Abstract:Objective In order to improve the treatment of the medicamentosa-like dermatitis cases caused by occupational trichloroethylene exposure,the complications of these cases and their potential affecting factors were summarized and analyzed. Methods Clinical data of 186 hospitalized cases from 1997 to 2011 were collected and analyzed.Results Complications were found among 173 cases(93%,173/186),and the complications,in the order of frequency,were:electrolyte disturbances(78.5%, 146/186),secondary infections(61.3%,114/186),ocular lesions(30.6%,57/186),anemia(19.9%,37/186),diabetes mellituses (19.4,36/186),drug eruptions(18.8%,35/186),hypoglycemia(18.8%,35/186),bleedings(18.3%,34/186) and thrombocytopenia (17.2%,32/186).The stepwise logistic regression analysis showed that:1 ) there was a correlation between occurrence of complications and the improper therapy of glucocorticoid;2) the total dosage of glucocorticoid was an independent risk factor for secondary infections,electrolyte disturbances and ENT lesions;3) pulse glucocorticoid therapy was an independent risk factor for bleedings;4) both the pulse therapy and the first dosage of glucocorticoid were the independent risk factors for diabetes mellitus. The fatality rate of these cases was 7.5%(14/186),9 cases due to acute hepatic failure,4 cases due to severe infection and 1 due to fall injury.Conclusion The incidence of complications of these disease was high,which would be the leading cause of death among these cases.Standardized glucocorticoid therapy as well as the correct and timely diagnosis of the complications contributed to reduce the fatality of the disease.
An Animal Model of Trichloroethylene-Induced Skin Sensitization in BALB/c Mice

Abstract:Trichloroethylene (TCE) is a major occupational hazard and environmental contaminant that can cause multisystem disorders in the form of occupational medicamentosa-like dermatitis. Development of dermatitis involves several proinflammatory cytokines, but their role in TCE-mediated dermatitis has not been examined in a well-defined experimental model. In addition, few animal models of TCE sensitization are available, and the current guinea pig model has apparent limitations. This study aimed to establish a model of TCE-induced skin sensitization in BALB/c mice and to examine the role of several key inflammatory cytokines on TCE sensitization. The sensitization rate of dorsal painted group was 38.3%. Skin edema and erythema occurred in TCE-sensitized groups, as seen in 2,4-dinitrochlorobenzene (DNCB) positive control. Trichloroethylene sensitization-positive (dermatitis [+]) group exhibited increased thickness of epidermis, inflammatory cell infiltration, swelling, and necrosis in dermis and around hair follicle, but ear painted group did not show these histological changes. The concentrations of serum proinflammatory cytokines including tumor necrosis factor (TNF)-α, interferon (IFN)-γ, and interleukin (IL)-2 were significantly increased in 24, 48, and 72 hours dermatitis [+] groups treated with TCE and peaked at 72 hours. Deposition of TNF-α, IFN-γ, and IL-2 into the skin tissue was also revealed by immunohistochemistry. We have established a new animal model of skin sensitization induced by repeated TCE stimulations, and we provide the first evidence that key proinflammatory cytokines including TNF-α, IFN-γ, and IL-2 play an important role in the process of TCE sensitization.

Abstract:Trichloroethylene (TCE) is an industrial solvent with widespread occupational exposure and also a major environmental contaminant. Occupational medicamentosa-like dermatitis induced by trichloroethylene (OMLDT) is an autoimmune disease and it has become one major hazard in China. In this study, sera from 3 healthy controls and 3 OMLDT patients at different disease stages were used for a screening study by 2D-DIGE and MALDI-TOF-MS/MS. Eight proteins including transthyretin (TTR), retinol binding protein 4 (RBP4), haptoglobin, clusterin, serum amyloid A protein (SAA), apolipoprotein A-I, apolipoprotein C-III and apolipoprotein C-II were found to be significantly altered among the healthy, acute-stage, healing-stage and healed-stage groups. Specifically, the altered expression of TTR, RBP4 and haptoglobin were further validated by Western blot analysis and ELISA. Our data not only suggested that TTR, RBP4 and haptoglobin could serve as potential serum biomarkers of OMLDT, but also indicated that measurement of TTR, RBP4 and haptoglobin or their combination could help aid in the diagnosis, monitoring the progression and therapy of the disease.
Trichloroethylene Alters Th1/Th2/Th17/Treg Paradigm in Mice: A Novel Mechanism for Chemically Induced Autoimmunity

