Abstract:SummaryThis paper discusses the progress and prospects of China's complex health care reform beginning in 2009. The Chinese government's undertaking of systemic reform has achieved laudable achievements, including the expansion of social health insurance, the reform of public hospitals, and the strengthening of primary care. An innovative policy tool in China, policy experimentation under hierarchy, played an important role in facilitating these achievements. However, China still faces gaps and challenges in creating a single payer system, restructuring the public hospitals, and establishing an integrated delivery system. Recently, China issued the 13th 5‐year plan for medical reform, setting forth the goals, policy priorities, and strategies for health reform in the following 5 years. Moreover, the Chinese government announced the “Healthy China 2030” blueprint in October 2016, which has the goals of providing universal health security for all citizens by 2030. By examining these policy priorities against the existing gaps and challenges, we conclude that China's health care reform is heading in the right direction. To effectively implement these policies, we recommend that China should take advantage of policy experimentation to mobilize bottom‐up initiatives and encourage innovations.
Health System Reform in Rural China: Voices of Healthworkers and Service-Users.

Abstract:Like many other countries China is undergoing major health system reforms, with the aim of providing universal health coverage, and addressing problems of low efficiency and inequity. The first phase of the reforms has focused on strengthening primary care and improving health insurance coverage and benefits. The aim of the study was to explore the impacts of these reforms on healthworkers and service-users at township level, which has been the major target of the first phase of the reforms. From January to March 2013 we interviewed eight health officials, 80 township healthworkers and 80 service-users in eight counties in Zhejiang and Yunnan provinces, representing rich and poor provinces respectively. Thematic analysis identified key themes around the impacts of the health reforms. We found that some elements of the reforms may actually be undermining primary care. While the new health insurance system was popular among service-users, it was criticised for contributing to fast-growing medical costs, and for an imbalance of benefits between outpatient and inpatient services. Salary reform has guaranteed healthworkers' income, but greatly reduced their incentives. The essential drug list removed perverse incentives to overprescribe, but led to falls in income for healthworkers, and loss of autonomy for doctors. Serious problems with drug procurement also emerged. The unintended consequences have included a brain drain of experienced healthworkers from township hospitals, and patients have flowed to county hospitals at greater cost. In conclusion, in the short term resources must be found to ensure rural healthworkers feel appropriately remunerated and have more clinical autonomy, measures for containment of the medical costs must be taken, and drug procurement must show increased transparency and accountability. More importantly the study shows that all countries undergoing health reforms should elicit the views of stakeholders, including service-users, to avoid and address unintended consequences.

Abstract:Objective To study China's policy of health human resources accommodated with the socio-economic development and medical demands. Methods The literature consultation method was used to analyze the status and experience in health human resources development in China since the reform and opening-up to the outside world over the past 30 years. Results Health human resources are the main motive power and decisive factor in the development of China's health work. Condition It is imperative to establish a mechanism to enhance and bring the initiative of health professionals into fall play, and set up a unified evaluation system consisting of responsibility, right and benefit, and a system of classified management of health human resources.

Abstract:Solving the present issue of the hardship and expensive to seek medical care is related to the access, equity and cost of health service. But it is not the final outcomes of health reform and development. It reflects the cause of institutional issue in China. The paper puts forward a blueprint of China's health reform and development. It contains one goal: universal coverage of basic health service, strengthening three systems: basic medical insurance system, basic healthcare system and basic essential drug system as well as six strategic measurements. In the future 15 years, China will under the ideology of people-oriented and scientific development develop a harmonious society and socialist "Xiao Kang" society. Universal coverage of basic health service is the vision of Chinese population. To strengthening the responsibility of government and promoting the equity of health service, the author suggests making the universal primary education and universal coverage of basic health service as two basic social policies. However, the goal of universal coverage of basic health service is a long-term process, it requires mobilizing community involvement and coping with the social and economic development.

Abstract:China experienced both economic and epistemological transitions within the past few decades, greatly increasing demand for accessible and affordable health care. These shifts put significant pressure on the existing outdated, highly centralized bureaucratic system. Adjusting to growing demands, the government has pursued a new round of health reforms since the late 2000s; the main goals are to reform health care financing, essential drug policies, and public hospitals. Health care financing reform led to universal basic medical insurance, whereas the public hospital reform required more complex measures ranging from changes in regulatory, operational, and service delivery settings to personnel management. This article reviews these major policy changes and the literature-based evidence of the effects of reforms on cost, access, and quality of care. It then highlights the outlook for future reforms. We argue that a better understanding of the unintended consequences of reform policies and of how practitioners’ and patients’ interests can be better aligned is essential for reforms to succeed.

