Expert Opinion on Pharmacotherapy

Abstract:Introduction: The prevalence of obesity is increasing worldwide and associated conditions, particularly type 2 diabetes mellitus (T2DM), also show increasing prevalence. Lifestyle intervention should be the first line of management for obesity but additional pharmacotherapy is often required and bariatric surgery is appropriate in more severe cases. Drugs acting as glucagon-like peptide-1 receptor agonists (GLP-1RAs) developed for the management of T2DM reduce body weight and liraglutide is the first GLP-1RA to be approved for the treatment of obesity in patients with and without T2DM. Areas covered: In this review of relevant published material, the authors summarize the pharmacokinetics, pharmacodynamics, clinical efficacy and safety of liraglutide for the treatment of obesity. Expert opinion: Liraglutide effectively reduces body weight and body fat through mechanisms involving reduced appetite and lowered energy intake, independent of its glucose-lowering effects. Like most of the other medications currently available for obesity, liraglutide has some common adverse effects, although generally not serious ones. Liraglutide has additional benefits in reducing cardiovascular events in patients with T2DM but the cost and the need for daily injections may limit its use in obesity. Newer GLP-1RAs, such as semaglutide, or other drugs in development for obesity may have advantages over liraglutide.

Abstract:The failure of islet β cell is the core pathogenic mechanism of diabetes,which is also the key target for type 2 diabetic mellitus treatment.A large amount of basic and clinical researches have been confirmed that glucagon like peptide-1 and its analogue liraglutide have abilities to significantly improve islet β cell function besides the glucose-dependent lowering hyperglycemia effects.Furthermore,liraglutide is approved to improve β cell function,thus providing a new choice for treatment of type 2 diabetes.
Liraglutide: a review of the first once-daily GLP-1 receptor agonist

Abstract:Liraglutide is an analog with 97% homology to human glucagon-like peptide (GLP-1) and acts as a GLP-1 receptor agonist. Several large, randomized, multicenter phase 3 trials evaluated the efficacy and safety of liraglutide by comparing monotherapy and combination therapy with other antidiabetic medications in adult patients with type 2 diabetes. The Liraglutide Effect and Action in Diabetes (LEAD) program demonstrated that liraglutide, when used alone or in combination with other antidiabetic medications, effectively controls hyperglycemia (glycosylated hemoglobin [A1C] reductions up to 1.6%) and assists patients in meeting established glycemic targets. Compared with certain other classes of antidiabetic agents, liraglutide is associated with a lower risk of hypoglycemia. Liraglutide has also been associated with weight loss (1.8 to 3.4 kg) and improved patient satisfaction and health-related quality of life. Several studies have demonstrated that GLP-1 receptor agonists may improve pancreatic beta cell function, which may delay disease progression if maintained over the long term. As with any drug, liraglutide is not without risk, and a patient's complete clinical status and benefit-to-risk profile should be considered before prescribing treatment. For patients with type 2 diabetes who have failed to achieve glycemic control through diet and exercise, liraglutide may be an important treatment option. The current consensus statement of the American Association of Clinical Endocrinologists (AACE) and the American College of Endocrinology (ACE) cites efficacy and low risk of hypoglycemia in preferring GLP-1 agonists and dipeptidyl peptidase-4 (DPP-4) inhibitors over sulfonylureas and glinides, after initial treatment with metformin. The guidelines prefer GLP-1 agonists over DPP-4 inhibitors because of their actions that promote weight loss and their somewhat greater effectiveness in reducing postprandial glucose excursions.

Abstract:Conventional treatment of type 2 diabetes is to use multiple oral anti-hyperglycemic drugs and/or insulin based on the control of diet and physical activity.We emphasize the efficacy and safety of the new drug —Glucagon-like peptide-1 analogue(Liraglutide) in clinical trials of treating patients with type 2 diabetes.The results of large scale of LEAD series study,clinical study in Asian especially Chinese patients approve liraglutide's multiple beneficial.Liraglutide decreases blood glucose in glucose-dependent manner,protects β cell function,decreases body weight as well as reduces systolic blood pressure.Besides,the rates of hypoglycemic and other side effects are low,thus providing a better way to treat type 2 diabetes.

