Abstract:Objective To evaluate the therapeutic effect and safety of heavy silicone oil used as interhal tamponade to treat complicated retinal detachment. Methods Vitreoretinal surgery and intraocular tamponade of heavy silicone oil (Oxane HD) were used in 13 patients (13 eyes) with complicated retina detachment. Scleral buckling, epiretinal membrane peeling, retinotomy, per-fluorocarbon liquid infusion, laser photocoagulation, phacoemulsification were performed in some patients. The best-spectacle corrected visual acuity (BCVA), retinal status and complications after surgery were observed. Results The mean duration of heavy silicone oil endotamponade was 93 days and the mean follow-up was 130 days after heavy silicone oil removal. BCVA was improved in 12 cases (92.3%). Retinal reattached and holes were closed in 10 cases with heavy silicone oil endotamponade (77%) in the end of the follow-up. The postoperative complications included moderate high intraocular pressure, temporary inflammatory reaction, lens opacity, fibrocellular membranes proliferation and heavy silicone oil emulsification. Conclusions The heavy silicone oil Oxane HD is effective and safe to treat complicated retinal detachment, especially with holes located in the inferior and posterior pole.

Abstract:BACKGROUND:Retinal detachment (RD) secondary to macular hole (MH) is a common complication in highly myopic eyes, usually leading to a poor visual prognosis. The purpose of this study was to evaluate the surgical outcome of silicone oil (SO) tamponade and internal limiting membrane (ILM) peeling in the treatment of RD caused by MH (MHRD) in highly myopic eyes with chorioretinal atrophy, and to identify clinical factors associated with the anatomical outcomes.METHODS:We retrospectively reviewed 21 eyes of 21 highly myopic patients affected by RD secondary to MH and chorioretinal atrophy. All eyes were treated with pars plana vitrectomy (PPV) with ILM peeling and SO tamponade. Anatomical success was defined as reattachment of the retina with the closure of the MH, as assessed by optical coherence tomography (OCT), after SO removal. Logistic regression was performed to determine the clinical factors influencing anatomical success.RESULTS:The mean patient age was 59.95 years [standard deviation (SD), 10.39; range, 34-77 years] and the mean axial length was 30.58 mm (SD, 1.52; range, 27.99-34.51 mm). After the first surgical procedure, the anatomical success rate was 61.9% (13 eyes in 21 eyes), with initial retinal attachment of 16 eyes (76.2%). A second surgical approach was performed for the five eyes with persistent or recurrent RD, and the final retinal reattachment rate was 100% (21/21). Logistic regression analysis showed that no specific factors were significantly associated with anatomical success.CONCLUSIONS:Primary silicone oil tamponade and ILM peeling can be a practical treatment for repairing MHRD in highly myopic eyes with chorioretinal atrophy.
The use of silicone oil-RMN3 (Oxane HD) as heavier-than-water internal tamponade in complicated inferior retinal detachment surgery

Abstract:Background: Conventional silicone oil provides suboptimal support of the inferior retina. In this study we evaluated the efficacy of Oxane HD in the management of complex retinal detachments involving lower quadrants of the retina. Methods: A prospective, interventional, comparative study. Eighteen patients were recruited. Treatment outcomes were compared with a historical control group of 14 patients. Patients with grade C3 PVR or greater and inferior retinal breaks, recurrent inferior retinal detachments (with or without PVR) and giant retinal tears were included. In those patients who re-detached under heavy silicone oil (n = 4), retro-oil epiretinal membranes (ERMs) were obtained at the time of subsequent surgery to analyse the immunopathological response to oxane HD. Immunohistochemistry was used to detect glia, retinal pigment epithelium cells (RPE), macrophages, T lymphocytes, or neural elements in the tissue using well-characterised monoclonal antibodies. Results: Retinal attachment of the posterior pole following removal of silicone oil was achieved in 66.6% of the treatment group (n = 12) and 64.3% of controls (n = 9) (p = 1.0). Post-operative PVR developed in five patients in the treatment group (27.8%) and five control patients (35.8%). Following removal of silicone oil, residual oil was observed in 27.8% of the treatment group and 7.1% of controls. Median visual acuity, 3 months following removal of silicone oil, was 2.0 (IQR 0.9-2.0) in the treatment group and 1.0 (IQR 0.6-1.8) in the control group. Complications in the treatment group included, hypotony (n = 3), uveitis (n = 2), glaucoma (n = 1). All ERMs analysed demonstrated microscopic appearances typical of PVR. The membranes were fibrocellular in nature, contained RPE and glial cells, and variable amounts of intracellular and extracellular pigment. In addition, all had a dense infiltrate of vacuolated (presumed oil-filled) macrophages. Conclusion: We failed to observe an advantage following the use of Oxane HD in the treatment of inferior retinal detachments. Moreover, Oxane HD was difficult to remove and was associated with a higher incidence of complications.
A pilot study on the use of silicone oil-RMN3 as heavier-than-water endotamponade agent