Abstract:The role of environmental factors in autoimmune diseases has been increasingly recognized. While major advance has been made in understanding biological pathogen-induced autoimmune diseases, chemically triggered autoimmunity is poorly understood. Trichloroethylene (TCE), a common environmental pollutant, has recently been shown to induce autoimmunity. This study explored whether TCE could cause imbalance of T helper (Th) cell subsets which would contribute to the pathogenesis of TCE-induced medicamentosa-like dermatitis. BALB/c mice were treated with TCE via drinking water at doses of 2.5 or 5.0 mg/mL for 2, 4, 8, 12, and 16 weeks. Trichloroethylene exposure caused time- and dose-dependent increase in Th1, Th2, and Th17 and decrease in regulatory cell (Treg) in the spleen at 2, 4, 8, 12, and 16 weeks, with greatest changes mainly at 4 weeks. These effects were mirrored by similar changes in the expression of their corresponding cytokines interferon-γ, interleukin 4 (IL-4), IL-17A, and IL-10. Mechanistically, these phenotypic changes were accounted for by alterations to their respective master transcription factors T-box expressed in T cells, GATA-binding protein 3, Retinoic acid-related orphan receptor ct (RORct), and forkhead box P3. Of interest, TCE treatment shifted the ratios of Th1/Th2 and Th17/Treg; specifically, TCE increased Th17/Treg. These findings provide the first evidence that TCE exposure significantly changes the Th1/Th2/Th17/Treg paradigm and their specific cytokines driven by altered master transcription factors. This may promote autoimmune reactions in the pathogenesis of TCE-induced skin sensitization and associated damage to other tissues.

Severe Hypersensitivity Dermatitis and Liver Dysfunction Induced by Occupational Exposure to Trichloroethylene

Abstract:Trichloroethylene-induced hypersensitivity dermatitis is one of the serious occupational health events in China, however, little is known about the clinical features and possible mechanism of this disorder. The objective of the present study was to report some typical trichloroethylene-induced dermatitis patients and investigate their occupational exposure as well as the clinical features. We sampled and tested some cleaning agents from the companies where TCE-induced skin disorder occurred, the trichloroethylene concentrations were also monitored in the workplace air. Additionally, the symptoms, signs and laboratory test results of patients were collected. TCE concentrations varied from 10.2% to 91.4% in the cleaning agent by gas chromatography-mass chromatography analysis, and TCE levels in the workplace air ranged between 18 mg/m(3) and 683 mg/m(3), at most sampled sites TCE levels were higher than China national health standard for TCE. The trichloroethylene exposure time of the patients was 5-90 days (average 38.2 d), the patients with headache, dizziness, skin itch, fever were 90.5%, 100%, 100%, and 61.9%, respectively. 85.7% patients had skin erythema, 90.5% with rashes, and 38.1% with blisters. In addition, liver enlargement occurred in 3 patients, the abnormal rate of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (T-Bil) were 90.5%, 85.7% and 76.2%, respectively. 6 out of 15 patients were with abnormal electrocardiogram, and trichloroacetic acid (TCA) elevated in 14 patients (66.7%). Taken together, the major detrimental effect of trichloroethylene was to induce hypersensitivity dermatitis and liver dysfunction, the occurrence of this disorder is likely related to the individual hypersensitivity to trichloroethylene exposure.
The skin sensitization and effect on the activation of lymph nodes CD4~+T cells of trichloroethylene

