Abstract:Objective:To introduce the serious adverse drug reactions (ADR) of antitumor medicines commonly used in clinic and the prevention and treatment methods.Methods:Serious adverse drug reactions of commonlyused antitumor medicines in clinic were summarized and analyzed according to the related parts of package inserts, and further elaborated with clinical practice.Results and Conclusion:The incidence of adverse drug reaction is higher in antitumor medicines than in other drug categories,and the ADRs of antitumor medicines have greater impact on the body,therefore prevention and treatment measures should be taken in order to avoid or to alleviate the impairment of ADRs on patients in order to improve the therapeutics effects and life quality in cancer patients.

Abstract:OBJECTIVE: To summarize the prevention and management of ADRs induced by antitumor drugs to provide references for their rational administration in the clinic. METHODS: A summarization and analysis were carried out regarding the toxic reactions of digestive system, hematopoietic system, urinary system, liver, heart, lung, nervous system and allergic reactions etc by reviewing the related literatures. RESULTS CONCLUSION: Antitumor drugs can lead to multisystem ADRs of different degree and different types, therefore, great importance should be attached to its early prevention, close monitoring and timely management to enhance efficacy of drugs and relieve pains of patients.
Retrospective Analysis of 661 Cases of Adverse Drug Reactions Induced by Antitumor Drugs

Abstract:Objective:To probe into the characteristics of adverse drug reactions(ADR)caused by antitumor drugs and to provide some rational clinical guidance.Method:661 reports of adverse reactions caused by antitumor drugs were collected in a 3A-Grade Hospital from January 2004 to December 2008 and statistically analyzed in respect of patients' sex and age,involved drugs,routes of administration,clinical manifestations,rating of correlated factors,etc.Result: Of the 661 cases,65.5%were male cases(430)and 34.95%,female cases;191 cases(38.43%)were the elderly at the age of more than 60 and 1145 case-times were involved in 64 kinds of antitumor drugs.The incidence rate of ADRs induced by cisplatin(210 case-times),fluorouracil(135 case-times)and oxaliplatin(124 case-times)took the front 3 places in the sequence.Drugs for intravenous administration resulted in the highest rate with 1027 case-times(89.69%). 1566 adverse reactions occurred,of which there were 773 cases of digestive system lesion(49.36%)that ranked the first. The incidence rate of new and severe ADR was 36.6%,accounting for 242 cases.Conclusion:The collection and arrangement of adverse drug reactions and analysis and evaluation of their basic information were helpful to ensure and promote safe and rational clinical administration.
Clinical analysis of 1946 cases of adverse drug reactions associated with chemotherapy in cancer patients

Abstract:Objective: To analyze the incidence and influence factors of adverse drug reaction (ADR) in cancer patients, and provide an effective basis for rational use of drugs in clinical oncology. Methods: The adverse drug reaction reports in our hospital during 2011 – 2016 were analyzed retrospectively, including the age and gender of the patients, the route of administration, the classification and ranking of drugs causing ADR, and the type of ADR. Results: There were 1946 cases of ADR in 2011 – 2016. In 6 years, the proportion of men and women was 1 : 1.02; the age of patients with ADR mostly concentrated in the 51 – 60 years old (590 cases, 30.32%); paclitaxel associated ADR were the most (328 cases, 11.57%), the most route of administration of those ADRs was intravenous infusion (2338 cases, 82.47%); the system and organ damages were involved in 2696 cases of ADR, including systemic damage, blood system damage, damage of skin and its accessories, etc. Conclusion: The clinical data should be combined with the results of relevant data, and the related factors of ADR in the process of cancer treatment should be paid attention to. The regular monitoring and reporting should be paid attention in order to reduce the occurrence of ADR.

Abstract:In recent years, considerable achievements have been made in pediatric oncology with the innovation and development of antitumor drugs. However, compared to adults, children as a special group have not yet matured fully in terms of liver and kidney function. Moreover, pediatric patients are prone to more adverse drug reactions (ADRs) from the accumulation of antineoplastic drugs due to their smaller body size and larger body surface area. Chemotherapy-related ADRs have become a non-negligible factor that affects cancer remission. To date, studies on ADRs in pediatric cancer patients have emerged internationally, but few systematic summaries are available. Here, we reviewed the various systemic ADRs associated with antitumor drugs in children and adolescent patients, as well as the advances in strategies to cope with ADRs, which consisted of neurotoxicity, hematological toxicity, cardiotoxicity, ADRs of the respiratory system and gastrointestinal system and urinary system, ADRs of the skin and its adnexa, allergic reactions, and other ADRs. For clinicians and researchers, understanding the causes, symptoms, and coping strategies for ADRs caused by anticancer treatments will undoubtedly benefit more children.