Abstract:Aims: This work was conducted to report an interventional non-comparative pilot study using Oxane HD, a mixture of ultra-purified silicone oil and RMN3, a partially fluorinated olefin, as heavier-than-water internal tamponade. Methods: Twenty-eight consecutive patients were recruited for this study. Indications included recurrent retinal detachment (RD) with proliferative vitreoretinopathy (PVR) (stage > or =C2) arising from inferior or posterior tears, recurrences after vitreoretinal surgery, penetrating trauma and combined rhegmatogenous and choroidal detachment. The patients underwent a pars plana vitrectomy, membrane peeling, and Oxane HD was used as long-term internal tamponade. Results: Oxane was removed after 88 days (range 45-96 days) and exchanged with BSS in five eyes, long-acting gas in 14 eyes and with silicone oil in nine eyes. Retinal reattachment was achieved in 15 eyes. The overall anatomical success rate obtained using Oxane HD was 53.5%. In 15 patients with previous marked scleral buckling, the success rate was 26%: in nine patients recurrent RD occurred in the inferior sector, in five patients new tears were detected in the lower sectors; membrane formation was observed in 15 eyes. In 13 patients without marked scleral indent, the success rate was 84.6%. There was no evidence of dispersion and excessive inflammation. Conclusion: Oxane HD may be a useful tool in complicated RD with large inferior breaks, inferior PVR or combined rhegmatogenous, and choroidal detachment without marked scleral buckling, which put the eye profile out of shape, led to a higher failure rate and reduced the tamponading effectiveness of Oxane HD.
Anatomical and Functional Outcomes after Densiron 68 Heavy Silicone Oil Tamponade for Complicated Retinal Detachment in Chinese Eyes

Abstract:AIM:To evaluate the safety and efficacy of Densiron 68 heavy silicone oil (HSO) tamponade for complicated retinal detachment (RD) in Chinese eyes.METHODS: Twenty-one eyes of 21 patients with complicated RD were included in this retrospective study. All patients underwent pars plana vitrectomy with an internal tamponade using Densiron 68 HSO. Anatomical and functional results and complications were evaluated, including retinal status, visual acuity (VA), intraocular pressure (IOP), intraocular inflammation, lens opacity, and HSO emulsification.RESULTS:All the patients were followed up for 3mo to 1y (5.8±1.16mo). Retinal reattachment was achieved in 19 of 21 patients (90.5%). VA improved in 18 of 21 patients (85.7%), from 1.93 logMAR (±0.48) to 1.52 logMAR (±0.45) (P=0.001). Postoperative complications included early dispersion of HSO in 7 eyes (38.8%), cataract in 10 of 18 phakic eyes (55.5%), moderate postoperative inflammation reaction in 10 eyes (47.6%), and elevated IOP in 5 eyes (23.8%), all of which were controlled by medication or by surgery.CONCLUSION: High anatomical and functional success rates can be achieved with primary vitrectomy for complicated RD by using Densiron 68 HSO; however, it should not be ignored that Densiron 68 HSO can cause some complications in the eye.

