1127: Contrast-Enhanced Ultrasound for Treatment-Based Classification of Abdominal Parenchymal Organs Trauma

Abstract:The purpose was to retrospectively analyze the value of contrast-enhanced ultrasound (CEUS) in treatment-based classification of abdominal parenchymal organs trauma. This study was approved by the local ethical committee and had institutional review board approval; all patients gave their written informed consent. One hundred and five patients with abdominal parenchymal organ trauma, who received CEUS, were analyzed. The results of CEUS were compared with those of CT and/or operation. Treatment modalities included percutaneous injection therapy, conservative treatment and operation. The percutaneous injection therapy at bedside was done: under the guidance of CEUS, firstly Haemocoagulase Atrox was injected with multi-point into the injury site of liver, spleen or kidney; then, α-cyanoacrylate was injected into the injury and active hemorrhagic sites of liver or spleen. The conservative treatment was performed by keeping bed rest, intravenous fluid and proper antibiotics. CEUS findings of all 105 patients were consistent with those of CT scanning and/or operation (consistency analysis, P<0.001). 55.2% (58/105) received the percutaneous injection treatment under the guidance of CEUS, 26.7% (28/105) received conservative treatment, and 18.1% (19/105) had to receive operation using rapid classification by bedside CEUS. CEUS can exactly evaluate hemorrhagic trauma at bedside. So it can classify the patients with abdominal parenchymal organs trauma for reasonable management. This study may suggest a more rapid and effective imaging modality for the early rescue of abdominal trauma.
