Abstract:Statins are a sort of widely used oral lipid-lowering drugs in clinic, which are mainly used for reducing the level of low-density lipoprotein cholesterol in plasma. The role of statins in the treatment of cardiovascular, cerebrovascular and metabolic diseases has been veriifed by a lot of research work, especially its important role in primary and secondary prevention for the cardiovascular and cerebrovascular diseases. Statins can also be used for the inhibition of renal cell proliferation, anti-tumor, anti-inlfammatory therapy and for the treatment of osteoporosis, prevention of dementia and immunosuppression. With the widespread use of statins, and long-term continuous medication recommended by international guidelines, it is signiifcant to understand the adverse reactions of short-term and long-term in our daily clinical practice. Data on the safety research of statins are summarized so as to provide references for clinical medication.
Effects and Safety Assessment of Nine Lipid-Lowing Agents

Abstract:Objective To assess the effectiveness and safety of nine lipid lowing agents in the national essential drug list (2000) and provide evidence for the adjustment and selection of essential drugs Methods Based on principles of health technology assessment (HTA) and evidence based medicine, we searched for all published clinical studies about these drugs from the following databases: MEDLINE (19662002.8), The Cochrane Library, EMBASE (19742002), CBMdisk (19792002.8) and VIP (19892002.8), the database of National Center for Adverse Drug Reaction(ADR) Monitoring of China and the database of WHO Uppsala drug monitoring center. Included studies were appraised, analyzed and compared for the reduction of triglyceride (TC) or low density lipoprotein (LDL C), the prevention for the coronary events and the incidence of ADR. Results The results from comparative trials for lipid lowing agents showed that the equivalent dose of statins for 25% reduction of LDL C was atorvastatin 10 mg/d, simvastatin 20 mg/d, pravastatin 40mg/d, lovastatin 40 mg/d, cerivastatin 0.3 mg/d and fluvastatin 80 mg/d. It was difficult to compare fenofibrate with gemfibrozil, acipimox with statins or fibrates based on available data. The study on the primary and secondary prevention of cardiovascular events showed that pravastatin and lovastatin were effective in primary prevention, and long term use could reduce the incidence of cardiovascular disease.Gemfibrozil could reduce the mortality from coronary heart disease (CHD) but the overall mortality was not changed. Pravastatin, simvastatin, atorvastatin, fluvastatin, gemfibrozil and fenofibrate had a confirmed effect in secondary prevention. Data from large scale clinical trials and the reports from ADR monitoring center of England, America, Canada and Australia suggested that the statins which had rare ADR were safe and tolerated. Rhabdomyolysis was rare but had a serious adverse reaction associated with statins. The rate of fatal rhabdomyolysis related to cerivastatin was the highest among 6 statins. The safety of simvastatin, lovastatin and atorvastatin was lower than cerivastatin but higher than simvastatin and atorvastatin. The number of ADR reports of fenofibrate was fewer than that of gemfibrozil. Conclusions At present, the best evidence focused on pravastatin, simvastatin and lovastatin are widely used and have a confirmed safety and efficacy. Atorvastatin, fluvastatin and fenofibrate still need more data to confirm their effects on coronary heart disease prevention. The drugs which were shown to be inferior or insufficient evidence are cerivastatin, gemfibrozil and acipimox.

Abstract:The hypothesis that raising high-density lipoprotein cholesterol (HDL-C) levels could improve the risk for cardiovascular disease (CVD) is facing challenges. There is multitudinous clear clinical evidence that the latest failures of HDL-C-raising drugs show no clear association with risks for CVD. At the genetic level, recent research indicates that steady-state HDL-C concentrations may provide limited information regarding the potential antiatherogenic functions of HDL. It is evident that the newer strategies may replace therapeutic approaches to simply raise plasma HDL-C levels. There is an urgent need to identify an efficient biomarker that accurately predicts the increased risk of atherosclerosis (AS) in patients and that may be used for exploring newer therapeutic targets. Studies from recent decades show that the composition, structure and function of circulating HDL are closely associated with high cardiovascular risk. A vast amount of data demonstrates that the most important mechanism through which HDL antagonizes AS involves the reverse cholesterol transport (RCT) process. Clinical trials of drugs that specifically target HDL have so far proven disappointing, so it is necessary to carry out review on the HDL therapeutics.

