Abstract:Dry eye-related ocular surface examination is very important in the diagnosis and treatment of dry eye disease. With the recent advances in science and technology, dry eye examination techniques have progressed rapidly, which has greatly improved dry eye diagnoses and treatment. However, clinically, confusion remains about which examination to choose, how to ensure the repeatability of the examination, and how to accurately interpret the examination results. In this review, we systematically evaluate previous examinations of dry eye, analyze the latest views and research hotspots, and provide a reference for the diagnosis and management of dry eye.

Abstract:Objective: To examine the time course of aqueous-deficient and meibomian gland dysfunction (MGD) in patients with primary Sjogren's Syndrome (pSS). Methods: This prospective study was conducted on pSS female patients in the Department of Rheumatism of the Second Affiliated Hospital, School of Medicine, Zhejiang University. The age-matched MGD female patients without pSS (non-SS-MGD) were recruited as MGD controls from the Eye Center of the Second Affiliated Hospital, School of Medicine, Zhejiang University. After providing written informed consent, the patients underwent an eye examination and completed an Ocular Surface Disease Index questionnaire that assessed the symptoms of dry eye disease. The subjects were evaluated using Schirmer I test (SIt), tear meniscus height (TMH), noninvasive keratographic break-up time (NIKBUT), corneal fluorescein staining (CFS), and meibomian gland evaluation (meibomian gland infrared, lid margin score, expressible meibomian glands number and the secretions quality). The patients were divided into two groups: early stage (≤3 years) and late stage (>3 years) according to their medical history of dry eye. The data were analyzed using SPSS 20.0. Results: There were 49 pSS and 52 non-SS-MGD female patients enrolled in this study from 1 January 2018 to 30 December 2018. There were no differences in age (49.38 ± 10.32 and 48.69 ± 13.57 years) and dry eye medical history (48.44 ± 40.16 and 47.79 ± 37.85 months) between the two groups. When the medical history was ≤3 years, the average SIt and TMH of the pSS patients were significantly smaller than those of the patients with MGD. However, the signs related to the MGD did not show a significant difference between the two groups. When the medical history was >3 years, both the SIt and TMH and the signs related to MGD in pSS group were significantly more severe than the MGD group. Conclusions: Our results demonstrated that 3 years may be an important time node for the dry eye development in pSS patients, before this, the lacrimal glands received a greater influence, and then the meibomian glands began to be greatly affected.
The treatment effectiveness of local application of glucocorticoid for meibomian gland dysfunction related dry eye

Abstract:Objective To investigate the treatment effectiveness of antibiotics alone or combined with glucocorticoid in patients with meibomian gland dysfunction(MGD) related dry eye.Design Prospective comparative case series.Participants 42 MGD related dry eye outpatients(84 eyes) in Second Affiliated Hospital of Zhejiang University School of Medicine in May 2012.Method The experimental group,22 patients(44 eyes),were treated with the tobramycin and dexamethasone eye ointment +tears naturale eye drops+ local physical therapy of meibomian gland;the control group: 20 patients(40 eyes) were treated with the tobramycin eye ointment + tears naturale eye drops + local physical therapy of meibomian gland for 1 month,respectively.Meibomian glands function and correlated indexes of dry eye were investigated before treatment and at 1st,2nd and 4th week after treatments.Slit lamp examination(the changes of limbus palpebralis and meibomian glands orifices,secretion characteristics,the difficulty of lipid excretion were evaluated for MGD degree);the symptom score,tear break-up time test(TBUT),corneal fluorescein staining(CFS),and Schirmer I test(SIt) were performed.Main Outcome Measures Secretion characteristics,the difficulty of lipid excretion,the symptom score,TBUT,CFS,and SIt.Results Before treatment,there were no statistical difference in age,the course of disease,MGD physical signs and dry eye related indexes between experimental group and control group(all P>0.05).At each time point after treatment,the secretion characteristics,the difficulty of lipid excretion,and the subjective symptom score were all decreased obviously than before treatment in both groups;and TBUT increased significantly(all P<0.05).The CFS at 2nd and 4th week after treatment were significantly reduced compared with before treatment(all P<0.05).At 1st week and 2nd week after treatment,secretion characteristics and the difficulty of lipid excretion scores were significantly reduced in experimental group than in control group(all P<0.05).Significant increase of TBUT was also observed at all time points after treatment in experimental group than in control group(all P<0.05).Conclusion The anti-inflammatory treatments with antibiotics alone or comined with glucocorticoid were both effective for meibomian gland dysfunction related dry eye.The glucocorticoid can more significantly improve the symptoms and signs for patients with meibomian gland dysfunction related dry eye.
Analysis of the Meibomian Glands Condition in Dry Eye Patients