Abstract:The review summarizes the ideology,institutional arrangements,policy measurements in governments at different levels,and the imperfectness of the policy on basic public health equality in the new round of health system reform in China. We concluded that though remarkable improvement had made in public health service equality,there were still many challenges to achieve the goal of universal health coverage by 2020. To achieve the goal,Chinese government should put more emphasis on reducing the health disparities across different regions and different social classes,increasing the service capacity of health institutions and health personnel,and improving the quality and efficiency of public health services.

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><p>In 2009, China launched a major health-care reform and pledged to provide all citizens with equal access to basic health care with reasonable quality and financial risk protection. The government has since quadrupled its funding for health. The reform's first phase (2009–11) emphasised expanding social health insurance coverage for all and strengthening infrastructure. The second phase (2012 onwards) prioritised reforming its health-care delivery system through: (1) systemic reform of public hospitals by removing mark-up for drug sales, adjusting fee schedules, and reforming provider payment and governance structures; and (2) overhaul of its hospital-centric and treatment-based delivery system. In the past 10 years, China has made substantial progress in improving equal access to care and enhancing financial protection, especially for people of a lower socioeconomic status. However, gaps remain in quality of care, control of non-communicable diseases (NCDs), efficiency in delivery, control of health expenditures, and public satisfaction. To meet the needs of China's ageing population that is facing an increased NCD burden, we recommend leveraging strategic purchasing, information technology, and local pilots to build a primary health-care (PHC)-based integrated delivery system by aligning the incentives and governance of hospitals and PHC systems, improving the quality of PHC providers, and educating the public on the value of prevention and health maintenance.</p>

Abstract:<h3 class="Heading">Background</h3><p class="Para">Public health system plays a vital role in the development of health sector in China and protects the health of Chinese people. However, there are few comprehensive reviews and studies focusing on its evolution and reform. It is worthwhile to pay attention to the public health development in China, given that the history and structure of public health system have their own characteristics in China.</p><h3 class="Heading">Methods</h3><p class="Para">The study is a retrospective review of the development public health over seven decades in China. It presents the findings from some national or provincial survey data, interviews with key informants, reviews of relevant published papers and policy contents.</p><h3 class="Heading">Results</h3><p class="Para">This study identified four key stages that public health experienced in China: the initial stage centering on prevention, the stage of deviation with more attention to treatment but little to prevention, the recovery stage after SARS(Severe Acute Respiratory Syndromes) Crisis, and the new stage to an equitable and people-centered system. In the latest stage, the National Basic Public Health Service Program (NBPHSP) is implemented to respond the threat of noncommunicable diseases (NCDs) and has achieved some initial results, while there are still many challenges including service quality, poor integration among service items and IT system, lack of quality professionals and insufficient intersectoral endeavor.</p><h3 class="Heading">Discussion</h3><p class="Para">There are unique Chinese wisdom and remarkable achievements as well as twists and turns on the development of China's public health. Prevention-first, flexible structure of the system, multi-agency collaboration and mass mobilization and society participation are the main experience of public health in early stage. Despite twists and turns since 1980s, public health system in China shows substantial resilience which may be from the government's continuous commitment to social development and people's livelihoods and its flexible governance. In 2010s, in order to achieve the well-off society, Chinese government pays unprecedented attention to health sector, which bring a new wave of opportunities to public health such as remaining the NBPHSP for priority. The evolution and reform of China's public health is based on its national condition, accumulates rich experience but also faces many common worldwide challenges. Getting this development and reform right is important to China's social and economic development in future, and China's experience in public health may provide many lessons for other countries.</p><h3 class="Heading">Conclusion</h3><p class="Para">Public health in China needs to focus on prevention, strengthen multi-agency coordination mechanism, improve the quality of public health services in the future.</p>

Abstract:Chinese government built and sustained a simple public service system based on egalitarianism and "from cradle to grave" in the pre-reform era. During 1980s and 1990s, public service reform was going on slowly with characters of urban-rural dichotomy, socialization, marketization and localization. Since 2002, Chinese government tried her best to build a more complete and universal public service system through reforms of social policy, fiscal regime and service provision mechanism. However, there are still many serious issues despite of great achievements: the proportions of public service spending to total fiscal spending still lower; the service standards far away from the true public needs; the obvious disparities among urban-rural, groups and regions remained. It is urgent for Chinese government to actively advance social policy reform and public service’s institution and mechanism innovation.