Abstract:Objective To analyze the characteristics and general rules of adverse drug reaction (ADR) in Beijing Cancer Hospital of Peking University ("our hospital" for short) in order to provide reference for further developing clinical pharmaceutist guiding rational drug use,and reduce the incidence of ADR.Methods The ADRs reports in our hospital from September 2015 to September 2016 were downloaded from the national ADR monitoring system,and the ADRs of antineoplastic drugs were screened,and the data was taken statistics and analysis by excel software.Results Among 273 cases of ADRs caused by antineoplastic drugs reported from September 2015 to.September 2016,there were 19 cases of ADRs caused by auxiliary antineoplastic drugs.The male to female ratio of ADRs was 0.9∶1,the age group was mainly concentrated in 50-＜60 years old,total 92 cases (33.70％).The main route of administration causing ADRs was intravenous dripping (255 cases,93.41％).The organs or systems involved by ADRs were constitutional reaction (27.97％),skin reaction (16.52％) and cardiovascular system reaction (13.22％).The main clinical symptoms were allergic reactions (51 cases,11.23％),shortness of breath (33 cases,7.27％),fever (28 cases,6.17％),chest distress (27cases,5.59％) and skin rush (26 cases,5.73％).The highest incidence rate of ADR drugs were Oxaliplatin (71 cases,20.70％) and Paclitaxel (57 cases,16.62％).Conclusion Clinical pharmacist should take initiative to monitor ADR of antineoplastics drugs.Combined with detection of pharmacogenomics,clinical pharmacist should formulate individualized treatment plan,so as to promote rational drug use,prevent and reduce the incidence of ADRs.

Abstract:Abstract Background The adverse reactions (ADRs) of targeted therapy were closely associated with treatment response, clinical outcome, quality of life (QoL) of patients with cancer. However, few studies presented the correlation between ADRs of targeted therapy and treatment effects among cancer patients. This study was to explore the characteristics of ADRs with targeted therapy and the prognosis of cancer patients based on the clinical data. Methods A retrospective secondary data analysis was conducted within an ADR data set including 2703 patients with targeted therapy from three Henan medical centers of China between January 2018 and December 2019. The significance was evaluated with chi-square test between groups with or without ADRs. Univariate and multivariate logistic regression with backward stepwise method were applied to assess the difference of pathological characteristics in patients with cancer. Using the univariate Cox regression method, the actuarial probability of overall survival was performed to compare the clinical outcomes between these two groups. Results A total of 485 patients were enrolled in this study. Of all patients, 61.0% ( n = 296) occurred ADRs including skin damage, fatigue, mucosal damage, hypertension and gastrointestinal discomfort as the top 5 complications during the target therapy. And 62.1% of ADRs were mild to moderate, more than half of the ADRs occurred within one month, 68.6% ADRs lasted more than one month. Older patients ( P = 0.022) and patients with lower education level ( P = 0.036), more than 2 comorbidities ( P = 0.021), longer medication time ( P = 0.022), drug combination ( P = 0.033) and intravenous administration ( P = 0.019) were more likely to have ADRs. Those with ADRs were more likely to stop taking ( P = 0.000), change ( P = 0.000), adjust ( P = 0.000), or not take the medicine on time ( P = 0.000). The number of patients with recurrence ( P = 0.000) and metastasis ( P = 0.006) were statistically significant difference between ADRs and non-ADRs group. And the patients were significantly poor prognosis in ADRs groups compared with non-ADRs group. Conclusion The high incidence of ADRs would affect the treatment and prognosis of patients with cancer. We should pay more attention to these ADRs and develop effective management strategies.

Analysis of Rational Drug Use in Severe Adverse Drug Reaction Cases during Lung Cancer Chemotherapy

Abstract:OBJECTIVE:To explore and analyze the drug use in lung cancer inpatients suffering severe adverse drug reactions (ADR) during chemotherapy.METHODS:A total of 132 lung cancer inpatients suffering severe ADR collected from our hospital from Jun.2006 to Sept.2010 were analyzed statistically in respect of chemotherapy regimens and drug combination.RESULTS:132 inpatients received chemotherapy for 604 times,and the incidence of severe ADR was 39.57% (239 cases).The inappropriate choice of chemotherapy regimens for patients with liver or renal dysfunction mainly resulted from the selection of chemotherapeutic drugs (63 cases).The irrational drug combination were mainly seen in combined toxicity between chemotherapeutic drugs and other drugs (205 cases),followed by chemotherapy drugs which were mainly metabolized by the liver through cytochrome P4503A4 enzyme combined with inhibitor or revulsant(180 cases).CONCLUSION:Inappropriate drug combination is quite common during chemotherapy.Clinical physicians should be familiar with ADR and drug interaction of drug combination.ADR monitoring should be strengthened to promote rational drug use during lung cancer chemotherapy.