Abstract:"Diabetes Dialogue" is an international forum of latest cutting-edge research on diabetes. The major topics in the 5th event in 2008 were the novel targets in the treatment of type 2 diabetes. In particular, the latest developments in incretin-based therapies, especially glucagon-like peptide-1 (GLP-1) -based therapies, and their effects on the β-cells have been provided. Studies suggested that type 2 diabetes is characterized by progressively deteriorated β-cell function with the presence of insulin resistance. Owing to its glucose-dependant glucose lowering effect and the effect on improvement of β-cell function, GLP-1 has become the novel target of therapies. Liraglutide, a GLP-1 analogue developed by Novo Nordisk, is one of the novel therapies in the treatment of type 2 diabetes. Many studies have demonstrated that liraglutide showed both an efficacious and sustained effect on glycemic control and a significant improvement of 13-cell function. As a result, recently the role of GLP-1 or GLP-1 analogue in the management of type 2 diabetes has gained more popularity and liraglutide is considered as a promising drug in the treatment of type 2 diabetes. Effects of Liraglutide on Glycemic Gontrol in Metformin-Treated Patients with Type 2 Diabetes Mellitus

Abstract:Objective To investigate the therapeutic effect of liraglutide on glycemic control in metformin-treated patients with type 2 diabetes mellitus (T2DM).Methods Twenty patients with T2DM by maximum tolerated dose of mefformin in this study.For at least 3 months,participants were treated by addingliraglutide.After 1 week,the dosage of liraglutide was increased to 1.2 mg for 3 months from 0.6 mg.Patients' blood glucose,glycosylated hemoglobin(HbA1c),islet function,blood lipid and body weight were observed.Results Compared with before treatment,there were significant change of fasting blood glucose ((8.9 ± 1.4) mmol/L vs.(6.9 ± 1.0) mmol/L,t =5.951,P ＜0.01),postprandial blood glucose ((14.4 ±1.5) mmol/L vs.(10.4 ± 1.4) mmol/L,t =8.878,P ＜ 0.01)),HbA1 c ((8.7 ± 1.3) ％ vs.(7.6 ± 1.1) ％,t =4.860,P ＜0.01),fasting C peptide ((1.7 ±0.6) mg/L vs.(2.9 ± 1.1) mg/L,t =5.752,P ＜ 0.01) and postprandial2 h C peptide ((5.0 ±2.3) mg/L vs.(10.2 ±3.5) mg/L,t =5.667,P ＜0.01) after treatment.Significant change of blood liquid and body weight were also observed after treatment than before treatment (P ＜ 0.01).Conclusion Liraglutide treatment resulted in persistent better glycaemic control,weight reduction,improvements of blood lipid and function of pancreatic island successfully.Liraglutide was also associated with low risk of hypoglycemic and should be chosen by patients with T2DM.
Meta-analysis of the clinical efficacy of liraglutide in treating type 2 diabetes mellitus complicated with non-alcoholic fatty liver disease

