Clinical analysis of meningeal carcinomatosis secondary to lung cancer

Abstract:Objective To investigate the clinical features of meningeal carcinomatosis secondary to lung cancer.MethodsThe clinical symptom,cerebrospinal fluid examination,imaging,PS score and prognosis of 18 patients with meningeal carcinomatosis caused by lung cancer,diagnosed by CSF cytological detection were analyzed.ResultsThe histopathological types of the primary tumor were adenocarcinoma in 55.6% patients.Most of the onset symptoms in these patients were dizziness and headache.The cerebrospinal fluid cytological positive ratio of first time was low(44.4%).The abnormal elevation of lactate dehydrogenase and carcinoembryonic antigen in cerebrospinal fluid were found respectively in 13 patients(72.2%)and 11 patients(61.1%).CT/MRI with enhancement is very helpful in the establishment of diagnosis.Most patients had poor KPS scores(median,52.5 scores).And the time from the onset to the date of diagnosis of meningeal carcinomatosis ranged from 0.5 to 4 months(median,1.8 months).Survival time from diagnosis of meningeal carcinomatosis ranged from 0.3 to 14 months(median,3.6months).ConclusionsCerebrospinal fluid cytology and enhanced CT/MRI scan are the most important ways to diagnose meningeal carcinomatosis secondary to lung cancer.The abnormal elevation of lactate dehydrogenase and carcinoembryonic antigen in cerebrospinal fluid could be an assistant index to decide the diagnosis.