Abstract:Objective Using LLNA BrdU-LLNA method to assess the skin sensitization and effect on the activation of CD4+T cells of trichloroethylene.Methods C57 mice were randomly divided into AOO vehicle control group,TCE tested groups and hexyl cinnamic aldehyde positive control group.25 μL of the appropriate dilution of the test substance,the vehicle alone,or the PC were applied to the dorsum of each ear on day 1,2 and 3.0.5ml of BrdU(10mg/ml) solution were injected inter-peritoneally to each mouse on day 6.Approximately 24 hours after BrdU injection,the animals were humanely killed,the draining auricular lymph nodes were excised and single-cell suspensions were generated by gentle mechanical disaggregation through 200 micron-mesh gauze.The counts of lymph node cells and BrdU labeling index were measured and the SI was calculated for each tested chemicals.The percentage of CD44+CD62L-cells、IFN-γ+ cells and Foxp3+ cells in CD4+T cells in lymph nodes were detected with flow cytometer.Results The organ coefficient of lymph nodes increased in 80% TCE and 25% HCA groups.The BrdU labeling index of TCE tested groups were significant higher than that of vehicle control group,and had an apparent dose-response relationship.The Stimulation Index(SI) of 5% TCE,10% TCE,20% TCE,40%TCE,80% TCE and 25% HCA were 2.24,1.79,1.97,2.36,2.81 and 2.62 respectively.The percentage of CD44+CD62L-cells、IFN-γ+ cells and Foxp3+ cells in CD4+T cells of 80% TCE group were significant higher than that of vehicle control group.Conclusions BrdU-LLNA might be an useful method for evaluating the skin sensitization of chemicals.Trichloroethylene might be a skin sensitizer.Trichloroethylene could activate CD4+T cells of lymph nodes,and increase the percent of Th1 cells and Treg cells in CD4+T cells.
Histopathological and Ultrastructural Alteration in the Skin of BALB/c Hairless Mice Induced by Acute Exposure with Trichloroethylene

Abstract:Objective To explore the effect of trichloroethylene(TCE)on histopathological and ultrastructural changes of skin in BALB/c hairless mice by acute exposure,and the mechanism of skin lesions by TCE.Methods The dorsal skin of BALB/c hairless mice were smeared with 20%,40%,80%or 100% TCE;water and olive oil were used as blank and solvent controls,respectively.One hour later,the smeared skins were observed by visual inspection,then taken for histological and ultrastructural examinatuions by light and transmission electron microscope(TEM),respectively.Results Acute irritation with TCE in skin could cause erythema and edema,even vesication and epidermal necrosis with 100% TCE.The predominant histopathological alterations were hyperkeratosis,intercellular edema,and inflammatory cell,mainly neutrophils infiltrated,but parakeratosis,equivocal exocytosis or acantholysis treated with high concentrations of TCE.Under TEM,cellular membrane crenation,endochylema edema,crowding and abnormalities of organelles could be seen,meanwhile,there were some swelling or vacuole in mitochondria,chromatin condensation and aggregated to the side of nucleus.Conclusions The results suggested that 20% of TCE has irritant action to skin of BALB/c hairless mice,and induces some histopathological and ultrastructural changes.

Abstract:Trichloroethylene (TCE) is a common environmental pollutant and industrial chemical that has been associated with adverse health effects, especially on organ systems. The purpose of this review is to summarize the current findings on organ system damage caused by TCE exposure and the underlying mechanisms involved. Numerous studies have shown that TCE exposure may cause damage to multiple organ systems, mainly the skin, liver, kidney, and circulatory system. The mechanisms leading to TCE-induced organ system damage are complex and diverse. TCE is metabolized in vivo to reactive intermediates, through which TCE can induce oxidative stress, interfere with cell signaling pathways, and promote inflammatory responses. In addition, studies have shown that TCE interferes with DNA repair mechanisms, leading to genotoxicity and potentially carcinogenic effects. This review highlights the importance of understanding the deleterious effects of TCE exposure on organ systems and provides insights into the underlying mechanisms involved. Further research is needed to elucidate the full range of organ system damage caused by TCE and to develop effective prevention and treatment strategies.