Abstract:This study aimed to systematically evaluate the efficacy of liraglutide in treating type 2 diabetes mellitus (T2DM) complicated with non-alcoholic fatty liver disease (NAFLD) by comparing liraglutide with placebo or other drugs (mainly insulin). The PubMed, Web of Science, and National Library of Medicine databases were systematically searched from their inception until December 1, 2023. A meta-analysis was performed using Stata 15.1 software. A total of 12 studies with 13 outcome measures were included. The meta-analysis results revealed that liraglutide significantly reduced body mass index (mean difference [MD] = -1.06, 95%CI: -1.41, -0.70, p < 0.001), triglycerides (MD = -0.35, 95%CI: -0.61, -0.09, p = 0.0009), visceral adipose tissue (MD = -21.06, 95%CI: -34.58, -7.55, p = 0.002), and subcutaneous adipose tissue (MD = -20.53, 95%CI: -29.15, -11.90, p < 0.001) levels in patients with T2DM and NAFLD. Of the 11 studies, 2 reported the occurrence of adverse reactions, which were primarily gastrointestinal. Compared with placebo and other drugs (e.g., insulin), liraglutide may improve glucose metabolism, lipid and liver function parameters, and visceral and subcutaneous fat in patients with T2DM and NAFLD, thus constituting an effective treatment for these patients.

Abstract:Glucagon-like peptide-1 receptor agonists (GLP-1RAs), through specific activation of the GLP-1 receptor, enhance insulin secretion in a glucose (GLU)-concentration-dependent manner while effectively suppressing the release of glucagon. This dual mechanism contributes to the reduction of blood GLU levels. Additionally, higher doses of GLP-1RAs can reduce food intake and increase satiety by delaying gastric emptying and suppressing the appetite center in the central nervous system. [ 1 ] Liraglutide, as a GLP-1RA, has been approved by the FDA (Food and Drug Administration) for the control of blood sugar in patients with type 2 diabetes and for weight loss in obese patients.

Abstract:BackgroundGlucagon-like peptide-1 (GLP-1) analogues have emerged as insulin secretagogues and are widely used in type 2 diabetic patients. GLP-1 analogues also demonstrate a promotion of beta cell proliferation and reduction of apoptosis in rodents. In the present study, we investigated the protection of pancreatic beta cells by early use (at the age of 2 weeks) of GLP-1 analogue, liraglutide in Gato-Kakizaki (GK) rats and explored the underlying mechanisms. MethodsThe effects of liraglutide on glucose tolerance were evaluated by intraperitoneal glucose tolerance test (IPGTT) and insulin release tests (IRT). Ki67 and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) immunostaining, Western blots and real-time polymerase chain reaction were applied to evaluate cell proliferation, apoptosis and related gene expressions. ResultsOur results demonstrated that early use of liraglutide improved glucose tolerance during liraglutide treatment in GK rats. Liraglutide increased pancreatic insulin contents and markedly reduced beta cell apoptosis. Liraglutide also downregulated pro-apoptotic gene expressions and reduced intra-islet macrophage infiltration. ConclusionsThis experiment reported for the first time that early use of liraglutide could protect beta cell failure in pre-diabetic GK rats through reduction of beta cell apoptosis and ameliorating islet inflammation.
Liraglutide Ameliorates Glycometabolism And Insulin Resistance Through The Upregulation Of Glut4 In Diabetic Kkay Mice

Abstract:Liraglutide, a long-lasting glucagon-like peptide-1 analogue, has been used for the treatment of patients with type 2 diabetes mellitus since 2009. In this study, we investigated the anti-diabetic effects and mechanisms of action of liraglutide in a spontaneous diabetic animal model, using KK/Upj-Ay/J (KKAy) mice. The KKAy mice were divided into 2 groups, the liraglutide group (mice were treated with 250 mu g/kg/day liraglutide) and the model group (treated with an equivalent amount of normal saline). C57BL/6J mice were used as the controls (treated with an equivalent amount of normal saline). The treatment period lasted 6 weeks. During this treatment period, fasting blood glucose (FBG) levels and the body weight of the mice were measured on a weekly basis. Our results revealed that liraglutide significantly decreased FBG levels, the area under the curve following a oral glucose tolerance test and insulin tolerance test, increased serum insulin levels, reduced homeostasis model assessment of insulin resistance and increased the insulin sensitivity index. Furthermore, liraglutide ameliorated glycometabolism dysfunction by increasing glycolysis via hexokinase and glycogenesis via pyruvate kinase activation. An ultrastructural examination of the pancreas revealed that liraglutide improved the damaged state of islet beta cells and increased the number of insulin secretory granules. The real-time PCR results revealed that the gene expression of glucose transporter 4 (GLUT4) increased following treatment with liraglutide. Liraglutide also upregulated the protein expression of GLUT4 in liver tissue and skeletal muscle. Our results suggest that liraglutide ameliorates glycometabolism and insulin resistance in diabetic KKAy mice by stimulating insulin secretion, increasing glycogenesis and glycolysis and upregulating the expression of GLUT4.
Efficacy and Safety of Liraglutide Versus Sitagliptin Both in Combination with Metformin in Patients with Type 2 Diabetes