Abstract:Purpose: To assess the success of the short-term location of the heavy silicone oil (Densiron 68, HSO) as endotamponade after pars plana vitrectomy (PPV) for rhegmatogenous complex retinal re-detachment (RRD). Methods: Consecutive, retrospective, nonrandomized, pilot study was conducted. Twenty-two eyes of 22 patients with complex inferior RRD previously tamponade with gas (SF6 or C3F8) or 1000cSt silicon oil (SSO) were selected. All were treated with heavy silicone oil HSO endotamponade, and its removal was performed after 1-month. The main outcomes were best corrected visual acuity (BCVA) and postoperative complications after the heavy silicone oil removal. Results: Of the 22 eyes, 10 were SSO endotamponade, 3 with fluorinated gas 14% C3F8, and 9 with 20% SF6 at first surgery. In all cases, a complex inferior RRD was observed after the first surgery, in 1-3 months after silicone oil removal or gas endotamponade introduction. In ten cases the proliferative vitreoretinopathy PVR (grade B or C 1-3) was found. The main BCVA before HSO removal was 0.55± 0.20 logMAR (range 0.4-0.7) and after the HSO removal, it was 0.32± 0.29 logMAR (0.1-0.4). Among the postoperative complications, only in 4 eyes the macular edema was found (medically resolved), in 4 eyes an increase of intraocular pressure (IOP), and none of these developed the epiretinal membrane (ERM). Conclusion: The main purpose of this study is to establish a short-term heavy silicone oil endotamponade in eyes with complex retinal detachment recurrences, reducing the possible postoperative complications and having a better prognosis for visual acuity outcomes.

Abstract:Objective To evaluate the efficacy and safety of the use of Densiron -68 as intraocular endotamponade for the treatment retinal detachment due to macular white hole in high myopia.Methods Vitreoretinal surgery and intraocular endotamponade of heavy silicone oil(Densiron - 68 ) were used in 8 patients with retinal detachment due to macular hole in high myopia.The mean duration of heavy silicone oil endotamponade was 8 weeks.Results Retinal reattched and macular holes were closed in 7 cases.BCVA was improved in 6 cases.The postoperative complication included high intraocular pressure, lens opacity and heavy silicone emulsification.No severe inflammatory reaction was found.Conclusion The heavy silicone oil(Densiron -68) is effective and safe to treat retinal detachment due to macular hole in high myopia.

Abstract:Objective To evaluate the efficacy and safety of Densiron 68 as an intraocular endotamponade in the clinical management of complex vitreoretinal patients.Methods Eight consecutive eyes with complicated retinal detachments and had undergone treatment with pars plana vitrectomy and Densiron 68 filling were retrospectively analyzed.During the precedure epiretinal membrane peeling,relaxing retinectomies,perfluorocarbon liquid filling,endolaser photocoagulation and phacoemulsification were performed if necessary.The main outcome measures included: anatomical retinal reattachment,visual acuity,and postoperative complications.Results Follow-up ranged from 208-360 days(a mean of [301.5±57.8] d).In the final follow-up,seven patients showed a stably reattached retina.One patient had high intraocular pressure(60 mmHg,1 mmHg=0.133 kPa) one week after operation,and the condition was not improved after general or local application of drugs;Densiron was removed three weeks after operation and the intraocular pressure returned to normal level.Seven patients with lens had different degrees of opacity of lens;two young patients(patient 2 and 3) developed cataract one week after operation.Conclusion Treatment with Densiron 68 has satisfactory effect for complicated retinal detachment with inferior breaks or giant holes,with the main complication being secondary cataract.(Shanghai Med J,2010,33: 194-196)
First experiences with high-density silicone oil (Densiron) as an intraocular tamponade in complex retinal detachment

Abstract:BackgroundIn this study, we evaluated the anatomic and functional outcome and intraocular adverse effects after a 3-month endotamponade with Densiron 68, a mixture of F6H8 with silicone oil, in complex inferior re-detachments.MethodsForty-eight eyes of 48 patients aged 27–82 years with retinal re-detachment due to proliferative vitreoretinopathy (PVR) grades CP1 to CA7 were included. Mean duration of the Densiron endotamponade was 108.7±66.9 days, with a mean follow-up after removal of 102.8±31.9 days.ResultsTwenty-two patients (45.8%) showed stable retinal reattachment after Densiron removal. Fourteen patients (29.2%) developed retinal re-detachment after removal, generally within 1 month and in the upper circumference (n=8). In 11 patients (22.9%) recurrent re-detachment (inferior n=8) appeared during Densiron endotamponade. In one eye (2.1%) treatment was primarily unsuccessful. Visual acuity improved from mean logMAR 1.66±1.03 to 1.47±0.97 (not statistically significant, P=0.257). Side effects included temporary inflammatory reaction (n=10), fibrin accumulation (n=6), sterile hypopyon (n=2), vitreous hemorrhage (n=6), elevated IOP (n=5), emulsification (n=4) and chronic hypotony (n=4).ConclusionThe anatomical success rate without further interventions of 45.8% (22 of 48 patients) seems unsatisfactory. However, in evaluating the potential of Densiron, it should be considered that all patients in this study had previous surgery with standard procedures, including silicone oil, which had already failed. Intraoperative laser photocoagulation of the periphery of the upper quadrants might reduce the risk of retinal re-detachments.