Abstract:Trichloroethylene (TCE) is one of the common organic solvents that has been widely used in cleaning or degreasing of metal and electronic products. However, hundreds of cases of hypersensitivity dermatitis have occurred after the workers were occupationally exposed to TCE in China over the past decade. The purpose of this study was to investigate mRNA expression of hepatic metabolic enzyme genes, immune-related genes, apoptosis genes and oncogenes in patients with hypersensitivity dermatitis induced by trichloroethylene. 12 typical patients with TCE-induced hypersensitivity dermatitis were investigated as the study cases, peripheral blood samples were taken from patients and control, and real-time fluorescence PCR assay was applied for detection of mRNA expression of hepatic metabolic enzyme genes, immune-related genes, apoptosis genes and oncogenes. It was found that the relative levels of mRNA expression of CYP1A2, CYP2E1, CYP3A4 and CYP2C9 increased by 723%, 318%, 385% and 216%, respectively, when compared with control (p < 0.01 or p < 0.05); Foxp3, GATA3 and CTLA4 mRNA expression increased by 104%, 106% and 253%, respectively, in TCE patients when compared with control (p < 0.01); T-bet expression decreased by 44% when compared with control (p < 0.01); these findings indicate that some immune-related genes and hepatic metabolic enzyme genes might play an important role in the process of trichloroethylene-induced hypersensitivity dermatitis.

Due to mass production and use, and improper waste disposal, TCE has become a common environmental contaminant, so there is a wide range of occupationally and environmentally exposed population. Occupational and environmental exposure to TCE can produce toxic effects on multiple organs and systems. This paper is a review of the immunotoxicity, reproductive toxicity, neurotoxicity, teratogenic effect and other non-carcinogenic toxic effects of TCE from the aspects of epidemiological study, experimental evidence on animals and toxic mechanisms.
[Study on Mrna Expression of Immune-Related Genes in Patients with Allergic Dermatitis Induced by Trichloroethylene].

Abstract:OBJECTIVE:To study mRNA expression of immune-related genes (Foxp3, GATA3, CTLA4 and T-bet) in peripheral blood of the patients with allergic dermatitis induced by trichloroethylene (TCE).METHODS:The peripheral blood samples were collected from 8 healthy workers (control group) and 8 patients with allergic dermatitis induced by TCE (case group). Real-time quantitative PCR was applied to detect mRNA expression of immune-related genes (Foxp3, GATA3, CTLA4, T-bet).RESULTS:The mRNA expression levels of Foxp3, GATA3 and CTLA4 genes increased by 115%, 97% and 241% in case group, as compared with control group (P < 0.01). The mRNA expression level of T-bet gene decreased by 47% in case group, as compared with control group (P < 0.01).CONCLUSION:The mRNA expression levels of some immune-related genes changed in patients with allergic dermatitis induced by TCE, those genes may play an important role in TCE-induced allergy.
[The establishment of diagnosis model on occupational medicamentosa-like dermatitis induced by trichloroethylene]

Abstract:Objective: Based on magnetic beads based weak cation exchange chromatography (MB-WCX), matrix assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF-MS) and ClinProTools software, the polypeptides of serum about occupational medicamentosa-like dermatitis induced by trichloroethylene (OMLDT) patients were studied, and a diagnostic model of OMLDT was built. Methods: According to diagnostic criteria of OMLDT, serum of 28 OMLDT patients and 28 controls which were diagnosed by Shenzhen prevention and treatment center for occupational disease were collected. With the combination of MB-WCX and MALDI-TOF-MS, the polypeptides fingerprint of serum of 14 OMLDT patients and 14 controls were detected, what's more, the ClinProTools software and SNN algorithm was used for screening characteristic polypeptides and establishing diagnostic model of OMLDT. Then other objects were applied to validate the model to evaluate accuracy. Results: A total of 159 peaks were attained by ClinProTools software, of which 33 peaks were statistical content (P < 0.05). What is more, comparing with the control group, 20 peaks in case group were decreased, and 13 peaks were increased. Two peaks of them were identified, that is 2106.29 and 3263.78, to classify and determine that two groups by receiver operating characteristic curve (ROC) analysis.2D peaks distribution map certified this finding and the area under the ROC curve was closed to 1. A model was established by SNN algorithm, whose cross validation and recognition capability were 87.5% and 98.5%, respectively. Its sensitivity and specificity were 84.8% and 82.1%, separately, which displayed good separating capacity. Conclusion: In the combination of MB-WCX, MALDI-TOF-MS and ClinProTools software, specifical different polypeptides were screened and OMLDT diagnostic model was built primarily. Also, the model and the results were positively validated, which would play a significant role in early diagnosis.