Abstract:Background: The aim of this systematic review was to evaluate the efficacy and safety of liraglutide versus sitagliptin both in combination with metformin in patients with type 2 diabetes and provide reference basis for rational use of clinical drugs. Methods: Several databases were searched, including Web of science, PubMed, Cochrane library, CNKI, and Wanfang database. Only randomized controlled trials (RCTs) of liraglutide versus sitagliptin both in combination with metformin up to 31 August 2016 were included. Data were extracted independently by 2 reviewers, and a fixed or random effects model were used to analyze outcomes that were expressed as odds ratio (OR) or mean difference (MD) and 95% confidence intervals (95% CIs) for different situations. Results: Five RCTs involving 1440 participants were included. Compared with sitagliptin combination with metformin group, participants' treatment with 1.2 mg and 1.8 mg liraglutide with metformin could significantly lower the level of glycosylated hemoglobin (HbA1c) (P < .00001, MD = −0.35, 95% CI −0.51 to −0.20). Moreover, patients with 1.8 mg liraglutide group had significant body weight loss (P < .00001, MD = −1.12, 95% CI −1.54 to −0.70). However, there were no obvious differences in cutting down the systolic blood pressure and diastolic blood pressure between liraglutide-metformin and sitagliptin-metformin groups. The incidence of gastrointestinal problems was significantly higher than sitagliptin with metformin groups. Conclusion: The results of this meta-analysis indicated that Liraglutide added on to metformin therapy could significantly lower the level of HbA1c and increase body weight loss. Meanwhile, the adverse reactions such as gastrointestinal problems were common in the liraglutide treatment group. Thus, this will provide an important reference for the treatment of patients with type 2 diabetes.

Abstract:Objective:To discuss the effect of liraglutide on glycemic control and reduction of cardiometabolic risk factors in patients with newly diagnosed type 2 diabetes (T2DM) and metabolic syndrome (MS).Methods:Sixty-three cases with newly diagnosed T2DM (HbA1c 7.5%-11%) and MS were selected, and randomly divided 1∶1 into metformin group (0.85 g,oral administration, two times a day) and liraglutide group (1.2 mg, subcutaneous injection, one time a day).The primary endpoint was change in metabolic parameters.Results:Sixty patients completed the study.There was a reduction in weight,body mass index,waist circumference,fasting glycemia,HbA1c, total and LDL-cholesterol, and triglycerides in both groups(P<0.01).Liraglutide was superior to metformin in reducing weight, BMI, and waist circumference (P<0.05).There was no adverse effect in two groups.Conclusions:Liraglutide is associated with improved glycemic control and reduction of cardiometabolic risk factors in newly diagnosed T2DM and the MS.

Abstract:Dulaglutide is a long-acting glucagon like peptide-1 (GLP-1) receptor agonist which can promote insulin secretion,protect islet beta cells,inhibit glucagon secretion,inhibit gastric emptying and reduce appetite.It has been used for the treatment of type 2 diabetes and obesity,but it does not apply to type 1 diabetes to be used as an alternative to insulin.By reviewing and analyzing recent literature research of pharmacological mechanism,pharmacokinetics,pharmacodynamics,clinical study and adverse reactions of dulalutide,this paper introduces dulaglutide and promote its rational use in the treatment of type 2 diabetes.