Abstract:OBJECTIVE:To evaluate the condition of meibomian glands in dry eye patients.METHODS:Non interventional, observational study. 388 eyes from 194 patients which were diagnosed as dry eye during March to December 2010 were collected, 69 males (138 eyes), 125 females (250 eyes). Patients' age range 7-85 years, mean (44±18) years. Symptom score, Schirmer test Ia (SIaT), tear film break-up time (BUT) and superficial punctuated keratopathy were measured. The meibomian glands were observed using the noncontact meibography system.RESULTS:In all subjects, symptom score 5.47±2.14, SIaT (5.58±4.22) mm, BUT (3.09±2.34) s, superficial punctuated keratopathy score 4.10±3.79. Meibo-score: 1 point, 3.09% (12 eyes); 2 points, 9.28% (36 eyes); 3 points, 20.10% (78 eyes); 4 points, 26.03% (101 eyes); 5 points, 21.91% (85 eyes); 6 points, 19.59% (76 eyes).CONCLUSIONS:Abnormal meibomian glands occupied a extremely high proportion of dry eye which indicate that meibomian glands disorder plays important role in dry eye. Noncontact meibography system could show accurate structure of meibomian glands.
The Efficacy of Intense Pulsed Light Combined with Meibomian Gland Expression for the Treatment of Dry Eye Disease Due to Meibomian Gland Dysfunction: A Multicenter, Randomized Controlled Trial.

Abstract:Objectives: To compare the efficacy of intense pulsed light (IPL) combined with Meibomian gland expression (MGX), and instant warm compresses combined with MGX, for treatment of dry eye disease (DED) due to meibomian gland dysfunction (MGD). Methods: In a prospective, multicenter, interventional study, 120 subjects with DED due to MGD were randomized 1:1 to an IPL arm or a control arm. Each subject was treated 3 times at 3-week intervals. The primary outcome measure was the tear break up time (TBUT). Tear break up time and a few additional outcome measures were evaluated at the baseline and at 3 weeks after the last treatment. Results: All outcome measures improved in both arms, but in general, the improvement was significantly larger in the IPL arm. Tear break up time increased by 2.3 +/- 1.9 and 0.5 +/- 1.4 sec, in the IPL and control arms respectively (P<0.001). SPEED was reduced by 38% and 22% in the IPL and control arms, respectively (P<0.01). Meibomian Gland Yielding Secretion Score was improved by 197% in the IPL arm and 96% in the control arm. Corneal fluorescein staining also decreased by 51% and 24% in the IPL and control arms respectively, but the differences between the two arms were not statistically significant (P=0.61). A composite score of lid margin abnormalities improved in both arms, but more in the IPL arm (P<0.05). Conclusions: Intense pulsed light combined with MGX therapy was significantly more effective than instant warm compresses followed with MGX. This suggests that the IPL component has a genuine contribution to the improvement of signs and symptoms of DED.

