Abstract:Objectives:This article discusses the definition and scope of drug safety,and is to identify the main risk factors in China.Methods:A systematic review was conducted through searching English database such as PubMed and some web sources to analyze,scope,and synthesize the definition and scope of drug safety,as well as the strategies and implementations of securing drug safety by using literatures within recent ten years.In addition,a quanlitative reviews was undertaken to describe the understanding of the definition and scope of drug safety through reviewing drug safety administrators,health personnel,and specialists.The review was also included their attitudes of the importance of drug safety.Results:290 literatures were included and six of them gave the definition of drug safety.Those included literatures focused on adverse drug reactions/events to monitor,prevent medication errors,risk management,as well as drug accessibility.The scope of drug safety mainly included drug monitoring,adverse drug reactions/events,drug security,drug research and development,pharmaceutical production,the flow of drugs,using and information of drugs.Depth interviews showed that the quality issues and inappropriate use of drugs were major influence factors.Conclusion:This article concluded that drug safety should be defined as a status that the external and internal risk be eliminated by supervising,besides necessary supply and information feedback.In addition,the scope of drug safety was defined as appropriate quality,acceptable adverse reaction risk,none of medical error and accessibility.

Abstract:Objective To discuss the definition and category of drug safety,and to identify the main risk factors in China.Methods Qualitative interview methods were implemented in this study.Results Depth interviews of 15 experts showed that most experts believed that drug safety should include quality of drugs,adverse drug reactions,non-rational use of drugs,policy safeguard and accessibility,the quality issues and non-rational use of drugs were major influence factors.Conclusion Drug safety should be defined as a status that the external and internal risk was eliminated by supervising,besides necessary supply and information feedback.

Abstract:By reviewing literature and conducting depth interviews,we analyzed domestic and international supervision systems relating to drug safety early warning,and discussed the mode of building up drug safety early warning system.The characteristics are apparently different in domestic and international drug safety early warning; they emphasize separate aspects.It is strongly recommended that an integrated drug safety early warning system should be built up in China for monitoring risks timely.

Abstract:The reasons of drug safety issues were analyzed deeply from perspective of risk society theory,from whichwe should keep correct view of drug safety issues,through strengthening risk education to improve awareness and ability of prevention for the whole society on risk.Elimination of unreasonable risk taking mechanism to avoid embarrassed situation of uncertain subject of responsibility,rational and scientific adjustment of medical industrial structure,improving enterprise's innovation capability and competitiveness,avoiding vicious competition homogenization and drug manufacturing defects which may be induced by this.To establish an open and transparent drug safety information release mechanism,regulate the media to popularize knowledge of drug safety to raise satisfaction about drug safety of public.
Nursing research concerning medication safety in China: a quantitative analysis of literature data

Abstract:Objective To explore strategies for promotion of medication safety in mainland China,and to identify the existing problems by analyzing nursing research targeted on medication safety.Methods Totally,624 research papers published from 1998 to 2010 and in Chinese were retrieved from CNKI and CBM databases,and were analyzed by quantitative analysis.Results Published papers concerning medication safety were increased year by year.Of all the papers,60.1% focused on experiences related to medication safety,and 77.2% did not employed statistic analysis.The main contents included process management of medication safety(23.1%),and analysis of medication error(21.3%) and so on.Conclusion Studies focused on medication safety in clinical nursing.However,these studies are mainly qualitative comments and summaries of experience,which lack strict design and sufficient use of statistical methods.It is strongly recommended that the studies concerning medication safety,promotion strategies,and medication safety in community treatment should be strengthened,so as to construct appropriate guideline for medication safety in clinical nursing in China.

Abstract:Objective: To understand the literatures on pharmacoepidemiology study of drug abuse in China.Methods:Chinese Biology Literature Database(CBLD) from 1987 to 2003 was searched with drug dependence and epidemiology as key word and second key word,respectively and the result of which was analyzed.Results: Altogether,350 papers were selected. The earliest paper was published in 1987; the year 2002 published the most; Beijing took the first place in the number of papers produced. The study subjects included general population, drug abusers doing detoxification in hospitals,compulsory settings, and students. Large sample studies in general supported by foundations were conducted mostly by related institutions in universities and hospitals. Most of the papers(88.6%)used cross section method and the contents covered a wide range. Conclusion: From the papers indexed in CBLD, people can get a general situation of pharmacoepidemiology study on drug abuse in China and understand the drug abuse problem, which provides evidence for scientists to do further study. However, with the search strategy adopted, some papers are missed.