Abstract:[Summary] Islet β-cell dysfunction and insulin resistance are two important factors for the development and progression of type 2 diabetes mellitus (T2DM ). Therefore ,protection of islet β-cell function and improvement of insulin resistance are promising in delaying the progression of T 2DM. However ,monotherapy using conventional anti-hyperglycemic agent plays a limited role in delaying the progression of T2DM. Recent studies have shown that Liraglutide as a glucagon-like peptide-1 receptor agonist can protect islet β-cell function and incresae insulin sensitivity ,thereby delaying the progression of T2DM ,through multiple mechanisms. Here we will summarize the research progress in this field in China.
Liraglutide on Type 2 Diabetes Mellitus with Nonalcoholic Fatty Liver Disease: A Systematic Review and Meta-Analysis of 16 RCTs

Abstract:Background: Nonalcoholic fatty liver disease (NAFLD) is a common comorbidity of type 2 diabetes mellitus (T2DM). Our aim is to investigate the effects of liraglutide on T2DM with NAFLD. Methods: Relevant articles published from the earliest publication to March 2022 were selected from several databases. The Cochrane Collaboration’s RevMan software was used for the analysis. Results: Sixteen studies are selected for this meta-analysis, which includes totally 634 patients in the treatment group and 630 patients in the control group. As a result, 14 studies show that fasting plasma glucose levels of the experimental group are lower than that of the control group; 15 studies show that glycosylated hemoglobin A1c levels of the experimental group are lower than that of the control group; 13 studies show that triglyceride levels of the experimental group are lower than that of the control group; twelve studies show that total cholesterol levels of the experimental group are lower than that of the control group; 10 studies show that alanine aminotransferase levels of the experimental group is lower than that of the control group; 10 studies show that no significant difference in changes in aspartate transaminase between 2 groups; 13 studies show that low density lipoprotein cholesterol levels of the experimental group is lower than that of the control group; 9 studies show that no significant difference in changes in high density lipoprotein cholesterol between 2 groups; 7 studies mentioned adverse effects and the difference is significant. Conclusion: Liraglutide is potentially curative for T2DM with NAFLD.

Abstract:Background: Liraglutide is an acylated glucagon-like peptide-1 (GLP-1) analog, and its pharmacokinetic and pharmacodynamic properties as a GLP-1 receptor (GLP-1R) agonist make it an important therapeutic option for many patients with type 2 diabetes mellitus. This study compared the bioequivalence and safety of liraglutide with the originator product in healthy Chinese adult subjects. Methods: Subjects (N = 36, both sexes) were randomized in a 1:1 ratio into two groups (18 cases each) for a two-cycle, self-crossover trial. Each cycle involved a single subcutaneous injection of the test and reference drugs, with a washout period of 14 days. The plasma drug concentration was quantified by liquid chromatography-tandem mass spectrometry (LC-MS/MS). The main pharmacokinetic parameters were statistically analyzed to assess drug bioequivalence. Furthermore, the safety of the drugs was assessed throughout the trial. Results: The geometric mean ratios of C max , AUC 0-t , and AUC 0-∞ were 103.73%, 103.01%, and 103.03%, respectively, and their 90% confidence intervals (CIs) were consistent with the range of 80.00%–125.00%, indicating that the two formulations had similar pharmacokinetics. Meanwhile, safety results showed that both drugs were well tolerated. Conclusion: Studies have shown that the test drug has similar bioequivalence and safety to the reference drug. Effects of GLP-1 Receptor Agonist on Glycolipid Metabolism and Micro Inflammatory Status in Patients with Abdominal Obesity and Type 2 Diabetes

