Abstract:Objective To investigate pulmonary capillary changes in patients with diabetes mellitus. Meth-otis Fifty-eight patients with diabetes mellitus were enrolled and forty-seven healthy subjects were taken as control. Diffusion capacity of carbonmonoxide (DLCO) and pulmonary ventilatory function were measured. DM and Vc were measured in twenty-one patients and twelve healthy subjects among them. Results FEV1/FVC was (81.02± 6.40) % in patients with diabetes mellitus and ( 81.20±6.96 ) % in controls, and FEV 1% was ( 102.03±14.40) in patients with diabetes mellitus and 103.94±11.42 in controls ,with no significant difference between patients with DLCO% was ( 72.79±19.85 ) % in patients with diabetes mellitus and ( 90.60±13.25 ) % in controls with a sig-patients whose course of disease was less than ten years,and DLCO% was (64.69±17.49)% in patients with dia-betes mellitus whose course of disease is equal or more than ten years and (80.90±18.98)% in patients whose course of disease is less than ten years,with significant difference between these two groups (t = 4.435, -3. 381, 13.88)% in patients with diabetes mellitas and (83.58±26.79)% in controls with a significant difference (t = 4. 612, P < 0.001 ). Vc was ( 61.40±52.84 ) ml in patients with diabetes mellitus and ( 66.99±19.63 ) ml in con-trols with no significant difference (P > 0.05 ), and Vc% was (78.05±64. 40)% in patients with diabetes mellitus and (79.33±23.32) % in controls, with no significant difference ( P > 0.05 ). Conclusions Diffusing capacity is decreased in patients with diabetes mellitus, which is related to the course of disease . DM decline is the main cause of DLCO decrease in patients with diabetes mellitus.
Pulmonary function in type 2 diabetes and its related factors

Abstract:Objective: To explore the changes of pulmonary function in type 2 diabetes and its related factors.Methods: The pulmonary functions of 72 patients with type 2 diabetes and 22 healthy objects were measured,and the related factors,such as duration of DM,HbA1c,BMI,were analysed with linear regression analysis.Ninteen of them underwent a 12-week-long intensive insulin therapy and pulmonary function tests before and after the treatment.Results: Vital capacity(VC),forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),total lung capacity(TLC) and carbon monoxide diffusion in the lung(DLco) were significantly decreased in the diabetes patients.Correlation analysis revealed that DLco was negatively correlated with the duration of DM.which was shown by linear regression analysis to be the only significant predictor.After a 12-week-long intensive insulin therapy,DLco and DLco/VA decreased significantly.Conclusion:Patients with type 2 diabetes have abnormal pulmonary ventilatory function and impaired pulmonary diffusive function,and the latter is related to the time of hyperglycemia,and can not ameliorated by short-term glycemic control.
Changes of Pulse Wave Velocity in Patients with Coronary Artery Disease Combined with Diabetes Mellitus

Abstract:Objective To investigate the changes of PWV and other relative clinical indices in patients with coronary artery disease combined with diabetes mellitus.Methods The patients were divided into control group(group A)(50 cases),coronary artery disease group(group B)(50 cases) and coronary artery disease combined with diabetes mellitus(group C)(50 cases),blood glucose,blood fat,glycosylated hemoglobin(HbA1c),nitric oxide(NO),endothelin(ET),high sensitive C-reactive protein(hs-CRP) and baPWV were examined in 3 groups.Results The levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2hPG),HbA1c,hs-CRP and baPWV in group C were obviously higher than those in group A and group B(P0.05,P0.01),the levels of NO was lower than that in group A and group B(P0.01).The level of baPWV was positively correlated with age,2hPG,LDL-C,low density lipoproteincholesterol(LDL-C),hs-CRP and ET in group C(P0.05,P0.01),negatively correlated with NO(P0.01).Conclusions At DM stage,PWV has been significantly accelerated in patients with coronary artery disease.Postprandial hyperglycemia,insulin resistance,inflammatory factors and endothelial function injury can speed up artery injury,which are also main disk factors for AS.

