Clinical Manifestations and Pathological Features of Autoimmune Hepatitis

Abstract:Objective In order to investigated the clinical and pathological characteristics of autoimmune hepatitis (AIH) and provide some guidance for its diagnosis and therapy.Methods According to the revised criteria for diagnosis of autoimmune hepatitis proposed by the International Autoimmune Hepatitis Group,77 cases of patients with AIH were selected and the clinical manifestations,pathological change and response to therapy were investigated.Results The age distribution showed a single peak in the fifties and the main clinical manifestations were abdominal distension,anorexia,jaundice and so on.The gamma globulin was 32.4±9.8 g/L and IgG was 22.8±3.0 g/L,which were obviously higher than normal level.The incidence of anti-nuclear antibody (ANA) and anti-smooth muscle antibody (SMA) were 74% and 32%,respectively.Over fifty percent patients suffered from the concurrent autoimmune disease out of liver.Twenty-six patients underwent liver biopsy.The main histologic change was interface hepatitis (65%),lobular hepatitis,rosette of liver cells and bridging necrosis were observed in heavy cases.According to the criteria proposed by the IAIHG,fifty patients (64.9%)had a complete response to immunosuppressive therapy,seventeen patients had partial response and thirty-two patients (41.6%) sustained remission.Conclusion Autoimmune hepatitis mainly affects middle-aged women and is characterized by the presence of circulating autoantibody and concurrent autoimmune disease out of liver;Interface hepatitis is the main histologic change;Immunosuppressive therapy is effective on autoimmune hepatitis.