Abstract:OBJECTIVE: The compatibility of anti-cancer drugs was studied to provide information for rational clinical drug usage and drug safety in patients. METHODS: Based on datum as package inserts, the compatibility and interaction of commonly used anti-cancer drugs were summarized, and some related clinical problems of drug usage were sorted and analyzed. RESULTS & CONCLUSION: Physicians and pharmacists should pay more attention to the compatibility, contraindication and interaction of drugs, and be very strict with prescription, verification and preparation.

Abstract:OBJECTIVE:To explore the reules and characleristics of ADR reduced by fluorouracil,capecitabine,epirubicin, cisplatin,irinotecan,paclitaxel,oxaliplatin and docetaxel for patients with gastric cancer. METHODS:The Chinese and English lit-eratures about the reports of fluorouracil,capecitabine,epirubicin,cisplatin,irinotecan,paclitaxel,oxaliplatin and docetaxel in the treatment of gastric cancer were extracted,and statistical analysis was carried out for the included studies. RESULTS:Totally 306 were chosen,including 511 patients involving 663 cases. The ration of male to female was 1.30∶1,227 cases aged 41-60 years old (accounting for 48.19%),ADR mainly included gastrointestinal damage(273 cases,accounting for 41.18%),cardiovascular system damage(156 cases,accounting for 23.5%),respiratory system(76 cases,accounting for 11.46%),mainly fifth were nausea (82 cases),vomiting(78 cases),bone marrow suppression(69 cases),diarrhea(52 cases)and digestive system reaotions(41 cas-es). 49.90% patients showed drug adverse reactions within 30 minutes,most patients were improved to heal. CONCLUSIONS:Prompt medical workers pay attention to patients age,sex,and the accumulative system in treatment with stomach cancer ,reduce the occurrence of adverse drug reactions.
Analysis of 281 Adverse Drug Reaction Reports of Antineoplastics Drugs in Our Hospital

Abstract:OBJECTIVE:To investigate the characteristics and regularity of adverse drug reactions(ADR)of antineoplastic drugs in our hospital.METHODS:281 ADR cases reported by our hospital from Jan.2009 to Apr.2011 were collected and summarized for analysis.RESULTS:A total of 281 ADR cases involved 52 kinds of antineoplastic drugs.Intravenous dripping was main route of administration(260 cases,82.02%).88.26% occurred in patients older than 40(248 cases).ADR mostly manifested as lesions of cardiovascular system in 172 cases,accounting for 25.04%.The incidence of top 13 drugs in ADR list accounted for 64.35% of the total,in which oxaliplatin ranked the first.CONCLUSION:ADR monitoring of antineoplastics drugs should be strengthened in cancer hospitals and the anti-allergic pretreatment of antineoplastics drugs should be standardized.

Abstract:Objective To investigate and analyze the characteristics of adverse drug reaction(ADR) in a 3A hospital from 2005 to 2011.Methods 10 097 ADR cases reported in our hospital were analyzed retrospectively regarding patients'sex and age,classification of ADR,pharmacological classification,the form of a drug,systems and organs involved in ADR and its clinical manifestations.Results Numbers of ADR cases were on the rise from 2005 to 2011,males in age of 21 or 70 had higher risk of ADR than females.Anti-infective drugs(32.05%) occupied the highest proportion in ADR,followed by anti-tumor drugs(29.81%).Powder or injection types of drug were the main forms of drugs which led to ADR,especially in chemical medicine(86.2%).Oral administration of traditional Chinese medicine had a 30.7% of proportion of ADR which was higher than that in chemical medicine(12.4%).The major systems involved were alimentary system(33.38%).Conclusions It is important to concentrate on the rational use of anti-infective drugs,decrease the intravenous route and focus on the characteristics of the juveniles or old people above 70 years old especially males in order to reduce or avoid the occurrence of ADR.

Abstract:OBJECTIVE:To analyze the causes of adverse drug reactions(ADRs) induced by anti-osteoporosis drugs.METHODS:Individual case reports and review articles about the anti-osteoporosis drug-induced ADR reported in journals both at home and abroad during the period 2000～2008 were retrieved.RESULTS:The incidence of anti-osteoporosis drug-induced ADRs was low and the degree was light,which manifested chiefly as discomfort of gastrointestinal tract.CONCLUSION:The reduction of the incidence of anti-osteoporosis drug-induced ADRs relies on rational combination and combined drug use.Great attention should be attached to the safety of anti-osteoporosis drugs if used for a long term.