Abstract:Occupational medicamentose-like dermatitis due to trichloroethylene (OMDT) is a key but unresolved question. OMDT patients often present multiple organ damage, including kidney damage. However, the underlying mechanism remains unknown. The purpose of our study was to explore the effect of tubule-specific C5b-9 deposition induced by TCE sensitization on renal tubular ferroptosis and its mechanism. By analyzing pathological changes of TCE-sensitization-mice kidney, we observed a significant renal tubular ferroptosis, which was alleviated by CD59, a C5b-9 inhibitory protein. Moreover, this phenomenon was also replicated in a C5b-9-attacked HK-2 cell model. Further experiments identified that C5b-9 induced cytosolic Ca2+ overload in renal tubular epithelia cells from TCE-sensitization-mice and HK-2 cells. Furthermore, in vitro experiments showed that BAPTA-AM, an intracellular Ca2+ chelator, could rescued ferroptosis induced by C5b-9 in HK-2 cells. Taken together, TCE sensitization induced renal tubular ferroptosis is mediated by C5b-9 and cytosolic Ca2+ overload may play a key role.

Abstract:The halogenated solvent trichloroethylene (TCE) has had many uses in medicine, construction, consumer products, and the military. Many of these uses have been discontinued or restricted due to its toxicity, which affects multiple target organs and includes both acute, high-dose toxicity and chronic, low-dose toxicity that also encompass several cancers. US and international agencies have conducted risk and hazard assessments for TCE, with comprehensive publications coming out in the last 10–15 years. Accordingly, the focus of this article is to review recently published data since that time (i.e., 2014) that clarify unsettled questions or provide additional insights into the metabolism and mechanisms of toxicity of TCE in several target organs. Besides metabolism, the review focuses on the kidneys, liver, immune system, nervous system, cardiovascular and pulmonary systems, the search for biomarkers, and recent analyses of human cancer risk and incidence from TCE exposure.

TCR Vβ Gene Repertoire Diversity in Patients with Occupational Medicamentose-Like Dermatitis Due to Trichloroethylene

Abstract:Objective To observe the diversity of TCR Vβ gene repertoire and clonal expansion in peripheral blood mononuclear cells from 10 health individuals and 5 patients with occupational medicamentose-like dermatitis due to trichloroethylene(OMLDT).Methods Complementarity determining region 3(CDR3) of 24 TCR Vβ genes were amplified using RT-PCR.The PCR products were further analyzed by genescan to evaluate the clonality of T cells.Results 24 Vβ subfamilies could be detected in 10 health individuals,while only 1~6 Vβ subfamilies were identified in 5 patients,showing that the detectable Vβ subfamilies in patients were significantly lower than those in the health individuals(P0.01).Clonal expansion T cells in one or more Vβ subfamilies were found in 2 patients which were oligoclonal pattern.Conclusion Skewed distribution and clonal expansion of TCR Vβ subfamily T cells could be found in OMLDT patients.The changed repertoire of TCR Vβ T cells may be the result of the immune cytotoxicity induced by TCE.

It has been reported that TCE exposure is associated with a wide range of diseases, including cancer, immune system diseases, and congenital heart defect. TCE is metabolized in vivo through two main pathways, Cytochrome P450-dependent oxidation and glutathione conjugation. The adverse effects of TCE in different target tissues may be partly due to specific reactive metabolites. In this review, we will evaluate the evidence on carcinogenicity, immunotoxicity, and cardiac developmental toxicity of TCE and will discuss the potential underlying molecule mechanisms.

Abstract:OBJECTIVE:To study skin sensitization as well as liver and kidney impairment in guinea pigs treated with trichloroethylene (TCE).METHODS:Guinea pig maximization test (GPMT) was applied in this study, guinea pigs were divided into 3 groups, namely negative control, positive control and TCE treatment. Animals of 3 groups were administrated with olive oil, 2, 4-dinitrochlorobenzene (DNCB), and TCE, respectively, by intradermal injection. The animal skin was observed and blood was collected after various treatment, the liver function tests were conducted, including detection of activities of ALT, AST, LDH and levels of creatinine, uric acid, and urea with automatic biochemical analyzer.RESULTS:Obvious skin impairment was observed in the groups of positive control and TCE treatment, the skin impairment included erythema and edema, the sensitization rate was 100% in positive control and 83.3% in TCE treatment group. Additionally, the activities of ALT, AST and LDH increased significantly in the groups of positive control and TCE treatment when compared with the negative control.CONCLUSIONS:Trichloroethylene is one of the strong hypersensitizing substances, it could induce skin allergic reaction and liver impairment in guinea pigs.

