultrasonography,CT scaning were performed.Results The lesions significantly decreased in size in 26 patients and 7 cases completely liquified.In 27 patients with abnormal AFP level,the AFP level dropped to normal in 4 weeks after the treatment.No surgical death was found in these patients.Conclusion Cool-tip RAF is a safe,convenient and effective treatment for hepatocarcinoma.

Abstract:Objective To compare the outcomes of cone beam computed tomography (CBCT)-guided radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC) with those of traditional spiral computed tomography (s-CT)-guided RFA. Methods This retrospective study analysed data from patients with HCC that underwent RFA guided by either CBCT or s-CT. A number of preoperative and postoperative characteristics, including operation time, ablation time, radiation dose and hospital stay were recorded for all patients. The incidence of intraoperative and postoperative complications was recorded. The therapeutic effect was evaluated at 1, 3 and 6 months after RFA. Results A total of 47 patients with HCC (12 females and 35 males) underwent successful RFA: 21 underwent CBCT-guided RFA and 26 underwent s-CT-guided RFA. Except for one case of pneumothorax in the s-CT group, no serious complications occurred. The objective response rate and disease control rate at 1, 3 and 6 months after RFA showed no significant differences between the two groups. Throughout the 6-month follow-up period, the complete ablation rate was 19 of 21 patients (90.5%) in the CBCT group and 19 of 26 patients (73.1%) in the s-CT group. Conclusions CBCT was a safe and effective guiding modality for RFA in patients with HCC.