Abstract:Objective:To explore the features of the adverse drug reaction (ADR) reports in our hospital.Method:A retrospec- tive study was applied to quantitatively analyze the 990 ADR reports spontaneously reported by medical workers rfrom 2002 to 2005 in our hospital.Result:The number of ADRs were increased year by year.1174 kinds of 25 categories of drngs were suspected,45.9% of which was anti-infective agents,second was anti-tumor agents;29.5% of the anti-infective agents were quinolones,26.7% be- longed to cephalosporins and 15.6% was penicillins;The most common displays of the ADRs was the lesion of skin and the appenda- ges,second was diseases of the digestive system;59.3% of them were cured,39.0% turned better,only 0.5% had sequelae or died; 13.0% of them were certain about causality assessment,65.4% probable,and 5.4% possible.Conclusion:It must be strengthen mo- nitoring of ADR,decrease the percentage of failing to report,and improve the quality of ADR reports gradually.

Abstract:Objective To analyse and summarize the status of adverse drug reactions(ADR) in our hospital in order to provide the reference for reasonable drug application in clinic and correct evaluation of ADR occurrence.Methods 468 cases of ADR were statistically analyzed in Shougang Hospital affiliated to Beijing University.Results ADR were associated with 263 drugs.The preceding three positions were common anti-infective drugs(232 cases,49.57%),cardiovascular agents(53 cases,11.32%) and traditional Chinese medicines(31 cases,6.62%) in turn.Conclusion Monitoring ADR is the effective method to prevent the occurrence of ADR,promote reasonable drug application and ensure the safety of drug use.

Abstract:Objective:To review and analyze the occurrence and characteristics of adverse reactions resulted from anti-infection drugs in our hospital and to provide references for the rational use of drugs in the clinic.Methods:ADR reports collected in our hospital caused by anti-infection drugs in 2008 year were analyzed statistically in terms of patients' age distribution,route of administration,drug varieties and the specific clinical manifestations.Results:Of the total cases induced by anti-infection drugs,most were female patients(60.96%);most of them were induced by ivgtt(97.33%).The most common clinical manifestations were the damage of skin and adnexa,digestive system and nervous adverse reaction.Conclusion:Rational use of anti-infection drugs and monitoring of the administration should be emphasized in clinic for the prevention and reduction of the occurrence of ADR.

Abstract:Objective To study and analyze the characterstics and rules of adverse drug reactions/event(ADR/ADE) for rational use in clinic.Methods A total of 342 ADR/ADE reports in our hospital collected from 2010 to 2011 were described and analyzed retrospectively in respect of patient gender,age,drug categories and clinical manifestation,ect..Results In 342 ADR/ADE cases,females were more than males;Patients between 51 to 81 years old age accounted for 55.85% in the first place,children between 0 to 17 years old age accounted for 15.50%;The drugs that induced ADR/ADE totaled 41 kinds,with anti-infectives showing the highest proportion(69.01%),followed by traditional Chinese medicine injection(18.72%);The main manifestation of the ADR/ADE was lesions of skin and its appendages(28.36%);Route of administration by intravenous infusion were accounted for the largest proportion(87.72%).Conclusion Anti-infective drugs and traditional Chinese medicine injection should be used rationally.Attach importance to the analysis of ADR/ADE data and information timely feedback to the clinical,to avoid or decrease repeat occurrence of ADR/ADE,so as to ensure the rational clinical drugs application and safety.
A Retrospective Study on 990 ADR Case Reports in Our Hospital During 2002～2005

Abstract:OBJECTIVE: To study the features of the adverse drug reaction(ADR) in our hospital and to promote rational use of drugs.METHODS: A retrospective analysis was made of 990 ADR reports submitted during 2002～ 2005 by our hospital' s medical staff on their own initiative.RESULTS: The ADR reports from our hospital grew year-on-year both in the total number and the number by category.Of the 990 ADR cases,9.4% were new and common cases;0.8% ,new and serious cases;16.3%,serious cases.Skin and its appendages suffered most from ADR impairment,followed by the digestive system.Antiinfectives topped with a 45.9% proportion of the suspected drugs,followed by anticancer agents.Injections and IV administration had caused most of the ADR cases.Except for 2 death cases,all ADR reports were submitted within the stipulated time limit.CONCLUSIONS: It is suggested that ADR monitoring be further strengthened,and that the quality of ADR reports be upgraded.