Abstract:Trichloroethylene (TCE) is a common organic solvent that has been widely used in industrial applications. Hundred cases of allergic reactions occurred after the workers were occupationally exposed to TCE in China in the past decade, but the underlying effector mechanisms of TCE remain unclear. The purpose of the present study is to examine the alteration of hepatic metabolic enzyme gene and apoptosis-related gene messenger RNA (mRNA) expression in L02 human hepatocytes (L02 cells) after treatment with TCE. L02 cells were cultured either with various doses of TCE (0.25, 0.5, 1.0 and 2.0 mmol/L) for 24 h or with a single dose of TCE (1.0 mmol/L) for different time intervals, whereas samples treated with dimethyl sulfoxide served as control. Quantitative real-time polymerase chain reaction analysis was performed to detect the mRNA expression of hepatic metabolic enzyme genes ( CYP1A2, CYP3A4 and CYP2E1) and apoptosis-related genes ( BAX and BAD). It was found that the transcript levels of hepatic metabolic enzyme genes and apoptosis genes including BAX and BAD were significantly increased after TCE treatment at various doses for 24 h when compared with controls. Additionally, when the cells were treated with a single dose of TCE (1.0 mmol/L) for different periods of time (3, 6, 12 and 24 h), the mRNA expression of these genes also increased significantly compared with control ( p < 0.05 or p < 0.01). The conclusion of the study is that TCE could induce alteration of mRNA expression of hepatic metabolic enzyme genes and apoptosis genes, which might be implicated in the effector mechanisms of TCE cytotoxicity in vivo.
Effects of chronic triclosan exposure on nephrotoxicity and gut microbiota dysbiosis in adult mice

Abstract:Triclosan (TCS), a broad-spectrum, lipophilic, and antibacterial agent, has been commonly used in cosmetics, medical devices, and household products. The toxicity of TCS has recently become a research hotspot. Emerging evidence has shown that TCS can easily migrate to humans and animals and cause adverse effects on various target organs. However, the effects of TCS exposure on nephrotoxicity and underlying mechanisms remain unknown. The aim of the present study was to explore TCS-induced nephrotoxicity. Therefore, we establish a mouse model based on adult male mice to explore the effects of 10-week TCS exposure (50 mg/kg) on kidney. After mice were sacrificed, their blood, feces, and renal tissues were harvested for further analysis. We found that TCS treatment dramatically caused kidney structural damage, and increased blood urea nitrogen (BUN) and creatinine (Cr) expression levels, which indicated renal dysfunction. In addition, TCS exposure increased the malondialdehyde (MDA) and decreased superoxide dismutase (SOD) and total cholesterol (TCHO) expression levels, which indicated oxidative stress and lipid metabolism changes. The RNA sequencing (RNA-seq) of kidney tissue identified 221 differentially expressed genes (DEGs) enriched in 50 pathways, including drug metabolism-other enzymes, oxidative phosphorylation, glutathione metabolism, and inflammatory mediator regulation of TRP channels signaling pathways. The full-length 16S rRNA gene sequencing results showed that TCS exposure altered the community of gut microbiota, which was closely related to renal function damage. The above findings provide new insights into the mechanism of TCS-induced nephrotoxicity.

Abstract:Objective: To investigation to an accident of occupational trichloroethylene explosion and analyze the clinical datas. Methods: An investigation was carried out on the explosion of trichloroethylene in a place in shandong province in June 2016, and the clinical data of 4 patients with trichloroethylene poisoning were analyzed. Results: The incident is caused by irregularities in workers, 4 patients were characterized by different levels of the respiratory system, nervous system, liver, and skin lesions. Among them, respiratory system damage was more prominent, mainly manifested as chemical bronchitis and pneumonia, after positive rescue and treatment all recovered and discharged. Conclusions: Trichloroethylene leak can cause severe ex-plosion accidents, can cause workers of the respiratory system, nervous system, liver and skin lesions, mainly glucocorticoids comprehensive therapy has good curative effect.
