The Effect of Thalidomide on Serum VEGF、bFGF and TNF-α in Patients with Non-Small Cell Lung Cancer and Their Clinical Significance

Abstract:Background and purpose:Thalidomide possesses the effect of antiangiogenesis.The purpose of this study is to evaluate the response rate in patients with advanced non-small cell lung cancer(NSCLC),who were treated with thalidomide plus vinorelbine and cisplatin(NP combined group)or NP alone(chemotherapeutic group)and to compare the levels of serum VEGF、bFGF and TNF-α of the patients before and after treatment.Methods:A total of 66 patients with advanced NSCLC were randomized into either trial group:combined group(vinorelbine 25 mg/m2 on day 1 and day 8,cisplatin 70-80 mg/m2 on day 1,thalidomise 200 mg daily from day 1)or chemotherapeutic group:NP regimen alone.The concentrations of the serum VEGF、bFGF and TNF-α were measured by enzyme-linked immunosorbent assay respectively in patients with NSCLC before and after treatment.Results:Of 66 assessable patients,overall response rate was 51.5% in combined group and 36.4% in chemotherapeutic group,respectively(P=0.22).The levels of serum VEGF in combined group were significantly decreased after treatment in effective cases(P0.05).There was no significant difference between the levels of serum VEGF before and after treatment in chemotherapeutic group with treatment response(P0.05).But the levels of serum VEGF and bFGF were significantly higher after the treatment of the two groups in the ineffective cases(P0.05).The levels of serum TNF-α were significantly decreased after treatment in combined group(P0.05).There were no significant differences between levels of serum TNF-α before and after treatment in chemotherapeutic group.Conclusions:The addition of thalidomide to NP regimen results in a more effective response rate than in NP alone group with advanced NSCLC.Thalidomide can inhibit tumor cell to generate VEGF and TNF-α,and cut down the levels of VEGF and TNF-α.
