The Role of Adjuvant Chemoradiotherapy in Urothelial Ureter Cancer

Abstract:Objective Carcinoma of the ureter is a relatively uncommon cancer.Local with/or distant recurrence are the major concerns after surgery alone.Our purpose is to assess the role of adjuvant chemoradiotherapy in urothelial ureter cancer.Methods 10 patients with T3/T4 or nodal involvement pathology confirmed transitional cell carcinoma underwent surgery followed by adjuvant radiotherapy(intraoperative radiotherapy combined with postoperative external beam radiotherapy)and M-VAC chemotherapy.The median IORT dose was 14 Gy.The median external beam radiation dose was 41 Gy.Results Median follow-up time was 4.31 years.The overall survival in the entire series at 1,3,and 5 years after radiotherapy was 80%,60%,and 36%,respectively.The estimate locoregional control rate at 1,3,and 5 year was 80%,60%,and 50%,respectively.The distant metastasis free survival rate at 1,3,and 5 years was 70%,70%,and 47%,respectively.Histological grade,and residual disease was the prognostic factors for overall survival.Conclusion In patients with locally advanced urothelial ureter cancer,overall survival and distant metastasis-free survival maybe improved with adjuvant chemoradiotherapy.Most of the patients could tolerate the treatment.High grade and incomplete surgical excision were associated with poor survival.
