The Prevention and Treatment for the Framework Exposure Following Auricular Reconstruction

Abstract:Objective By analysis of the region,postoperative period and causality of framework exposure after auricular reconstruction,we want to find the methods for its prevention and treatment.Methods A retrospective analysis was performed for thirty-two patients who suffered from framework exposure after auricular reconstruction.The subcutaneous tissue and temporal superficial fascia flaps combining with split skin graft transplantation,temporal superficial scalp flaps,and neighboring flaps were used in the treatment of framework exposure.Results The framework exposure was mostly found within one month after auricular reconstruction.It occurred mostly in the posterior-superior margin of the helix,followed by the junction of the ear vestige and expanded skin flap in the helix.The excess tension of flaps,inappropriate bandaging,a sharp edge of frameworks and flap damage were the main causes of framework exposure.All the skin deficits on the autogenous costal cartilage framework were repaired after operation,and there were no infection and framework exposure once again.And the appearance of the reconstructed ear was esthetically pleasing too.The skin deficits on the Medpor framework were repaired successfully in four patients,however,the Medpor frameworks were removed after two surgical attempting corrections in other two patients because the framework exposures were still existed.Conclusions The location liable to induce frameworks exposure should be focused on during operation.The reconstructed ear should be closely evaluated for risks for one month postoperation.Only if it is promptly found and appropriate surgical management is applied,the framework exposure could be prevented and cured completely.The framework exposure rate is higher in the patients reconstructed with a Medpor framework than with an autogenous costal cartilage framework.Medpor framework exposure is difficult to deal with,so that Medpor frameworks should not be used in the patients who have sufficient costal cartilage.
