Abstract:Objective To assess the clinical value of transcranial sonography (TCS) in diagnosis of Parkinson's disease (PD). Methods Eighty PD patients (PD group) and 80 normal people (control group) underwent TCS. The echo singal intensity of the nigra was classified into Ⅰ—Ⅴ as half quantitative analysis. When the echo singal intensity of the substantia nigra ≥Ⅲ,the echo enhanced area >0.20 cm2 and the ratio of substantia nigra hyperechogenicity to the area of the whole midbrain (S/M) ≥7%,the patient was thought to be with PD. Results TCS detected more patients (58/80,72.50%) with substantia nigra hyperechogenicity in PD group than those in control group (10/80,12.50%). In PD group, the median and the quartile of the area of bilateral substantia nigar hyperechogenicity was 0.46 cm2 and 0.69 cm2,those of S/M was 10.00 and 14.00,respectively,greater than those of control group (both P<0.01). The sensitivity and the speciality of TCS on diagnosis of PD was 72.50% and 87.50%,respectively. Conclusion According to the change of nigra echogenicity,TCS may provide some valuable information for clinical diagnosis of PD,the main limiting factor is that some individuals are unsuitable to the exam for their unadequated temporale bone windows.