Abstract:Background: Air pollution exposures have been associated with hypertension, but limited studies have examined the associations in diabetes mellitus patients.Objectives: We aimed to examine the associations of exposure to air pollution with risk of hypertension in diabetes mellitus patients.Methods: We identified hypertension cases from hospital records among patients who were previously diagnosed as diabetes mellitus from January 2013 to May 2017.We used generalized additional models to estimate adverse effects of air pollution on systolic/diastolic blood pressure in diabetes patients.Results: A total of 2,050 patients were enrolled in this study. A 10μg/m3 increase in PM2.5, PM10, SO2, and O3 exposure was associated with an increased risk of higher systolic blood pressure[D1] level (risk ratio (RR) =1.001; 95% confidence interval (CI):1.000, 1.002; RR =1.001; 95%CI: 1.000, 1.002; RR =1.004; 95%CI: 1.001, 1.006). Significant associations were observed with fasting blood glucose(fbg) ≥7.0mmol/L, overweight/ obesity, and younger age. Little evidence has been observed for the associations between particulate matter (PM), SO2 and O3 exposure and diastolic blood pressure in present study.Conclusions: In our study, diabetes patients were at increased risk for higher blood pressure when exposed to higher residential PM and SO2. Individuals with higher BMI, younger age, and FBG≥7.0mmol/L were more susceptible to ambient air pollution.

Abstract:Air pollution exposure has been associated with hypertension, but limited studies have examined the associations in diabetic patients. We aimed to examine the associations of exposure to air pollution with blood pressure in diabetic patients. We identified patients who were previously diagnosed as type-2 diabetes mellitus from hospital records from January 2013 to May 2017. We used generalized linear models to estimate adverse effects of air pollution on systolic/diastolic blood pressure in diabetic patients. A total of 40,125 patients were enrolled in this study. An IQR increase in PM2.5, PM10, NO2, SO2, and O3 exposure was associated with an increased risk of higher systolic blood pressure level (β = 0.628 for PM2.5, β = 0.758 for PM10, β = 0.766 for NO2, β = 0.516 for SO2, β = 0.482 for O3, all P value < 0.05). Significant associations were observed for some air pollutants and SBP among patients with fasting blood glucose (FBG) ≥ 7.0 mmol/L, normal weight, and younger age. Little evidence was available for the associations between particulate matter (PM) or O3 exposure and diastolic blood pressure in present study. In our study, diabetic patients were at increased risk of higher blood pressure when exposed to higher residential PM, NO2 and SO2. Individuals with lower BMI, younger age, and FBG ≥ 7.0 mmol/L were more susceptible to the adverse effects of ambient air pollution.

Abstract:Abstract Background Nowadays chronic respiratory diseases are a major burden in terms of morbidity and mortality and of increasing public health concern in China. Meanwhile, prevalence of diabetes has increased more than 10 times over forty years. A few studies investigating the association between chronic respiratory diseases and diabetes mellitus have been done these years. However, the association was not clear, and we aimed to answer this question with solid proof. Methods Our study is a single-center study performed in MJ Healthcare Center in Beijing. We enrolled subjects aged ≥ 20 years undergoing at least two regular health check-ups between 2009 and 2019 in MJ Healthcare Center in Beijing. Each health check-up included regular tests, biochemical tests, a pulmonary function test and a questionnaire. 11,107 adults were included and cross-sectional and longitudinal analysis were performed. Results We found that both pre-diabetic and diabetic adults had lower lung function compared to normal population at baseline, indicating that lung function decrement may start from prediabetic status. Quantitatively, with 1 mmol/L increase of FPG (fasting plasma glucose), FVC (forced vital capacity), FEV1 (forced expiratory volume), FVC% and FEV1% lowered 25ml, 13ml, 0.71%-1.03% and 0.46%-0.72% respectively. However, no statistical significance was found regarding the rates for the lung function decline between different baseline diabetes status. Conclusion People with higher blood glucose had more severe lung function decline and lung function decrement started from pre-diabetic status, but there was no difference in the rate of lung function decrease based on different baseline diabetic status.
Pathogenic Bacteria and Risk Factor Analyses of Fungal Infection in Patients with Chronic Obstructive Pulmonary Diseases Complicated with Diabetes

Abstract:Objective To investigate the distribution of pathogenic bacteria and the main risk factors of fungal infection in patients with chronic obstructive pulmonary diseases(COPD) complicated with diabetes mellitus.Method Pathogenic bacteria and prognosis analyses were performed to 58 cases and the results were compared with those of 76 patients with chronic obstructive pulmonary diseases and fungal infection but without diabetes mellitus.Result Blastomyces albicans was the major pathogenic bacteria in all of the patients.The rate of fungal infection was higher and the prognosis was more serious in patients with diabetes mellitus than in those without diabetes mellitus.Conclusion Blastomyces albicans is still the major pathogenic bacteria in patients with chronic obstructive pulmonary diseases and fungal infection.Diabetes mellitus can improve the risk of fungal infection and affect the prognosis in patients with chronic obstructive pulmonary diseases.

