Perioperative management of lung transplant recipients at a single center: analysis of 105 cases

Abstract:Objective To evaluate the prophylaxis and treatment of complications of lung transplantation (LT) during perioperative period.Methods From September 2002 to April 2011, a total of 105 cases of LT were performed at a center, including single lung transplant in 73 cases and bilateral sequential lung transplant in 32 cases, 63 of which were operated under circulation support. All of the patients received mechanical ventilation, immunosuppressive agents, antibacterial prophylaxis, and prevention of primary graft dysfunction (PGD) in the ICU ward after surgery. Patients undergoing PGD were scored by using the International Society for Heart and Lung Transplantation grading system and were accordingly treated with negative fluid balance, prolonged mechanical ventilation, prostaglandin E1, and extracorporcal membrane oxygenation.Results The perioperative survival rate was 81.9% (86/105). Nineteen patients died at an early stage after LT owing to pulmonary infection in 10 cases, PGD in 6 cases, and pulmonary embolism, acute rejection and stoma fistula in 3 cases respectively. Complications included lung infection (12 cases), PGD grading 3 (10 cases), acute rejection (3 cases), tracheostenosis (10 cases), stoma fistula (4 cases), hemorrhage (3 cases), pulmonary embolism (3 cases), pulmonary artery stenosis (1 case), and deep vein thrombosis (1 case). The long-term survivors all had a good quality of life with excellent cardiorespiratory function.Conclusion Appropriate management such as antibacterial prophylaxis, use of immunosuppressive agents, negative fluid balance, and prolonged mechanical ventilation can reduce incidence of complications and mortality after LT.
