Surgical treatment of thoracolumbar vertebral burst fracture: anterior approach versus posterior approach

Abstract:Objective To investigate the curative effects on thoracolumbar burst fracture treated by anterior and posterior approach surgery,and evaluate their superiority.Methods From January 2005 to January 2008,38 cases with thoracolumbar burst fracture were summarized.Among them,20 cases underwent posterior decompression,bone graft and internal fixation,while 18 cases underwent anterior decompression,bone graft and internal fixation.All cases were followed up to observe the height of injured vertebrae,the change of kyphosis angle and the consequence of internal fixation instrument,grafted bone and degree of spinal canal.Results All cases were followed up respectively for 6~36 months with average of 18 months.Anterior surgical group gained full bony union without loss of vertebral height according to radiography,while in the posterior surgical group had complication of vertebral height loss,kyphosis deformity as well as internal fixation failure in another 3 cases.Spine canal apparently stenosis was found in posterior surgery cases.Conclusion Anterior approach is much superior to the posterior approach in the surgical treatment of thoracolumbar burst fracture.