However, no significant difference was found in the rates for the lung function decline among different baseline diabetes statuses. Conclusion People with higher blood glucose level had more severe lung function decline, with decline starting from prediabetic status, but no significant difference was noted in the rate of lung function decline based on different baseline diabetic statuses.

Abstract:BACKGROUND: There are a large number of works devoted to the study of the state of the bronchopulmonary system in diabetes mellitus in the literature in the last 20 years. However, these studies are often conflicting. Some researchers identify a deterioration in the function of external respiration and a connection with metabolic changes and complications of the disease, others associate it with vascular pathology. There are a number of reports of increased pulmonary ventilation in diabetes. AIM: To assess the structural and functional state of the lungs in patients with diabetes mellitus and in combination with coronary heart disease and obesity. METHODS: 395 patients with type 1 and type 2 diabetes mellitus were under observation. The diagnosis of diabetes mellitus was verified in accordance with International Programs and was based on WHO criteria. The glycemic level of patients was determined using a One Touch® basic glucometer (Johnson&Johnson, USA). The degree of carbohydrate metabolism compensation was assessed by the level of glycated hemoglobin (HbA1c), determined using a laboratory analyzer DCA-2000 MT (BAYER, Germany). The concentration of C-peptide in the blood serum was determined by the method of immunoluminometric analysis "Immunotech" (Czech Republic). Caro and HOMA-IR indices were calculated to identify and assess the insulin resistance (IR). The indices of hormone metabolism were determined by ELISA using DSL kits (USA) with subsequent measurement of optical density on a Spectra Classic reader from Tecan (Austria): corticotropic hormone (CTG), adrenaline, noradrenaline, cortisol, free hydrocortisone; 17-ketosteroids, 17-oxycorticosteroids, glucogone, insulin, somatotropic hormone (STH); thyroid stimulating hormone (TSH); thyroxine (T4); thyroxine (T3). Instrumental-functional and radiation research methods: X-ray methods for lungs examining, computer spirography, fibrobronchoscopy of the bronchi was performed in all patients. Morphological changes were assessed using histological and morphometric methods. RESULTS: Pathogenetic mechanisms of the bronchopulmonary system disorders in patients with type 1 diabetes mellitus are associated with a decrease in the function of external respiration due to the volumetric air flow rates of the predominantly central airways and an increase in bronchial resistance. Alveolar hypoventilation of a restrictive type with impaired diffusion of gases through the alveolar-capillary membrane was detected in patients with type 2 diabetes mellitus. Restrictive and obstructive type disorders with impaired ventilation-perfusion ratios and pulmonary blood flow are formed in patients with type 2 diabetes mellitus combined with coronary artery disease and obesity. CONCLUSION: A decrease in the function of external respiration due to the central respiratory tract and an increase in bronchial resistance were noted in 38.4% of patients with type 1 diabetes mellitus. Restrictive alveolar hypoventilation was registered in 23.3% of patients with type 2 diabetes mellitus, catarrhal endobronchitis – in 21.31% of patients with type 2 diabetes mellitus. Damage and fibrosis of the alveolar tissue, damage to the endothelium and disorganization of the connective tissue of the lungs were characteristic of microscopic examination of the ultrastructure of the lungs in patients with diabetes mellitus.