Abstract:Background: Describe complications and clinical outcomes of heavy silicone oil (HSO) Oxane HD® use as an alternative to overcome the challenges of performing vitrectomy to treat tractional and rhegmatogenous retinal detachments with proliferative vitreoretinopathy (PVR). Methods: A retrospective, observational study was performed on patients from one center from August 2014 to Aug 2023. It was included patients who underwent surgery using HSO Oxane HD® to treat rhegmatogenous retinal detachment with PVR or mixed tractional and rhegmatogenous diabetic retinal detachment. Severely ill patients who could not attend to follow up were excluded. The primary outcome was successful retinal attachment at first postoperative month. A descriptive analysis was performed. Results: Among the 31 patients, 29 (93.5%) underwent surgeries due to rhegmatogenous retinal detachment and two (6.5%) for diabetic retinal detachment. The primary anatomic success was achieved in 27 (87.1%) patients. At the final visit, 17 (56.6%) had vision better than 20/400 (range, 20/30 to light perception). The vision was stable or improved in 22 (76.8%) patients at the end of follow-up. Nineteen (61.3%) patients required hypotensive eye drops after HSO use and twelve (38.7%) still required hypotensive eye drops at the final follow-up; three (9.7%) patients required additional glaucoma surgeries. Conclusions: HSO is safe and useful for complex retinal detachments cases specially with inferior tears and PVR. Ocular hypertension is frequent and usually clinically controlled with hypotensive eyedrops. Close postoperatively follow-up is advised due to the ocular complications, particularly elevated intraocular pressure and emulsification.

Abstract:·AIM:To assess the clinical setting of heavy silicone oil tamponade(Densiron-68)for retinal detachment after silicone oil tamponade.·METHODS:Nine eyes of 9 patients with retinal detachment after silicone oil tamponade were recruited between January 2008 and June 2009.Densiron-68 was used in all cases.Among these cases there were 6 men and 3 women.The youngest was 23 years old,the oldest was 45 years old,the mean age was 33.78±6.89.Among these cases there were 6 cases of perforating injury of eyeball,3 cases of rhegmatogenous detachment of retina.9 cases have secondary proliferative vitreoretinopathy.Surgical techniques included silicone oil removal,membrane peeling,heavy water application,air-fluid exchange,endophotocoagulation,cryocoagulation,Densiron-68 endotamponade et al ·RESULTS:The rate of retinal attachment one week after Densiron-68 endotamponade was 100%.While recurrent retinal detachment appeared in 4 cases during the follow-up time.The earliest re-detachment occurred 1.5 months after the operation,the latest occurred 3 months after the operation.2 cases occurred in eyes with Densiron-68 endotamponade and 2 cases occurred after Densiron-68 removal.Visual acuity was improved in 5 patients by the last follow-up,the improvement rate was 56%;7 cases had increased IOP compared with the preoperation,and could be controled by medicine;the anterior chamber reaction was found in all case.Heavy silicone oil emulsification occurred in all case,the shortest time of emulsification was two weeks,the longest was 3 months,with an average of 2.08±0.58 months.·CONCLUSION:The desirable attitude about heavy silicone oil in complicated retinal detachment is to control indications strictly,observe closely.·

Abstract:Silicone oil has been the only long-term vitreous substitute used in the treatment of retinal detachment since 1962 by Cibis. Nevertheless, its effects on retinal vascular morphology and oxygen supply to the retina are ambiguous in current research. We previously invented a foldable capsular vitreous body (FCVB) to use as a new vitreous substitute in the treatment of severe retinal detachment, but its effects on the retinal vessel were unknown. Therefore, in this study, a standard three-port pars plana vitrectomy (PPV) was performed on the right eye of each rabbit and then silicone oil and FCVB were injected into the vitreous cavity as vitreous substitutes. After 180 days of retention, the retinal vascular morphology did not display any distinct abnormalities and hypoxia-induced factor-1alpha (HIF-1α) and vascular endothelial growth factor (VEGF) did not vary markedly during the observation period in silicone oil tamponade- and FCVB-implanted eyes. This study may suggest that silicone oil and FCVB tamponade in rabbit eyes did not cause retinal vascular pathologic changes or retinal hypoxia for 180 days.
Complications and Treatment of Cases Complicated Retinal Detachment by with Silicone Oil Injection