CASE REPORTS The examination including slitlamp examination,tear break-up time(BUT), schirmerⅠtest(SIt) and fluorescent(fl) staining were performed in 97 outpatients with symptoms of asthenopia,foreign body sensation and dryness.The positive cases in BUT,SIt,fl staining and obstruction of tarsal glands and were 51,38,31 and 34 cases,respectively.DISCUSSION Less part of patients with symptom of dry eye can be diagnosed as xerophthalmia.
Correlation Study Between Abnormal Morphology of Meibomian Glands and Meibum in Patients With Dry Eye Disease Under in vivo Confocal Microscopy

Abstract:Objective: To observe differences in meibomian gland morphology among patients with meibomian gland dysfunction (MGD) with liquid meibum, with solid meibum, and a non-MGD group by laser scanning in vivo confocal microscopy (IVCM), and then analyze the correlation between meibomian gland abnormalities and dry eye disease. Methods: Twenty-one patients with MGD (42 eyes) with liquid meibum, 21 patients with MGD (38 eyes) with solid meibum, and 24 non-MGD patients (39 eyes) were enrolled in the study. IVCM examination and Schirmer II test were performed, and non-invasive tear-film breakup time (NIBUT) was measured. Results: Data obtained from all the patients were normally distributed; therefore, one-way analysis of variance was performed. The meibomian gland opening diameter and gland opening area of the patients with MGD were greater than those of the non-MGD patients, and values of the liquid meibum group were greater than those of the solid meibum group ( F = 17.96, p < 0.001; F = 8.84, p < 0.001, respectively). Due to changes in meibomian gland opening diameter and dilation of meibomian gland acini, the acinar longest diameter and unit area of the patients with MGD were also greater than those of the non-MGD patients, and the values of the solid meibum group were greater than those of the liquid meibum group ( F = 36.52, p < 0.001; F = 27.81, p < 0.001, respectively). In the aspect of acinar shortest diameter, there was no difference among the three groups ( F = 0.24, P > 0.05). Highest inflammatory cell density was observed in the solid meibum group, followed by the liquid meibum group, and the non-MGD group ( F = 111.54, p < 0.001). Similarly, the results of the Schirmer II test and NIBUT showed that the condition of the patients with MGD in the solid meibum group was worst, followed by that of the liquid meibum group and the non-MGD group ( F = 99.57, p < 0.001; F = 11.87, p < 0.001, respectively). Conclusions: The different meibum in the patients with MGD is accompanied by different meibomian gland morphologies. Compared with the patients with liquid meibum, those with solid meibum have more obvious dilatation of meibomian glands under confocal microscopy and in these patients, the density of inflammatory cells among glands is higher, and the quality of tears is worse.

Abstract:Objective To evaluate the differences in three kinds of meibomian phospholipid composition in healthy subjects and in patients suffering from meibomian gland dysfunction (MGD).Methods In this prospective study, 22 MGD patients (MGD group) and 30 age and sex matched heahhy individuals (control group) were enrolled.One eye of each subject was included in the study.MGD patients underwent sequential examinations as follows: the property of meibomian secretion, tear film break-up time (BUT), corneal fluorescein staining (CFS), Schirmer Ⅰ test and lid margin and ocular surface examinations by slit lamp.Meibomian gland secretions were collected using sterile stainless steel curettes in the healthy control group and MGD group after gentle massage of the lid margin with sterile cotton swabs.The meibomian phospholipids, mainly lysophosphatidylcholine,phosphatidylethanolamine, and sphingomyelin, were detected using liquid chromatography/tandem mass spectrometry (LC-MS/MS), and comparisons were made between the MGD group and the healthy control group.The data from the two groups were compared by an independent-samples t test and a Pearson correlation analysis was used to assess the correlation between the relative composition concentrations of the three kinds of phospholipids with the clinical parameters of the objective signs of MGD.Results The relative concentration level of lysophosphatidylcholine in the MGD group was significantly higher than that in the healthy controls (t=-2.45, P＜0.05), while phosphatidylethanolamine and sphingomyelin levels were lower in the MGD group than in healthy controls (t=16.73, P＜0.01 and t=18.59, P＜0.01, respectively).The relative concentration of lysophosphatidylcholine was positively correlated with the property of meibomian secretion, CFS, lid margin and ocular surface examination by slit lamp (r=0.602, 0.716, 0.710, P＜0.01, respectively);while a negative correlation was found with the BUT and Schirmer Ⅰ test (-0.628,-0.477, P＜0.05, respectively).The relative concentrations of phosphatidylethanolamine and sphingomyelin were negatively correlated with the properties of the meibomian secretion, CFS, lid margin and ocular surface examination (r=-0.565,-0.724,-0.702;r=-0.597,-0.719,-0.698, P＜0.01, respectively).However, a positive correlation was detected with the BUT and Schirmer Ⅰ test (r=0.659, 0.498, P＜0.05;r=0.631, 0.468, P＜0.05, respectively).Conclusion The profile of the three kinds of meibomian phospholipids (lysophosphatidylcholine, phosphatidylethanolamine and sphingomyelin) in MGD patients is dramatically different from healthy subjects.Its significant correlation with the clinical parameters of the signs of meibomian gland dysfunction suggest that a change in phospholipids composition in meibomian gland secretions is a probable reason for the development of MGD.

