Major Vascular Injury During Anterior Iumbosacral Surgery

Abstract:Objective To identify the risk factors and prevention measurements of major vascular injury in patients treated with anterior lumbosacral surgery.Methods Totally 85 patients undergoing anterior lumbosacral spinal surgery between October 2003 and July 2007 were included in the study,including 47 males and 38 females aged 12 to 66 years.Tuberculosis was present in 44 cases,tumor in 6,intervertebral discitis in 9,fracture in 20,and hemivertebrae in 6.Sixty-four patients underwent anterior surgery by a paramedian incision through the medial edge of the rectus abdominal muscle and retroperitoneal route,18 by a midline transperitoneal approach,and 3 by a lower abdominal oblique muscle splitting approach respectively.All patients were evaluated for the major vascular injury during surgery and in the early postoperation period.Results Five patients(5.8%)were identified as having major vascular injury,including common iliac vein laceration in 3 patients(3.5%),left internal iliac vein laceration in 1(1.2%),and common iliac vein thrombosis in 1(1.2%).The above-mentioned vascular injury occurred in 3 cases(6.8%)with tuberculosis,1 (16.7%)with tumor,and 1(11.1%)with intervertebral discitis.Fifty-nine patients were followed up for an average of 23 months,and all of them returned to normal life.Conclusion Wide exposure and careful handling of the vascular structures can help control the major vascular injury during anterior lumbosacral surgery.