Abstract:Diabetes mellitus (DM) is a chronic metabolic disorder that affects millions of individuals worldwide. With an increasing prevalence, understanding its implications for respiratory health is essential. Chronic lung diseases, such as asthma and chronic obstructive pulmonary disease (COPD), significantly impact morbidity and healthcare costs, with COPD alone accounting for substantial economic burdens. This comprehensive review investigates the intricate relationship between DM and chronic lung diseases. A systematic search across multiple databases yielded 1,078 articles, from which 10 studies were selected for detailed examination. The findings reveal a bidirectional relationship: diabetes increases the risk of developing chronic lung conditions, while chronic lung diseases can exacerbate glycemic control. Shared inflammatory pathways and comorbidities complicate patient outcomes, underscoring the urgent need for integrated treatment approaches. By elucidating the mechanisms linking these conditions, this review provides valuable insights for healthcare professionals, emphasizing the importance of interdisciplinary care to enhance the quality of life for individuals affected by both diabetes and chronic lung diseases. The results highlight the necessity for further research to explore targeted therapies and preventive measures addressing these interconnected health issues.

Abstract:Objective. The main objective is to study the effect of diabetic nephropathy on pulmonary function and clinical outcomes. Methods. The method is to retrospectively analyze patients with diabetic nephropathy (DN) in our hospital from April 2018 to March 2022 as study subjects. The differences in baseline data, serum indicators, renal function indicators, and pulmonary function of patients at different clinical stages were analyzed and then explored. Finally, logistic regression was used to analyze the risk factors affecting patients' clinical outcomes and to evaluate the diagnostic effects. Results. Baseline information (age, disease duration, BMI, and systolic and diastolic blood pressure), serum indicators (HbA1c, FBG, 2hPG, TG, TC, and LDLC), renal function indicators (CysC, BUN, and Scr), and pulmonary function (TLC, VC, FEV1, FEV1/FVC, MVV, MEF25, MEF50 MEF75, DLCO, and DLCO/VA) were significantly different P < 0.01 ; multiple logistic regression analysis showed that SBP, HbA1c, FBG, 2hPG, BUN, Scr, TLC, VC, FEV1/FVC, MVV, DLCO, and DLCO/VA were all key factors in the development of clinical outcomes in DN P < 0.05 . ROC analysis showed that all of these important factors had an AUC greater than 0.75 for the diagnosis of DN with high sensitivity and specificity. Conclusion. Serum and renal function indices of DN patients gradually increased with stage, accompanied by a decrease in pulmonary ventilation, and diffusion function; SBP, HbA1c, FBG, 2hPG, BUN, Scr, TLC, VC, FEV1/FVC, MVV, DLCO, and DLCO/VA were all key factors affecting the clinical outcome of DN; controlling blood glucose, lipids, improving pulmonary ventilation, and diffusion function can better prevent the occurrence and worsening of DN.

Abstract:Diabetes Mellitus presents a formidable challenge as one of the most prevalent and complex chronic diseases, exerting significant strain on both patients and the world economy. It is recognized as a common comorbidity among severely ill individuals, often leading to a myriad of micro- and macro-vascular complications. Despite extensive research dissecting the pathophysiology and molecular mechanisms underlying vascular complications of diabetes, relatively little attention has been paid to potential lung-related complications. This review aims to illuminate the impact of diabetes on prevalent respiratory diseases, including chronic obstructive pulmonary disease (COPD), acute respiratory distress syndrome (ARDS), idiopathic pulmonary fibrosis (IPF), tuberculosis (TB), pneumonia infections, and asthma, and compare the vascular complications with other vascular beds. Additionally, we explore the primary mechanistic pathways contributing to these complications, such as the expression modulation of blood-tissue-barrier proteins, resulting in increased paracellular and transcellular permeability, and compromised immune responses rendering diabetes patients more susceptible to infections. The activation of inflammatory pathways leading to cellular injury and hastening the onset of these respiratory complications is also discussed.
Impact of Diabetes on Clinical Presentation and Treatment Outcome of Pulmonary Tuberculosis in Beijing

Abstract:SUMMARYDiabetes mellitus (DM) is currently known to be one of the risk factors for pulmonary tuberculosis (PTB) and the proportion of DM in PTB is rising along with the increased prevalence of DM in countries with high PTB burden. This study was designed to explore the impact of DM on clinical presentation and treatment outcome of PTB in China. In an urban setting in Beijing, 1126 PTB patients, 30·6% with positive sputum smear, registered in two PTB dispensaries from January 2010 to December 2011 were screened for DM and were followed up prospectively during PTB treatment. DM was observed in 16·2% of patients with PTB. PTB with DM appeared to be associated with older age and a higher proportion of re-treatment. On presentation, DM was associated with more severe PTB signs with higher proportions of smear positivity [odds ratio (OR) 2·533, 95% confidence interval (CI) 1·779–3·606], cavity (OR 2·253, 95% CI 1·549–3·276) and more symptoms (OR 1·779, 95% CI 1·176–2·690). DM was also associated with non-TB deaths (OR 5·580, 95% CI 2·182–14·270,P < 0·001) and treatment failure (OR 6·696, 95% CI 2·019–22·200,P = 0·002). In Beijing, the findings of this study underlined the need to perform early bi-directional screening programmes and explore the underlying mechanism for different treatment outcomes for PTB with DM.
Vascular Endothelial Function as a Valid Predictor of Variations in Pulmonary Function in T2DM Patients Without Related Complications