Abstract:Objective To analyze the efficacy and complications of vitreoectomy with silicone oil injection for complicated retinal detachment. Methods Nine cases of complicated retinal detachment received silicone oil therapy and were followed up 10 to 20 months (average 12 months). The longest case following-up with silicone oil retention in the eye was 20 months,while the shortest with silicone oil taken out was 3 months after surgery because of emulsification. Results The visual acuity and retinal reattachment of all patients were improved in different degrees. The main post operative complications were complicated cataract, secondary glaucoma, silicone oil emulsification and cornea degeneration. The main complications after oil taken-out were retinal detachment, oil residuel in the eye, secondary glaucoma. Conclusion Vitreoectomy with silicone oil injection is effective for complicated retinal detachment. Complications can be minimized with suitable treatment.

Abstract:Abstract Background Silicone oil tamponade has become a mainstay in treatment of advanced retinal detachment due to multiple etiologies. The aim of this study is to assess the characteristics, fates and complications of long-term silicone oil tamponade after par plana vitrectomy (PPV), and to compare the outcomes of different silicone oil viscosities used in a cohort of consecutive patients. Methods This is a retrospective comparative case series of eyes undergoing vitrectomy with silicone oil tamponade for retinal detachment by a single surgeon using different oil viscosities that were followed for one year with the silicone oil in situ. Visual acuity (VA), intraocular pressure (IOP) and complications associated with the follow up period were analyzed and compared. Results Eighty-five eyes of 85 patients were included in this study. Forty three patients had 1000 centistoke (cs) oil injected and 42 patients had 5000cs oil utilized. Demographic, cause of retinal detachment and preoperative ocular characteristics were similar in both groups. Long term complications in both groups included ocular hypertension (67.4% vs 66.7%), keratopathy due to silicone oil emulsification and migration to the anterior chamber (7.0% vs 11.9%), recurrent retinal detachment (4.7% vs 19%) and epiretinal membrane formation (7% vs 19%). In the 1000cs oil group, there was no significant difference between baseline IOP and any subsequent visit. There was a significant difference between baseline IOP and visits at day 1 (with IOP difference of 2.61 mmHg (±6.5)) ( p = 0.028), 1 month (with IOP difference of 3.52 mmHg (±8.1)) ( p = 0.026), 4 months (with IOP difference of 6.38 mmHg (±9.3)) ( p = 0.005), and one year (with IOP difference of 4.24 mmHg (±11.1)) ( p = 0.048), all higher in the post-operative period in the 5000cs oil group. Excluding the first post-operative day, no significant difference was found for VA between baseline visits and subsequent visits for either silicone oil groups. Conclusion In this cohort of patients with long-term silicone oil tamponade after PPV to treat retinal detachment, IOP increased significantly in patients who received 5000cs silicone oil. There was no significant difference between other complication rates in patients receiving either oil viscosities. Long term silicone oil tamponade remains a viable option in certain cases, and a vigilant follow up for complications is necessary to limit any adverse effects and improve visual and surgical outcomes.

Abstract:Objectives: There have been reports of unexplained visual loss following intra-ocular silicone oil (SiO) tamponade in retinal detachment patients, yet the underlying mechanism is unknown. The aim of this study was to investigate the mechanisms behind retinal toxicity following intra-ocular SiO tamponade in retinal detachment patients.Methods and Results: Vitreous fluid samples were acquired from 27 patients (27 eyes). Twelve eyes for data-independent acquisition (DIA) were divided into four groups: pars plana vitrectomy (PPV) for rhegmatogenous retinal detachment (RD group), SiO removal after successful retinal reattachment (SO group), cataract surgery after successful retinal reattachment with sterilized air tamponade (FA group), and PPV for epiretinal membrane (ERM group). The remaining 15 eyes were used for enzyme-linked immunosorbent assay analysis. DIA was combined with two-dimensional liquid chromatography-tandem mass spectrometry to find expression changes in the proteome of vitreous. Mean number mass spectra, statistically differentially expressed proteins, gene ontology (GO), pathway representations, and protein interactions were analyzed. GO analysis showed that the protein categories of synapse organization, cell adhesion, and regulation of cell migration in the SO group were differentially expressed compared to the control or FA groups (p < 0.05). Through Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis, lysosome and cell adhesion were found to be significantly enriched in the SO group compared to the FA and control groups (p < 0.05). Cadherin 2, transferrin, and lysosome function may partially contribute to silicone oil-related vision loss.Conclusion: Vision loss-inducing novel molecular signatures and pathways that may be associated with SiO toxicity were identified. Transferrin may be a potential visual outcome biomarker for SiO tamponade.
Preliminary application of heavy silicone oil in vitrectomy for complicated ocular trauma