Abstract:Objective:To summarize the research status of drug shortages and to provide references for. Methods:The databases such as CNKI, Wan Fang Data and VIP were electronically searched to collect studies related to drug shortages and related issues such as the varieties, the reasons, strategies and so on. The retrieval was from January, 1989 to April, 2015.The literature was screened independently according to the inclusion and exclusion criteria, thendata were extracted and ifnallythe related studies were analyzed.Results:The varieties of drugs in-short in China added up to 148, including 126 kinds of chemical drugs and 22 kinds of traditional Chinese medicines. The chemical medicines in-short were mostly biological products, cardiovascular drugs, hematological system drugs, antimicrobial agents, antiphrastic drugs and so on. While the shortages of traditional Chinese medicineswere mostly internalmedicines. Moreover, reasons for the drug shortages in China were complicated, such as the unreasonable pricing policy, the unsound supporting measurements and bidding systems, loose supervision in relateddepartments and some institutional issues in hospital systems and lack of medical knowledge in patients. The strategies to deal with drug shortage includedestablishing special department and platform to manage drug shortages,formulatingguidelines to handle the problems, setting reasonable drug price and bidding policy, strengthening drugs registration,normalizing reservation of drugs in shortage and so on. Conclusion: Up to now, the status of drug shortages in China was not so optimistic. Drug shortages in China were varied, andthe analysis of the reasons was not profound enough.Furthermore, the strategies for solving related problems were superifcial and not in detail.What’s more, therewere few strategies toimprove this situation and few concerns about the impact of drug shortages. More studies about drug shortages were needed to provide support and advice.
Analysis of the problems of drug sampling and testing programme under tripartite system reform in China

Abstract:Rationale: To ensure that quality medicines are available to the populace. Aim and objective: To provide a new perspective for drug sampling and testing using a risk-based approach globally, and provide a reference for improving the efficiency of the drug sampling and testing programme in China. Method: This study used a combination of theoretical analysis and problem analysis. First, we analysed the high-risk drugs that should be given attention to theoretically in the drug sampling testing programme based on the healthcare reform in China. Second, this study leveraged on data on the sampling and testing of drugs extracted from self-constructed data by the China Food and Drug Administration (CFDA). The integrated and structured data sets were used to conduct an innovative analysis that identifies the actual content of the drug sampling and testing programme and contrasts them with the theoretical content to understand the problems. This study used quantitative and qualitative approaches for data analysis. Results: Following analysis, we discovered that there is still a big gap between the reality of drug sampling and testing in China and the ideal state of the objective requirements of healthcare reforms. The major sources of risk are the retail pharmacies or hospital pharmacies in the distribution chain of drugs; however, the CFDA sampled mostly drugs from drug manufacturers. Additionally, the CFDA did not sample adequate essential medicines, bid-winning drugs (especially those with large price drops in centralized procurement), biological products and other high-risk drug varieties. The high-risk drugs produced by companies or enterprises with a history of noncompliance, especially bid-winning enterprises in centralized procurement, were not sampled. Conclusion: We suggest that the major content of drug sampling and testing in China should focus on the link of drug utilization, varieties of drugs with large price drops in centralized procurement and bid-winning enterprises with a history of noncompliance.

Abstract:The unduly rapid growth of drug expenses in China at present exceeds the countrys economic development and patients endurance, turning into one of the central social issues. The paper gives a detailed analysis of the causes of the problem, which can be attributed both to the field of production and the process of consumption. It also puts forward appropriate countermeasures from the perspective of the structure of the drug industry, hospital management, drug prices, doctors supervision and the medical insurance system.
Patient Safety Research in China: A Literature Review

Abstract:OBJECTIVE: A number of articles on patient safety have been published in Chinese-language journals in last decade. However, until now there have been no studies of their overall quality or contribution to current thinking. This study explored the themes and designs of studies on patient safety published in Chinese-language journals.METHOD: We searched the major Chinese databases, Chinese Biomedical Literature Database (CBM) and Chinese Journals Full-text Database (CNKI), for Chinese-language patient safety research papers published between January 2001 and December 2010. We analyzed the articles' publication years, research themes, author affiliations, and research methodologies. Quality and statistical methods were only appraised based on study design type.RESULTS: In the decade studied, a total of 6,336 articles were included. 2008 saw the greatest number of patient safety articles published in China (865). There were 11 journals which each published at least 55 relevant articles over 10 years. The most popular topic of patient safety studies was nosocomial infection (5,182/81.79%). Non-comparative studies accounted for the majority of the literature (4,390/ 69.3%); of these, most were case series (4,080/92.94%). Comparative studies accounted for only 2.6% of the studies found (167). Most of the first authors of included articles worked at hospitals (5,193), many in nursing departments (2,044).CONCLUSIONS: There was no change trend in the number of articles on patient safety research published in Chinese in the past 10 years. Most articles described non-comparative studies, which lack rigorous designs. More methodologically rigorous designs are needed to improve the overall quality of patient safety research in China.

