A Study about Associated CT Findings of N Staging in Rectal Carcinoma with Pathological Correlation

Abstract:Objective To investigate the relevant characteristic findings of rectal carcinoma in CT images which are associated with N staging.Methods Fifty-nine patients (38 male, 21 female, media age 58, range from 36 to 80 years old) underwent radical resection for rectal carcinoma after preoperative CT examinations were obtained. For N staging, pN0, pN1, pN2 were considered on the basis of pathological examination of excised specimens according to AJCC N staging criterion. Images were reviewed by two radiologists blindly using CT cine on workstation, making the consensus on the size, number and distribution of lymph nodes which were displayed perirectally, along superior rectal artery or along iliac vessels. The relationships between lymph node metastases and CT findings were analyzed statistically by SAS using Kruskal-Wallis test and χ 2 test. Results Lymph nodes were depicted in all node positive cases. Diameters of the largest nodes displayed in the group of pN0, pN1, pN2 were(4.13±3.21)mm, (7.43±3.27)mm, and (10.27±3.88)mm, respectively, which showed a statistically increase with N stage developing(χ 2=23.842,P0.01). The mean numbers of lymph nodes in the group of pN0, pN1 and pN2, (3.40±2.75), (5.07±3.02), and (8.93±2.99), respectively, were different significantly(H=21.834,P0.01), and also showed an increasing trend. The summation diameters of all depicted nodes in the CT images also showed a significant difference in the pN0, pN1 and pN2 groups (H=32.037, P0.001). The metastatic rate of those cases with lymph nodes displayed perirectally, distributed both perirectally and along superior rectal artery and along iliac vessels displayed combined either of previous two distributions patterns were 3/12, 17/29 and 8/11. The distribution patterns of lymph nodes were different significantly in the pN0, pN1 and pN2 groups(χ 2=19.517,P0.05). Serosa disorders and circumferential infiltration were shown at a statistically higher frequency in nodes positive groups than in nodes negative group(χ 2=8.979,P0.01;χ 2=5.107,P0.05).Conclusion The involvement of lymph nodes and its extension in rectal carcinoma are associated with not only the size of nodes, but also the number, distribution of the depicted lymph nodes, serosa appearances and circumferential infiltration status of primary tumor in CT. It is probably helpful to consider these relevant factors comprehensively for improving N staging of CT in patients with rectal carcinoma.