Abstract:Objective To investigate the effect of clinical application of heavy silicone oil in vitrectomy for complicated ocular trauma.Methods Nine patients(9 eyes)were with complicated ocular trauma,all eyes were seen the retinal tear in the posterior pole or in the inferior part.Six eyes were with obvious haemorrhage and proliferation around the retinal tear.Retinal tears of 2 eyes closed to the optic disc(1 located at inferior and another at nasal),4 eyes in the macular area,and 3 in the inferior retina(all tears were larger than 1.5 PD and accompanied with obvious proliferation).Appropriate laser photocoagulation was performed,and the heavy silicone oil was used.Postoperative condition such as retinal reattachment,visual acuity and intraocular pressure were observed.Results Intraocular pressure was normal in all eyes at preoperative 3 days and postoperative 1 week.It was 36.53 mmHg(1 kPa=7.5 mmHg)at postoperative 2 weeks.Intraocular pressure of 5 eyes couldn't control with drugs to decrease and treated with paracentesis of anterior chamber.The average corrected visual acuities were 0.04,0.05 and 0.08 at postoperative 1 week,2 weeks and 4 weeks,higher than that at pre-operation.Heavy silicone oil was taken out at postoperative 16 days to 41 days,averaged in 32 days.Five eyes with silicone oil emulsion and first appeared in 1 eye at postoperative 15 days.Retinal reposition was well in all eyes.Conclusions With heavy silicone oil,the elevation of intraocular pressure occurres easily and the emulsification time appears earlier;Heavy silicone oil application indications should be strictly controlled,only used to the patients who can not lie in prone position.

Abstract:Purpose: To report the anatomical and functional outcomes of medium-term perfluoro-n-octane (PFO) tamponade as a rescue procedure in very complex retinal detachments (RDs). Methods: We reviewed the case records of 35 consecutive patients who underwent vitrectomy for very complex RDs due to diverse etiologies. The surgical complexity was so graded because of the intraoperative failure to ascertain complete retinal reattachment; perfluoro-n-octane was used as rescue tamponade for 2 to 4 weeks. The second intervention included additional membrane peeling, retinectomy, endophotocoagulation, and gas/silicone oil tamponade. The minimum follow-up was 3 months after the final intervention: the primary outcome was retinal reattachment and the secondary outcome was change in best-corrected visual acuity (BCVA). Results: The most common presentations were severe trauma with retinal incarceration, preretinal and subretinal hemorrhage, or chronic/recurrent RDs with anterior proliferative vitreoretinopathy. Preoperative BCVA was ≤counting fingers in 31 (88.6%) patients. Complete retinal attachment without any tamponade was achieved in 33 (94.3%) eyes. best-corrected visual acuity improved in 30 (85.7%) eyes: 16 (45.7%) had BCVA ≥20/200 and 21 (60%) regained ambulatory vision (≥5/200). Two eyes developed keratopathy, and four needed antiglaucoma medications. Conclusion: We achieved excellent anatomical outcomes and acceptable functional outcomes in nearly inoperable RDs with few side effects. Medium-term perfluoro-n-octane tamponade can be used as a salvage procedure in very complex RDs where intraoperative reattachment cannot be ensured.
Contamination Profile of Different Formulations of Silicone Oil Tamponade Before and After Intraocular Permanence for Rhegmatogenous Retinal Detachment