Abstract:Purpose To compare the levels of inflammatory molecules in tear samples between patients with meibomian gland dysfunction (MGD)-related evaporative dry eye (EDE) and healthy subjects and to analyze the correlations between the levels of tear inflammatory molecules and ocular surface parameters. Methods A total of 30 MGD-related EDE patients (48 eyes) and ten healthy volunteers (15 eyes) were enrolled. Dry eye-related examinations and questionnaires were obtained from all participants. The levels of nine inflammatory molecules were determined through multiplex bead analysis. Results Inflammatory molecules including ICAM-1, IFN-γ, CXCL8/IL-8, IL-6, TNF-α and IL-12p70 were detected in 100% of the patients, while IL-1α, IL-1β and IL-10 were detected in 56.25%, 13.60% and 45.83% of the patients, respectively. Moreover, ICAM-1, IL-8, IL-6, TNF-α, IL-12p70 and IFN-γ were detected in 86.67–100% of the healthy subjects, and the detection rates of IL-10, IL-1α and IL-1β were below 50%. The levels of IL-8, IL-6, IFN-γ and ICAM-1 were significantly higher in the patient group compared with the control group. In addition, IL-8 and IL-6 were negatively correlated with Schirmer I test. Besides, IFN-γ was negatively correlated with tear film breakup time. Furthermore, ICAM-1 and IL-6 were positively correlated with meibography score. Conclusions Collectively, patients with MGD-related EDE had higher levels of inflammatory molecules in their tears, and some molecules were correlated with ocular surface parameters. These findings suggested that inflammation played an important role in MGD-related EDE, and several inflammatory molecules could be used in the diagnosis and the treatment of MGD-related EDE.
[Changes in human meibum with meibomian gland dysfunction].

Abstract:Meibomian gland dysfunction (MGD) is a common ocular surface disease, characterized by terminal duct obstruction and (or) qualitative and quantitative changes in the glandular secretion. Meibomian lipids are a mixture consisting of various lipids, and they form the lipid layer of tear film and play important roles in preventing the evaporation and maintaining the stability of tear film. Patients with MGD may have different compositions of meibomian lipids, which could be an important indicator for diagnosis of MGD. The methods to analyze the composition of meibomian lipids mainly include chromatography and spectrum analysis. Because the pathogenic factors of MGD are still unclear, the main treatment is to relieve the symptoms. It can contribute to diagnose and treat MGD if we can find some convenient and effective methods to analyze the quality and quantity of meibomian lipids.