Abstract:Objective: To explore the impact of Liraglutide, a GLP-1 receptor agonist, on glycolipid metabolism and microinflammatory status in patients with abdominal obesity and type 2 diabetes. Methods: This study consecutively enrolled 60 patients with abdominal obesity and type 2 diabetes at Jiande Hospital of Traditional Chinese Medicine from October 2020 to December 2021. They were randomly divided into a control group (30 cases) receiving routine treatment and a liraglutide group (30 cases) receiving Liraglutide in addition to routine treatment for 3 months. The control group received routine treatment, which included metformin at a dose of 1g twice daily, along with dietary and exercise guidance. The liraglutide group received an initial dose of 0.6 mg daily for the first week, increased to 1.2 mg daily in the second week, and further increased to 1.8 mg daily in the third week, continuing at this dose until the end of the 12-week treatment period, with adjustments based on tolerance. Results: Significant improvements were observed in SBP, DBP, BMI, waist-to-hip ratio, and various glycolipid and microinflammatory indexes in the Liraglutide group compared to controls (P < .05). Specifically, SBP, DBP, BMI, waist-to-hip ratio, FPG, 2hPG, HbA1c, TG, TC, hs-CRP, TNF-α, and IL-6 levels decreased more significantly in the liraglutide group. Conversely, C-peptide and fasting insulin levels increased more in the liraglutide group. No significant difference in the incidence of adverse reactions was observed between the two groups (P > .05). Conclusion: Liraglutide can improve glycolipid metabolism and microinflammatory status in patients with abdominal obesity and type 2 diabetes, which has high medication safety.

Abstract:Type-2 diabetes mellitus (T2DM) is a complicated long-term disorder associated with metabolism that is identified by insulin resistance, imbalance in glucose regulation and reduced secretion of insulin. GLP-1(Glucagon-like peptide-1) is an incretin mimetic that has excellent effects on the regulation of blood glucose levels and also the management of disorders associated with vital organs. GLP-1 agonist is an effective class of drug for the treatment of type-2 diabetes mellitus and associated complications. Liraglutide is one of the potent drugs of this class having similar effects as biological GLP-1. This review includes clinical trials and patents related to the pharmaceutical formulation, synthesis and biological action of liraglutide.
Liraglutide Reduces the Body Weight and Waist Circumference in Chinese Overweight and Obese Type 2 Diabetic Patients

Abstract:Aim: To investigate the effects of liraglutide, a glucagon-like peptide-1 (GLP-1) receptor activator, on body weight and waist circumference in Chinese overweight and obese type 2 diabetic patients. Methods: A total of 328 Chinese overweight and obese type 2 diabetic patients were included in this multi-center, open-labeled and self-controlled clinical study. The patients were subcutaneously injected with liraglutide once daily for 24 weeks as add-on therapy to their previous hypoglycemic treatments. Statistical analyses were performed using SPSS software package version 11.5 for Windows. Results: Liraglutide treatment caused significant reduction of the mean body weight (from 86.61±14.09 to 79.10±13.55 kg) and waist circumference (from 101.81±13.96 to 94.29±14.17 cm), resulting in body weight lose of 5%–10% in 43.67% patients, and body weight loss above 10% in 34.06% patients, who had significant lower plasma creatinine levels. Baseline waist circumference, BMI and HOMA-IR were independently correlated with the body weight loss. Furthermore, liraglutide treatment significantly decreased HbA1c levels (from 8.66%±2.17% to 6.92%±0.95%) with HbA1c<7.0% in 35.37% patients, who had a significantly lower baseline level of HbA1c, but higher baseline levels of C peptide and glucagon. Moreover, liraglutide treatment resulted in greater body weight loss in patients with a long duration of diabetes, and better glycemic control in patients with a short duration of diabetes. Conclusion: Liraglutide significantly reduces body weight and waist circumference in Chinese overweight and obese type 2 diabetic patients. Patients with apparent visceral obesity, insulin resistance and a long duration of diabetes may have greater body weight loss; whereas patients with high insulin-secreting ability, hyperglucagonemia, and short-duration diabetes may obtain better glycemic control with liraglutide.