Abstract:Background: Cardiovascular disease (CVD) poses a significant global health and economic challenge, with atherosclerosis being a primary cause. Over the past 40 years, substantial research has been conducted into the prevention and reversal of atherosclerosis, resulting in the development of lipid-lowering agents such as statins and fibrates. Despite the extensive literature and formulation of numerous therapeutic guidelines in this domain, a comprehensive bibliometric analysis of the current research landscape and trends has not been performed. This study aimed to elucidate the evolution and milestones of research into lipid-lowering treatments for coronary heart disease (CHD) in conjunction with hyperlipidemia through bibliometric analysis, offering insights into future directions for treatment strategies. Methods: This study examined publications from 1986 to 2023 retrieved from the Web of Science database (Core Collection). Utilizing tools such as VOSviewer, Pajek, and CiteSpace, we analyzed publication and citation numbers, H-indexes, contributions by countries and institutions, authorship, journal sources, and keyword usage to uncover research trajectories and areas of focus. Results: Our analysis of 587 publications revealed a recent surge in research output, particularly post-2003. The American Journal of Cardiology published the highest number of studies, with 40 articles, whereas Circulation received the highest number of citations (6,266). Key contributors included the United States, Japan, and China, with the United States leading in citation numbers and the H-index. Harvard University and Leiden University emerged as pivotal institutions, and Professors J. Wouter Jukema and Robert P. Giugliano were identified as leading experts. Keyword analysis disclosed five thematic clusters, indicating a shift in research towards new drug combinations and strategies, signaling future research directions. Conclusion: The last 4 decades have seen a notable rise in publications on lipid-lowering therapies for CHD and hyperlipidemia, with the United States retaining world-leading status. The increase in international collaboration aids the shift towards research into innovative lipid-lowering agents and therapeutic approaches. PCSK9 inhibitors and innovative combination therapies, including antisense oligonucleotides and angiopoietin-like protein 3 inhibitors, provide avenues for future research, intending to maximize the safety and efficacy of treatment approaches.
Current Treatment of Dyslipidemia: Evolving Roles of Non-Statin and Newer Drugs

Abstract:Since their introduction, statin (HMG-CoA reductase inhibitor) drugs have advanced the practice of cardiology to unparalleled levels. Even so, coronary heart disease (CHD) still remains the leading cause of death in developed countries, and is predicted to soon dominate the causes of global mortality and disability as well. The currently available non-statin drugs have had limited success in reversing the burden of heart disease, but new information suggests they have roles in sizeable subpopulations of those affected. In this review, the status of approved non-statin drugs and the significant potential of newer drugs are discussed. Several different ways to raise plasma high-density lipoprotein (HDL) cholesterol (HDL-C) levels have been proposed, but disappointments are now in large part attributed to a preoccupation with HDL quantity, rather than quality, which is more important in cardiovascular (CV) protection. Niacin, an old drug with many antiatherogenic properties, was re-evaluated in two imperfect randomized controlled trials (RCTs), and failed to demonstrate clear effectiveness or safety. Fibrates, also with an attractive antiatherosclerotic profile and classically used for hypertriglyceridemia, lacks evidence-based proof of efficacy, save for a subgroup of diabetic patients with atherogenic dyslipidemia. Omega-3 fatty acids fall into this category as well, even with an impressive epidemiological evidence base. Omega-3 research has been plagued with methodological difficulties yielding tepid, uncertain, and conflicting results; well-designed studies over longer periods of time are needed. Addition of ezetimibe to statin therapy has now been shown to decrease levels of low-density lipoprotein (LDL) cholesterol (LDL-C), accompanied by a modest decrease in the number of CV events, though without any improvement in CV mortality. Importantly, the latest data provide crucial evidence that LDL lowering is central to the management of CV disease. Of drugs that inhibit cholesteryl ester transfer protein (CETP) tested thus far, two have failed and two remain under investigation and may yet prove to be valuable therapeutic agents. Monoclonal antibodies to proprotein convertase subtilisin/kexin type 9, now in phase III trials, lower LDL-C by over 50 % and are most promising. These drugs offer new ability to lower LDL-C in patients in whom statin drug use is, for one reason or another, limited or insufficient. Mipomersen and lomitapide have been approved for use in patients with familial hypercholesterolemia, a more common disease than appreciated. Anti-inflammatory drugs are finally receiving due attention in trials to elucidate potential clinical usefulness. All told, even though statins remain the standard of care, non-statin drugs are poised to assume a new, vital role in managing dyslipidemia.