Abstract:To assess the variations in pulmonary function and vascular endothelial function in their early stages (without related complications). A total of 162 type 2 diabetes mellitus (T2DM) patients without diabetes complications and 55 healthy people were selected, comprising the T2DM group and the control group, respectively, to evaluate changes in vascular endothelial function and lung function and determine the correlation between them. In this study, the T2DM group exhibited significantly lower pulmonary function than that of the control group ( P < 0.05). The T2DM group also showed significantly lower flow-mediated dilation (FMD) and nitric oxide (NO) ( P < 0.05) than those of the control group. Pulmonary functional indexes correlated positively with FMD and NO ( P < 0.05) and correlated negatively with endothelin-1 (ET-1) ( P < 0.05). FMD and NO correlated negatively with diabetes duration/HbA1c ( P < 0.05), whereas ET-1 correlated positively with glycosylated hemoglobinA1c (HbA1c)/diabetes duration ( P < 0.05). Pulmonary functional indexes negatively correlated with HbA1c/diabetes duration ( P < 0.05). Multiple linear regression was used to analyze the relationship between vascular endothelial function indexes (FMD, ET-1, and NO) and pulmonary functional indexes. The results indicated that each vascular endothelial function index (FMD, ET-1, and NO) was significantly correlated with the pulmonary functional index ( P < 0.05). The patients with T2DM presented changes in the subclinical vascular endothelial and pulmonary function. They also had impaired vascular endothelial functions, which were characterized by reduced vascular endothelial function relative to those of healthy people. Regulating glycemia may improve vascular endothelial and pulmonary functions. Moreover, microvascular lesions in preclinical stages, vascular endothelial function indexes (FMD, ET-1, and NO) were valid predictors of alterations in pulmonary function in T2DM patients without related complications. Air Pollution and Diabetes Mellitus: Association and Validation in a Desert Area in China

Abstract:Abstract Context Despite the growing evidence pointing to the detrimental effects of air pollution on diabetes mellitus (DM), the relationship remains poorly explored, especially in desert-adjacent areas characterized by high aridity and pollution. Objective We conducted a cross-sectional study with health examination data from more than 2.9 million adults in 2 regions situated in the southern part of the Taklamakan Desert, China. Methods We assessed 3-year average concentrations (2018-2020) of particulate matter (PM1, PM2.5, and PM10), carbon monoxide (CO), nitrogen dioxide (NO2), and sulfur dioxide (SO2) through a space-time extra-trees model. After adjusting for various covariates, we employed generalized linear mixed models to evaluate the association between exposure to air pollutants and DM. Results The odds ratios for DM associated with a 10 μg/m3 increase in PM1, PM2.5, PM10, CO, and NO2 were 1.898 (95% CI, 1.741-2.070), 1.07 (95% CI, 1.053-1.086), 1.013 (95% CI, 1.008-1.018), 1.009 (95% CI, 1.007-1.011), and 1.337 (95% CI, 1.234-1.449), respectively. Notably, men, individuals aged 50 years or older, those with lower educational attainment, nonsmokers, and those not engaging in physical exercise appeared to be more susceptible to the adverse effects of air pollution. Multiple sensitivity analyses confirmed the stability of these findings. Conclusion Our study provides robust evidence of a correlation between prolonged exposure to air pollution and the prevalence of DM among individuals living in desert-adjacent areas. This research contributes to the expanding knowledge on the relationship between air pollution exposure and DM prevalence in desert-adjacent areas.

