Abstract:Background:There are different surgical methods for the adult recurrent patellar dislocation. Each operation has its indications and cannot achieve satisfactory clinical results for complicated injuries. Objective:To introduce preoperative imaging evaluation for recurrent patellar dislocation and to evaluate the effect of combined surgical regime on the disease. Methods:Between January 2014 and September 2015, 6 adult patients (8 knees) with recurrent patellar dislocation were treat-ed by combined surgery. There were 2 males (2 knees) and 4 females (6 knees) with an average age of 26 years (range, 19-35 years). The mean course of disease was 6 years (ranging from 2 d to 23 years). Risk factors including patellar tilt, outer tibial tubercle, patella alta, femoral anteversion, external tibial rotation, genu valgum, femoral trochlear dysplasia and genu hyper-extension were assessed preoperatively in all patients. Lateral retinaculum release, tibial tubercle shift, femoral rotation oste-otomy, tibial rotation osteotomy, genu valgum osteotomy, femoral trochlear osteotomy and genu hyperextension orthopedics were implemented basing on the evaluation. Medial patellofemoral ligament reconstruction surgery was carried out in all pa-tients. Lysholm score and Kujala score were compared before and after operation. Results:Preoperative patellar tilt angles, tibial tubercle-trochlear groove distance (TT-TG) and Caton-Deschamps index was 33.5°±12.7°(range, 20.6°-60.7°), (20.4± 6.1) mm (range, 11.6-30.2 mm), 1.17±0.16 (range, 1.01-1.47), respectively, which were corrected by 10.5°±3.9°(range, 4.6°-16.1° ), (11.5 ± 5.2) mm (range, 3.9-18.7 mm), and 1.09 ± 0.08 (range, 1.02-1.18) postoperatively. Lateral retinaculum release and medial patellofemoral ligament reconstruction was implemented in 3 knees of 2 cases. Lateral retinaculum release com-bined with tibial tubercle medial shift and medial patellofemoral ligament reconstruction was implemented in 3 knees of 2 cases. Lateral retinaculum release and tibial tubercle medial-distal shift and medial patellofemoral ligament reconstruction was implemented in 2 knees of 2 cases. The mean follow-up was 9 months (range, 1-20 months). There was not dislocation or subluxation during the follow-up. And no other complications occurred. Lysholm and Kujala scores were significantly in-creased postoperatively as compared with preoperative one (91.57±2.64 vs. 52.57±5.88, 90.57±3.74 vs. 59.86±6.47, P<0.05). Conclusions: Detailed assessment of preoperative imaging information, determination of combined surgery and guide for each exacting implementation of operation can make good outcomes for the recurrent patellar dislocation operation.