Abstract:Despite treatment with statins, patients with elevated low-density lipoprotein cholesterol (LDL-C) and triglycerides remain at increased risk for adverse cardiovascular events. Consequently, novel pharmaceutical drugs have been developed to control and modify the composition of blood lipids to ultimately prevent fatal cardiovascular events in patients with dyslipidaemia. This article reviews established and emerging lipid-lowering drugs regarding their mechanism of action, development stage, ongoing clinical trials, side effects, effect on blood lipids and reduction in cardiovascular morbidity and mortality. We conducted a keyword search to identify studies on established and emerging lipid modifying drugs. Results were summarized in a narrative overview. Established pharmaceutical treatment options include the Niemann-Pick-C1 like-1 protein (NPC1L1) inhibitor ezetimibe, the protein convertase subtilisin-kexin type 9 (PCSK9) inhibitors alirocumab and evolocumab, fibrates as peroxisome proliferator receptor alpha (PPAR-α) activators, and the omega-3 fatty acid icosapent ethyl. Statins are recommended as the first-line therapy for primary and secondary cardiovascular prevention in patients with hypercholesterinaemia and hypertriglyceridemia. For secondary prevention in hypercholesterinaemia, second-line options such as statin add-on or statin-intolerant treatments are ezetimibe, alirocumab and evolocumab. For secondary prevention in hypertriglyceridemia, second-line options such as statin add-on or statin-intolerant treatments are icosapent ethyl and fenofibrate. Robust data for these add-on therapeutics in primary cardiovascular prevention remains scarce. Recent biotechnological advances have led to the development of innovative small molecules (bempedoic acid, lomitapide, pemafibrate, docosapentaenoic and eicosapentaenoic acid), antibodies (evinacumab), antisense oligonucleotides (mipomersen, volanesorsen, pelcarsen, olezarsen), small interfering RNA (inclisiran, olpasiran), and gene therapies for patients with dyslipidemia. These molecules specifically target new cellular pathways, such as the adenosine triphosphate-citrate lyase (bempedoic acid), PCSK9 (inclisiran), angiopoietin-like 3 (ANGPTL3: evinacumab), microsomal triglyceride transfer protein (MTP: lomitapide), apolipoprotein B-100 (ApoB-100: mipomersen), apolipoprotein C-III (ApoC-III: volanesorsen, olezarsen), and lipoprotein (a) (Lp(a): pelcarsen, olpasiran). The authors are hopeful that the development of new treatment modalities alongside new therapeutic targets will further reduce patients' risk of adverse cardiovascular events. Apart from statins, data on new drugs' use in primary cardiovascular prevention remain scarce. For their swift adoption into clinical routine, these treatments must demonstrate safety and efficacy as well as cost-effectiveness in randomized cardiovascular outcome trials.