Abstract:Abstract Background There is increasing evidence that the lung is a target organ of diabetes. This study aimed to examine in detail the association between diabetes mellitus and pulmonary function using a national cohort. We also aimed to explore the non-linear association between pulmonary function and blood glucose, insulin resistance, and C-reactive protein (CRP). Methods A total of 30,442 participants from the National Health and Nutrition Examination Survey from the period between 2007 and 2012 were included. The cross-sectional association between diabetes mellitus and pulmonary function was assessed using multiple linear regression. Where there was evidence of non-linearity, we applied a restricted cubic spline with three knots to explore the non-linear association. Partial mediation analysis was performed to evaluate the underlying mechanism. All analyses were weighted to represent the US population and to account for the intricate survey design. Results A total of 8584 people were included in the final study population. We found that diabetes was significantly associated with reduced forced expiratory volume in one second (FEV 1 ) and forced vital capacity. We further found L-shaped associations between hemoglobin A1c (HbA1c) and pulmonary function. There was a negative association between HbA1c and FEV 1 in diabetes participants with good glucose control (HbA1c < 7.0%), but not in patients with poor glucose control. A non-linear association was also found with fasting plasma glucose, 2 h-plasma glucose after oral glucose tolerance test, insulin resistance, and CRP. Finally, we found that diabetes duration did not affect pulmonary function, and the deleterious effect of diabetes on pulmonary function was mediated by hyperglycemia, insulin resistance, low-grade chronic inflammation (CRP), and obesity. Conclusions Diabetes mellitus is non-linearly associated with pulmonary function. Our finding of a negative association between HbA1c and FEV 1 in diabetes patients with good glucose control but not in patients with poor glucose control indicates that a stricter glycemic target should be applied to diabetic patients to improve pulmonary function. Given, the cross-sectional nature of this research, a longitudinal study is still needed to validate our findings.

Abstract:Pulmonary damage and function impairment were frequently noted in patients with diabetes mellitus (DM). However, the relationship between lung function and glycemic status in non-DM subjects was not well-known. Here, we evaluated the association of longitudinal changes of lung function parameters with longitudinal changes of glycated hemoglobin (HbA1c) in non-DM participants. The study enrolled participants without prior type 2 DM, hypertension, and chronic obstructive pulmonary disease (COPD) from the Taiwan Biobank database. Laboratory profiles and pulmonary function parameters, including forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1), were examined at baseline and follow-up. Finally, 7055 participants were selected in this study. During a mean 3.9-year follow-up, FVC and FEV1 were significantly decreased over time (both p < 0.001). In the multivariable analysis, the baseline (unstandardized coefficient β = −0.032, p < 0.001) and longitudinal change (unstandardized coefficient β = −0.025, p = 0.026) of FVC were negatively associated with the baseline and longitudinal change of HbA1c, respectively. Additionally, the longitudinal change of FVC was negatively associated with the risk of newly diagnosed type 2 DM (p = 0.018). During a mean 3.9-year follow-up, our present study, including participants without type 2 DM, hypertension, and COPD, demonstrated that the baseline and longitudinal change of FVC were negatively and respectively correlated with the baseline and longitudinal change of HbA1c. Furthermore, compared to those without new-onset DM, participants with new-onset DM had a more pronounced decline of FVC over time.