Influence of drug safety environment on pharmacists' drug safety practice

Abstract:Objective To explore the influence of drug safety environment on pharmacists'drug safety practice.Methods The subjects of the investigation were 1959 pharmacists (1763 from 3A hospitals and 196 from 2A hospitals),who were selected from 112 hospitals of 26 provincial administrative regions in China using stratified sampling method.Baseline survey questionnaire about patient safety and medication errors was designed by the research group and the survey was performed by filling out an online questionnaire and paper questionnaires.In this questionnaire,4 dimensions,including drug safety knowledge,attitude,environment,and practice,and 6,3,8,and 6 sub-dimensions corresponding to above 4 dimensions were designed.There were 102 questions altogether.Five-point scale was used for each question and higher scores represented good performance.Specified ordinary least squares regression models were used to analyze dataset from this survey.Results The scores of drug safety knowledge,attitude,environment,and practice in 1959pharmacists were (3.45 ± 0.23),(3.72 ± 0.45),(3.35 ± 0.50),and (3.68 ± 0.48),respectively.The results showed that drug safety knowledge,attitude,and environment had significant positive influences on drug safety practice and among them the influence of drug safety environment was the largest.Among the 8sub-dimensions of drug safety environment,hospital regulation and drug safety culture of work team had the largest influence on pharmacist's drug safety practice.The pharmacist's drug safety knowledge had the largest influence on basic drug safety practice,while drug safety environment had the largest influence on pharmacists'advanced drug safety practice.Conclusions The drug safety environment is the most important factor to influence pharmacists' drug safety practice in China.The most effective way to increase the levels of pharmacists'drug safety practice is to improve pharmacists'drug safety knowledge levels and improvement of the drug safety environment is the most effective way to increase the levels of drug safety practice.

Abstract:Drug-related adverse reactions are among the main reasons for harm to patients under care worldwide and even their deaths. The pharmacovigilance system has been proven to be an effective method of avoiding or alleviating such adverse events. In 2019, after two decades of implementation of the drug-related adverse reaction reporting system, China formally implemented a pharmacovigilance system with the Pharmacovigilance Quality Management Standards and a series of supporting technical documents created to improve the safety of medication given to patients. China’s pharmacovigilance system has faced many problems and challenges during its implementation. This spontaneous reporting system is the main source of data for China’s medication vigilance activities, but it has not provided sufficiently powerful evidence for regulatory decision-making. In conformity to the health-centered drug regulatory concept, the Chinese government has accelerated the speed of examination and approval of urgently needed clinical drugs and orphan drugs along with the requirement to improve the safety supervision of these drugs after their listing. China’s marketing authorization holders（MAHs）must strengthen its pharmacovigilance capabilities as the primary responsible department for drug safety. Chinese medical schools generally lack professional courses on pharmacovigilance. The regulatory authorities have recognized such problems and have made efforts to improve the professional capacity of pharmacovigilance personnel and to strengthen cooperation with stakeholders through the implementation of an action plan of medication surveillance and the establishment of patient-based adverse events reporting system and active surveillance systems, which will help China bridge the gap to bring its pharmacovigilance practice up to standards.

Abstract:Objective: The necessity of shifting from management to governance of drug safety has gained a wide consensus in both academic and practical circles, but how to act and develop has not been adequately discussed and become a key issue that needs to be addressed nowadays. This article combines the theoretical essence of governance to establish an analytical framework of “initial conditions-joint action-governance effectiveness” based on governance theory to analyze and explain the connotation and mechanism of cross-sector cooperative governance of drug safety. Methods: The essence of governance lies in the creation of joint action capabilities through cross-sector cooperation, as well as the construction of a national drug safety governance action framework of “3 stages + 1 incubation period”. This article analyzes the connotation of each stage and possible problems or obstacles. Results: Three deep-seated challenges stem from the lack of information sharing, resource action sharing, and power sharing. Conclusion: In order to improve the modernization construction of China’s drug safety governance system and capacity, it is recommended to continuously improve Chinese drug safety governance in terms of governance norms, communication mechanisms, capacity development, and cross-sector coordination based on a holistic perspective.

Abstract:: Although the current drug service system can meet the fundamental healthcare needs of China, outstanding issues exist in all service sectors, including research and development, manufacturing, marketing, and clinical use. Through a literature search, data queries, specialist consultation, and a panel discussion, this study investigated the current status of the drug service system and identified the main problems. We propose an idea for constructing a new national drug service system to provide drugs to the Chinese people that are of high quality, have a reliable therapeutic effect, come at a reasonable price, and exhibit good medical compliance, thus ensuring the efficacy, safety, availability, and reasonability of drugs for clinical use, as well as promoting a structural transformation of the pharmaceutical industry that will enable China to finally transition from a big country into a great power in the global pharmaceutical industry.

Frontiers of Chemical Engineering in China

Abstract:Drug safety management is an important issue in China drug management system and attracts great attentions from the whole society. In order to reduce drug incident, this study discusses some important elements associated with China drug safety management system and analyzes the data collected by questionnaires. Besides, a methodology for rating the important elements is described and applied. The non-structural fuzzy group decision method not only considers the insufficient precise information but also combines the opinions of different kinds of respondents in China’s four municipalities. The results indicate that the sample systems are the most important in these important elements, and the order of importance is sampling systems, licensing systems, traceability systems, transaction models, pharmacovigilance and emergence management. This study not only points out the important ranking of the pivotal elements in China drug safety management but also gives some specific proposals about how to enhance drug safety management in China.