Abstract:INTRODUCTION: Hypercholesterolemia is a major risk factor for cardiovascular disease (CVD). Low-density lipoprotein cholesterol (LDL-C) reduction has been demonstrated to decrease CVD-related morbidity and mortality. However, several patients do not reach LDL-C target levels with the currently available lipid lowering agents, particularly statins. Lipid and non-lipid parameters other than LDL-C may account for the residual CVD risk after adequate LDL-C lowering with statins.AREAS COVERED: This review focuses on the efficacy and safety of emerging drugs aiming at high-density lipoprotein cholesterol (HDL-C) elevation (i.e., recombinant or plasma-derived wild-type apolipoprotein (apo) A-I, apo A-I mimetic peptides, reconstituted mutant HDL, partially delipidated HDL and cholesterol ester transfer protein inhibitors), microsomal triglyceride transfer protein inhibitors and antisense oligonucleotides.EXPERT OPINION: Several lipid modifying agents in development may potently reduce the residual CVD risk. Ongoing and future studies with clinical outcomes will clarify their efficacy in clinical practice.

Abstract:Despite advances in pharmacotherapy, intervention devices and techniques, residual cardiovascular risks still cause a large burden on public health. Whilst most guidelines encourage achieving target levels of specific lipids and lipoproteins to reduce these risks, increasing evidence has shown that molecular modification of these lipoproteins also has a critical impact on their atherogenicity. Modification of low-density lipoprotein (LDL) by oxidation, glycation, peroxidation, apolipoprotein C-III adhesion, and the small dense subtype largely augment its atherogenicity. Post-translational modification by oxidation, carbamylation, glycation, and imbalance of molecular components can reduce the capacity of high-density lipoprotein (HDL) for reverse cholesterol transport. Elevated levels of triglycerides (TGs), apolipoprotein C-III and lipoprotein(a), and a decreased level of apolipoprotein A-I are closely associated with atherosclerotic cardiovascular disease. Pharmacotherapies aimed at reducing TGs, lipoprotein(a), and apolipoprotein C-III, and enhancing apolipoprotein A-1 are undergoing trials, and promising preliminary results have been reported. In this review, we aim to update the evidence on modifications of major lipid and lipoprotein components, including LDL, HDL, TG, apolipoprotein, and lipoprotein(a). We also discuss examples of translating findings from basic research to potential therapeutic targets for drug development.

Comparison of plasma lipid levels of inpatients of cardiovascular department in the third affiliated hospital of Sun Yat-sen University in 2005 and 2012

Abstract:Objective To compare the plasma lipid levels of inpatients of Department of Cardiovascular in the Third Affiliated Hospital of Sun Yat-sen University (SYSU) in 2005 and 2012, and to provide further evidences for prevention and treatment of dyslipidemia in patients with cardiovascular disease. Methods 1 647 patients admitted in 2005 and 2012 were recruited in this cross-sectional retrospective study. Data including general information, lipid levels, diagnosis of these patients were obtained. The patients were divided into four groups by age (≤44, 45-59, 60-74, and ≥75 years old) or three groups by diagnoses (uncomplicated hypertension group, coronary artery disease or its equivalent group, other cardiovascular diseases group). The data were analyzed by SPSS 13.0. Results The ratios of inpatients with dyslipidemia, hypercholesterolemia and high low-density lipoprotein-cholesterol (LDL-C) levels in 2012 were decreased compared with those in 2005 (P<0.001). The ratios of patients with low high-density lipoprotein-cholesterol (HDL-C) levels in these two years were both 45%. Compared with male patients, female patients showed higher total cholesterol (TC) and LDL-C levels in 2005 (P<0.001, P=0.02, respectively), and higher TC, LDL-C, HDL-C levels in 2012 (all P<0.01). Patients less than 44 years old showed the lowest levels of TC, LDL-C, while patients in 45-59 years old group showed highest levels of TG compared to other 3 groups (all P<0.05), both in 2005 and 2012. Patients with uncomplicated hypertension showed significantly higher TG levels in 2005 and higher TC, HDL-C, LDL-C levels in 2012 compared with patients with CAD and its equivalent (P<0.05). Conclusions This study shows significant lower ratios of inpatients with dyslipidemia in Department of Cardiovascular in 2012 compared with 2005. However the ratios are still high, especially in female group and uncomplicated hypertension group. As one of the risk factors for hypertension and cardiovascular diseases, the prevention and treatment of dyslipidemia remains important issues in chronic diseases management.

