Clinical analysis of oncologic safety of laparoscopic distal gastrectomy with D_2 lymphadenectomy for gastric cancer

Abstract:Objective To compare the number of lymph node dissected and resection margin between laparoscopic and open distal gastrectomy with D2 lymphadenectomy, and evaluate the oncologic safety and feasibility of laparoscopic D2 lymphadenectomy for gastric cancer. Methods Clinical data of 363 gastric cancer patients undergoing laparoscopic distal gastrectomy with D2 lymphadenectomy from January 2010 to June 2012,and 412 patients undergoing open distal gastrectomy with D2 lymphadenectomy from January 2008 to January 2010 ,in the Department of General Surgery , the Second Hospital of Jilin University , were analyzed retrospectively. The number of lymph node dissected and resection margins were compared between the two groups. Results There were no significant differences in age, gender, BMI and T stages between the two groups. The numbers of lymph node dissected in laparoscopic and open groups were (23.5±8.4) and (24.7±9.3), The proximal and distal resection margin were (6.95±1.92)cm and (4.89±1.80)cm in the laparoscopic group , (7.28±2.01)cm and (4.05±1.77) cm in the open group. There were no significant differences in the number of lymph node dissected and resection margins between the two groups (P>0.05). The intraoperative blood loss and mean hospital stay were (110.6±35.2)ml and (9.5±2.6)d respectively in the laparoscopic group , both significantly less than (420.5±112.2)ml and (11.5±2.9)d in the open group (P<0.05). Conclusion As for oncologic safety, laparoscopic D2 lymphadenectomy for gastric cancer is safe and feasible , and can achieve the same curative effect as D2 lymphadenectomy, with less intraoperative blood loss and minimally invasive effect.