Abstract:The Chinese health reform did make some significant achievements since 2009. However, affordability and accessibility to primary care and public health remain to be serious concerns. Health governance reforms are necessary to address affordability and accessibility issues in the next stage of health reform. In 2009, China initiated a new round of health reform to establish a well‐functioning health system. The 2009 health reform did make some significant achievements in improving affordability and accessibility. In particular, social health insurance has been expanded significantly and various social health insurance plans have covered over 95% of total population in China by 2011. The Chinese government also has significantly increased fiscal input for health financing. However, affordability remains a serious concern as the social health insurers, as purchasers, were not very effective in containing the growth of health expenditure. Accessibility to primary care and public health is another concern. In this context, health governance reforms are necessary to address affordability and accessibility issues. Health governance set rules for key actors of the health system (including service providers, health insurers and government departments) by taking into account the strategies and incentives of these actors in their interactions. In recent years, a series of reforms in health governance have been initiated. Some progresses have been achieved. For the next stage of health reform, issues including how to further reform the governance structure of medical institutions and how to improve resource allocation in the health system are critical.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.

Abstract:After the launch of the historical Reform of the Medical and Health Care System in 2009, China's primary health care (PHC) system has witnessed significant progress and breakthroughs. In this review, we summarized published literature and official policies and synthesized data from the electronic registration information system of the National Health Commission, national statistical reports, or yearbooks in health care. The review describes the structural design and major achievements of PHC in China in the last decade, emphasizing the national policy foundation, budgeting and insurance coverage, evolution of service delivery, professional team development, and the advancement of supporting technologies. Over 40 years ago, primary health care (PHC) was defined in the Alma‐Ata Declaration as a critical component of the health care system to address the basic health demand of the people. In China, the Government attaches great importance to health care at the primary level. After the launch of the historical Reform of the Medical and Health Care System in 2009, the PHC system in China has witnessed major progress and breakthroughs, especially in its steadily increased capacity, continuously improved accessibility, and betterment in equality. In this review, we summarized published literatures and official policies, synthesized data from the electronic registration information system of the National Health Commission, national statistical reports, and yearbooks in health care. The review is intended to describe the systematic development of PHC in China in the last decade. The main results include: the solid national policy foundation, increasing number of PHC institutions and workforce, better training of PHC professionals, major achievements in primary health indicators, government financial support to PHC institutions, improved PHC budgeting and insurance coverage, and the advancement of supporting technologies. Challenges and prospects are also discussed.

A Few Thinking about Public Health Development in China

Abstract:Public health is a social welfare undertaking, and the government takes the lead in this area given its responsibility to guarantee and provide the people’s interests. The explicit goal of China’s healthcare reform is to establish universal coverage that provides safe, effective, convenient, and affordable basic health services to all urban and rural residents [1]. In our opinion, China’s healthcare reform has the following characteristics: establishing a health system to ensure the social welfare and fairness; emphasis on the foundation (system building), strengthening primary healthcare (human resource development) and establishing mechanisms (health service delivery and health financing); and gradually realizing equitable access to public health services. From the above, it is clear that the government attaches great importance to its leadership role in health. This provides a strong policy environment for the future development of public health in China.

Abstract:China's 3 year, CN¥850 billion (US$125 billion) reform plan, launched in 2009, marked the first phase towards achieving comprehensive universal health coverage by 2020. The government's undertaking of systemic reform and its affirmation of its role in financing health care together with priorities for prevention, primary care, and redistribution of finance and human resources to poor regions are positive developments. Accomplishing nearly universal insurance coverage in such a short time is commendable. However, transformation of money and insurance coverage into cost-effective services is difficult when delivery of health care is hindered by waste, inefficiencies, poor quality of services, and scarcity and maldistribution of the qualified workforce. China must reform its incentive structures for providers, improve governance of public hospitals, and institute a stronger regulatory system, but these changes have been slowed by opposition from stakeholders and lack of implementation capacity. The pace of reform should be moderated to allow service providers to develop absorptive capacity. Independent, outcome-based monitoring and evaluation by a third-party are essential for mid-course correction of the plans and to make officials and providers accountable.

Abstract:The health care reform was to build the basic health care system in China. As a health care organizers’ goal,this target is too remote and abstract. People couldn’t understand the relation between it and their life and what it was going to bring about to them. For the governments at different levels in local areas,they couldn’t be aware of grasping what they should do with commitment and service to the people during the reform. Based on the research results over the past 30 years from Collaborative Innovation Center for Social Risks Governance in Health,the paper discovered that China needs the social target for the public during health care reform to get the people cohere and local government perform their function.“Basic health protection for all”and“defending high expense disease”is the dream for so many people. More than 85%of the people have been dreaming of the National Health Care System,which protects Chinese from high medical expenditure and somewhat basic risks. China has enough resources to develop this system under the scientific design. The key to develop the system is the government’s decision under the scientific and mature technology.