Abstract:INTRODUCTION: Hypercholesterolemia is the main modifiable risk factor for atherosclerosis progression and cardiovascular disease (CVD) development. Its pharmacological management is usually based on the prescription of statins, that in some cases are not however fully effective to reach the desired Low-Density-Lipoproteins cholesterol (LDL-C) target, or are not tolerated by patients due to side effects. Areas covered: This manuscript summarizes the basic properties of the emerging new classes of lipid-lowering drugs such as ezetimibe, Proprotein Convertase Subtilisin/Kexin type 9 (PCSK9) inhibitors, and Microsomal Triglyceride Transfer Protein (MTP) inhibitors, also citing new drugs in development. Our aim is to describe the main pharmacodynamic and pharmacokinetic characteristics, the available efficacy, tolerability and safety data obtained in randomized clinical trials where these drugs were tested. Expert opinion: Non-statin lipid-lowering drugs can be considered an excellent strategy to reduce the residual CV risk, also represented by non-target LDL-C values and high lipoprotein(a) serum levels. In particular, the approved PCSK9 inhibitors (Evolocumab and Alirocumab) have been very effective in optimizing plasma LDL-C values and reducing CV event risk.

Abstract:The development of coronary artery atherosclerosis contributes to high levels of low-density lipoprotein, inflammation, and endothelial dysfunction, as well as to other conditions. Recent clinical trials have demonstrated that the administration of statins could improve prognosis of acute coronary syndrome thanks to lowering of lipid levels, anti-inflammation and enhancing endothelial function. The pleiotropy of statins has been a recent focus of research attention. It has been confirmed by intravascular ultrasound that therapy involving high doses of statins promotes the regression of atherosclerotic plaque.

Abstract:Cardiovascular diseases are the leading cause of death worldwide. Dyslipidemia is one of the most significant modifiable risk factors for the CVD development and potentiation. The main drugs in the treatment of dyslipidemia in modern clinical practice are statins, although there are other effective hypolipidemic drugs that are gaining popularity, such as ezetimibe, proprotein convertase subtilisin/kexin type inhibitors - PCSK9 inhibitors, antisense oligonucleotide (pelacarsen), small interfering RNA - siRNA (inclisiran), and some others. In real clinical practice, the most effective approach to achieve LDL-C targets is adding ezetimibe to a statin (simvastatin, atorvastatin, rosuvastatin, etc.), rather than titrating the statin dose to the maximum possible. The addition of siRNA to statin therapy resulted in a stable, significant reduction in LDL levels by an average of 50% in all groups compared with statin monotherapy.

New, Novel Lipid-Lowering Agents for Reducing Cardiovascular Risk: Beyond Statins

Abstract:Statins are the cornerstone of the prevention and treatment of atherosclerotic cardiovascular disease (ASCVD). However, even under optimal statin therapy, a significant residual ASCVD risk remains. Therefore, there has been an unmet clinical need for novel lipid-lowering agents that can target low-density lipoprotein cholesterol (LDL-C) and other atherogenic particles. During the past decade, several drugs have been developed for the treatment of dyslipidemia. Inclisiran, a small interfering RNA that targets proprotein convertase subtilisin/kexin type 9 (PCSK9), shows comparable effects to that of PCSK9 monoclonal antibodies. Bempedoic acid, an ATP citrate lyase inhibitor, is a valuable treatment option for the patients with statin intolerance. Pemafibrate, the first selective peroxisome proliferator-activated receptor alpha modulator, showed a favorable benefit-risk balance in phase 2 trial, but the large clinical phase 3 trial (PROMINENT) was recently stopped for futility based on a late interim analysis. High dose icosapent ethyl, a modified eicosapentaenoic acid preparation, shows cardiovascular benefits. Evinacumab, an angiopoietin-like 3 (ANGPTL3) monoclonal antibody, reduces plasma LDL-C levels in patients with refractory hypercholesterolemia. Novel antisense oligonucleotides targeting apolipoprotein C3 (apoC3), ANGPTL3, and lipoprotein(a) have significantly attenuated the levels of their target molecules with beneficial effects on associated dyslipidemias. Apolipoprotein A1 (apoA1) is considered as a potential treatment to exploit the athero-protective effects of high-density lipoprotein cholesterol (HDL-C), but solid clinical evidence is necessary. In this review, we discuss the mode of action and clinical outcomes of these novel lipid-lowering agents beyond statins.