Abstract:Background::Previous studies have provided conflicting evidence about the increased overall survival (OS) in lung cancer patients with diabetes mellitus (DM) compared with those without DM. This study assessed progression-free survival (PFS)/OS in lung cancer patients with or without DM and tentatively analyzed the impact of blood glucose levels on PFS/OS in lung cancer patients.Methods::Data were collected from lung cancer patients based upon admission records from January 2010 to January 2012 and follow-up records from January 2010 to January 2015 in the Department of Pulmonary Medicine, Zhongshan Hospital, Fudan University, Shanghai. The data included patient sex, age, body mass index (BMI), smoking status, history of DM, level of blood glucose, pathological type, clinical stage of cancer, chemotherapy regimen, and history of anti-DM drugs. The Cox regression model and Kaplan-Meier method were used for the analysis of hazard factors and PFS/OS. For comparison of PFS/OS in lung cancer with or without DM, patients were divided into three groups: lung cancer with DM, lung cancer without DM but with elevated level of blood glucose, lung cancer without DM or elevated level of blood glucose.Results::In total, the data from 200 lung cancer patients (138 males/62 females, aged 29.0 to 78.0 years, mean 60.0 ± 8.6 years) were collected. For the comparison of PFS/OS in lung cancer patients with or without DM, patients were divided into three groups: lung cancer with DM ( n = 31); lung cancer without DM but with elevated levels of blood glucose ( n = 40); and lung cancer without both DM and elevated levels of blood glucose ( n = 128), whereas 1 patient dropped out of the study. All the patients underwent complete chemotherapy and were followed up for 36.0 to 60.0 months. Kaplan-Meier survival analysis showed that lung cancer patients with DM had increased PFS and OS compared with those without DM (log-rank, P < 0.05, P < 0.01); the median PFS in lung cancer with DM was 12.0 months (95% confidence interval [CI], 4.0-16.0) vs. 6.0 months in those without DM (95% CI, 5.8-6.3); and the median OS in lung cancer patients with DM was 37.0 months (95% CI, 29.0-46.6) vs. 12.0 months in those without DM (95% CI, 10.9-13.1). For the other two groups of patients without DM, there was a trend toward a shorter PFS and OS in patients with elevated blood glucose compared with those without elevated blood glucose. Cox regression showed that PFS in lung cancer patients was favorably associated with the usage of anti-DM drugs, BMI, clinical stage of cancer, and chemotherapy regimen (all P < 0.05) but was inversely associated with the level of blood glucose ( P < 0.05). Conclusions::Lung cancer patients with DM have prolonged PFS and OS compared with those without DM, and the level of blood glucose was inversely associated with PFS. The current results indicate that PFS may be a meaningful intermediate endpoint for OS and that the levels of blood glucose hopefully represent a prognostic factor in lung cancer patients.

Abstract:Objective:To discuss the related factors for the development of pulmonary tuberculosis among diabetes mellitus patients.Methods:71 patients with type 2 diabetes mellitus(T2DM) were devided into the observation group(87 cases) and the control group(84 cases) depending on whether complicated with pulmonary tuberculosis.And then 13 factors were contrasted between the two groups.Results:There was no significantly difference on family history of diabetes,drinking,and stress events nearly three months between the two groups(P>0.05);but there were significantly differences on duration of diabetes,daily blood glucose control,HbA1c,diet control,complications of diabetes,smoking,history of exposure to tuberculosis,tuberculosis family history,and daily physical exercise between the two groups(P<0.05);and daily blood glucose control,diet control,complications of diabetes,smoking,history of exposure to tuberculosis,tuberculosis family history,and daily physical exercise between the two groups were all independent risk factors for diabetic patients with tuberculosis.Conclusion:Controlling glycemic and diet strictly,treat the complications of diabetes aggressively,quiting smoking,strengthening publicity and education of the tuberculosis knowledge,adhering to proper physical exercise have important significance for the prevention of diabetic patients with tuberculosis.
Comparative study of spirometry parameters in chronic smokers with and without Type 2 Diabetes Mellitus (T2DM)

Abstract:Aim: The study aimed to know the changes in spirometry parameters in chronic smokers and evaluate how the presence of type 2 diabetes mellitus (T2DM) affects their lung function. Materials and methods: A prospective, cross-sectional, observational study was done for 12 months at a tertiary care hospital in the western region of Maharashtra State in India. Two groups of patients and one group of healthy volunteers aged 18 years or more were studied (with 50 in each group, n = 150). Group A consisted of smokers with T2DM, Group B- smokers without T2DM and Group 3- healthy controls who were non-smokers and non-diabetic. Spirometry was done for the following parameters: forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1/FVC ratio, and peak expiratory flow (PEF) and results compared between the three groups. Results: The mean age of the participants was 51.13 ± 10.74 years. There were 137 (91.3%) males and 13 (8.6%) females. Among the enrolled subjects, 66% had smoked for more than ten years. All spirometry parameters were significantly different across all three groups. When the spirometry parameters were compared between smokers with and without T2DM, all the parameters were significantly decreased (P < 0.05). FEV1 and FEV1/FVC were significantly decreased in T2DM patients with HbA1c >7%. Conclusion: The presence of T2DM in smokers significantly affects their pulmonary function tests. Uncontrolled T2DM (HbA1c >7%) can result in increased abnormality in the spirometry parameters studied. Thus, adequate glycemic control and cessation of smoking can be beneficial for the improvement of lung functions in smokers.