Abstract:Purpose: The purpose of this study was to search for contaminants in silicone oil tamponades removed from eyes treated for retinal detachment, and to correlate chemical results with some clinical/functional parameters of the considered eyes. Methods: We examined a sequential cohort of eyes grouped according to the tamponade received: (1) Siluron2000 (S2), (2) RS-OIL ECS5000 (S5), and (3) Densiron Xtra (DX). Samples were collected at the beginning of the scheduled removal and analyzed by untargeted headspace gas-chromatography mass spectrometry (HS-GC-MS). Visual acuity and optic coherence tomography assessments were obtained before and after the tamponade removal. Results: Forty-one samples were analyzed: 22 belonging to the DX group, 13 to the S2 group, and 6 to the S5 group. For each group, a mixture of uninjected commercial preparation was analyzed as the reference. Different siloxanes and fluorinated compounds including perfluorodecalin (PFCL) were the most prevalent chemicals, found in 55% to 100% of the intraocular samples of the 3 groups. Some siloxanes were present also in the control matrices, whereas PFCL was only in the extracted tamponades. In the DX group, the concentration of hexamethylcyclotrisiloxane showed an inverse correlation trend with the duration of its permanence inside the eye (P = 0.054). Different alkanes, propanol, and acetaldehyde were identified only in the control matrices. Conclusions: Several contaminants including siloxanes were identified in the intraocular samples and in the control matrices. A time-related ocular uptake of some of these is conceivable. PFCL was also highly present but only in intraocular samples. Translational relevance: After intraocular permanence silicone oils (SOs) have various unlabeled contaminants with some relevant differences with the commercial formulation chemical profile.
Long-term Follow-Up of Primary Silicone Oil Tamponade for Retinal Detachment Secondary to Macular Hole in Highly Myopic Eyes: a Prognostic Factor Analysis.

Abstract:Purpose To investigate the risk factors associated with retinal detachment recurrence after first vitrectomy in high myopic eyes with macular hole retinal detachment (MHRD). Methods Patients with high myopic eyes with MHRD who underwent pars plana vitrectomy and silicone oil (SO) tamponade with a follow-up period more than 12 months and more than 3 months after SO removal were included in this retrospective study. Logistic regression was performed to determine the risk factors associated with retinal re-detachment. Results A total of 45 eyes from 43 patients were included in this study (11 male and 34 female patients). The retinal re-detachment rate after the first removal of silicon oil was 35.5% (16/45) in a mean postoperative follow-up time of 35.64 +/- 32.94 months. Complete macular atrophy on fundus photography (odds ratio (OR) = 17.021, 95% confidence interval (95% CI): 2.218-130.609, p = 0.006) was a risk factor for MHRD after SO removal, while internal limiting membrane (ILM) peeling (OR = 0.091, 95% CI: 0.013-0.633, p = 0.015) and duration of SO tamponade (OR = 0.667, 95% CI: 0.454-0.980, p = 0.039) were protective factors. Conclusion For high myopic eyes with MHRD, complete macular atrophy was a significant risk factor for retinal re-detachment after silicon oil removal. ILM peeling and the duration of silicon oil tamponade were protective factors.

Abstract:Objective To examine the outcomes of early vitrectomy with silicone oil tamponade in open-globe injuries with retinal detachment.Methods Retrospective analysis of 26 patients(26 eyes) with open-globe injuries and retinal detachment were conducted.All patients underwent vitrectomy and silicone oil tamponade within 72 hours of the trauma.Other necessary surgical procedures were done according to the clinical situations.The retina morphology,visual acuity and surgical complications were recorded.Results The postoperative best corrected visual acuity at the last follow-up visit was 0.12 to 0.3 in 3 eyes(11.54%),counting fingers to 0.1 in 16 eyes(61.54%),LP to HM in 5 eyes(19.23%),and NLP in 2 eyes(7.69%).Recurred retina detachment was found in 4 cases(15.38%) during the follow-up,and was successfully treated with a second surgery.There was 1 case where ocular atrophy occurred(3.85%).There was no postoperative endophthalmitis or severe proliferative vitreoretinopathy(PVR).Conclusion Early vitrectomy with silicone oil tamponade for open-globe injuries complicated with retinal detachment is an effective way to reduce the risk of ocular atrophy,to restore the retina morphology,and to save vision.Surgical complications are few and tolerable.uld effectively control ocular atrophy,reset the retina to save a certain vision,and fewer complications.