Abstract:For nearly three decades , lipid-lowering agents , especially statins have been widely used for regulating blood lipids, and preventing atherosclerosis for its safety and efficacy in lowering low -density lipoprotein cholesterol ( LDL-C) and elevating high-density lipoprotein cholesterol ( HDL-C) , thwarting lipids infiltration and deposition in the vascular wall and stabilizing plaques.However, the side effects of this kind of drugs have been revealed by clinical trials and applications , subsequently the application of them has been limited .This paper is a review on the adverse effects of lipid -lowering agents and corresponding treatment methods , which provide guidance to doctors to deal with these agents reasonably .

Abstract:High-density lipoprotein (HDL) plays a role in anti-atherosclerosis and anti-coronary heart disease through its unique function that reverse cholesterol transport.A large number of clinical and epidemiology studies have proved that high density lipoprotein cholesterol (HDL-C) levels were negatively correlated with cardiovascular risk, so how to improve the level of HDL-C has become the focus of attention, but the development of raising HDL-C drugs failed repeatedly.Further discussion on the structure and function of HDL will help to analyze the possible causes and mechanisms of the failure of raising HDL-C drugs, which is of great significance for clinical and basic research.

Abstract:Although the incidence of dyslipidemia in Chinese population is increasing,the awareness rate,control rate and target rate of dyslipidemia are still low.Many doctors couldn't well understand the dyslipidemia.In order to reduce the incidence and the mortality of coronary disease and stroke,all doctors(especially community physicians)should improve the ability to regulate lipid,pay more attention to non-drug treatment(lifestyle change)and establish reasonable,effective and safe management strategies.

Abstract:OBJECTIVE: To synthetically analyse the protective effects of statins on hearts. DATA SOURCES: An online search of Pubmed was undertaken to identify English articles about the protective effects of statins on coronary artery disease (CAD) and heart published from January 2000 to January 2005 by using the Keywordsof "statins, coronary artery disease, heart protection". Meanwhile, Wanfang and CNKL database were searched with computer for articles about the correlation between statins and CAD, adjustment of cholesterol as well as heart protection published in Chinese from January 2000 to January 2005 by using the keywords of "statins, CAD, heart protection". STUDY SELECTION: Articles were retrieved in the first trial and literatures related to statins and heart protection were selected and further identified for correlations. Randomized control studies were selected and parallel control group should be involved in the experiments or clinical researches. As to literatures in the same region, those published in recent time or published by authorized magzines were selected. DATA EXTRACTION: A total of 58 articles about the protective effects of statins on heart were collected and 18 literatures were in accordance with the inclusive criteria. Among 40 excluded articles, 30 were non-randomized or repetitive studies, 10 were Meta analysis studies. DATA SYNTHESIS: By inhibiting HMG-CoA reductase, statins reduced the free cholesterol in cells, accelerated the clearance of low density lipoprotein cholesterol (C-LDL) and very low density lipoprotein cholesterol (C-VLDL) granules, accordingly reduced the blood lipid. Meanwhile statins also had a protective effect on heart by improving endothelial function, restoring injured endothelium, counteracting inflammation, stabilizing vulnerable plaques and so on; Accordingly provided powerful evidences for primary and secondary prevention for patients with CAD. CONCLUSION: Effects of statins have been confirmed in reducing lipidemia and preventing cardiovascular events, its safety and toleration withstood large scale of inspection in clinical trials. At present, clinical experiments of reducing lipidemia with multiple statins is in progress, and the safety and clinical effects need further investigating.