Abstract:Meibomian gland dysfunction (MGD)is a common ocular surface disease in clinic and characterized by terminal duct obstruction and/or qualitative/quantitative changes in the glandular secretion.Meibomian gland obstruction may decrease superficial lipid element of the tear film,resulting in evaporative dry eye.Inflammation of lid margin and meibomian gland may result in conjunctival and corneal disease.Diagnosis of MGD depends on symptom and sign.Symptom of MGD is similar to dry eye,so MGD is nonspecific in diagnosis.Signs of MGD include the changes in morphology of lid margin,quantification of meibum expressibility/quality and dropout of meibomian gland.The managing regimes for MGD are warm,clean compresses and lid massage to increase secretion of meibomian,meanwhile,anti-infection and anti-inflammation and artificial lubricants also are available.Antiinflammation is essential for moderate to severe cases,and the treating medicines include steroid,nonsteroidal antiinflammatory drugs (NSAIDs)and immunosuppressive agents.Evaluation of meibomian gland function is very important when ophthalmologist check ocular surface with slit-lamp,especially for the patients going to perform refractive and intro-operation,so that avoid severe ocular surface complications and evade medical risks.
Meibomian Gland Dropout in Patients with Dry Eye Disease in China.

Abstract:Purpose: To examine the morphological changes in the meibomian glands of eyes of patients with dry eye disease using the non-contact infrared meibography system and to assess their relationship with meibomian dropout, signs, and tear-film function.Methods: Subjects included 264 randomly selected patients (528 eyes) suffering from dry eye disease (95 males, 169 females; age range, 7-85 years; mean male age, 39.83 +/- 19.17 years; mean female age, 46.16 +/- 17.38 years). Tear-film break-up time (BUT) was measured and tear-film production was evaluated by the Schirmer test I (SIT). Subjective symptoms were also scored. The upper and lower eyelids were turned over, and the meibomian glands were observed using the non-contact meibography system. Partial or complete loss of the meibomian glands (meibomian dropout) was scored for each eyelid from grade 0 (no loss) through grade 3 (lost area was >2/3 of the total meibomian gland area).Results: The average SIT result was 6.71 +/- 6.13 mm (range 0-30 mm) and that for BUT was 3.13 +/- 2.39 s (range 0-10 s). The average fluorescein staining score was 4.25 +/- 4.05 (range 0-12). In addition, the mean meibomian gland photographic score was 4.35 +/- 1.39. The correlation between the meibomian gland photographic scores and various ocular surface examinations were evaluated using Kendall's correlation. SIT and BUT were significantly negatively correlated with the meibomian gland photographic score, whereas corneal fluorescein staining was positively correlated.Conclusions: The results suggest a large proportion of meibomian dropout cases among patients with dry eye disease, indicating that treatment targeted at the meibomian gland will become an important direction for treating dry eye disease. Meibography is recommended as a routine test for dry eye disease.

Abstract:Dry eye disease is one of the most common ophthalmic complaints; it results from the activity of various pathways and is considered a multifactorial disease. An important factor that contributes to the onset of dry eye disease is meibomian gland dysfunction. Meibomian gland dysfunction causes a disruption in the tear film lipid layer which affects the rate of tear evaporation. This evaporation leads to tear hyperosmolarity, eventually triggering the onset of dry eye disease. Dry eye disease and meibomian gland dysfunction are strongly associated with each other, such that many of their risk factors, signs, and symptoms overlap. This review aimed to provide an update on the association between dry eye disease and meibomian gland dysfunction. A stepwise approach for diagnosis and management is summarised.

Methods: A total of 144 eyes were recruited in the study, in which 72 eyes were from patients diagnosed with dry eye and 72 eyes were from healthy subjects. All subjects finished following tests or examinations:ocular surface disease index (OSDI) to evaluate eye symptoms; Keratograph 5M examination to obtain tear meniscus height (TMH), noninvasive tear break-up time (NIBUT) including first NIBUT (NIBUT-Fir) and average NIBUT (NIBUT-Ave), and infrared meibography; and fluorescein sodium staining to obtain fluorescein tearbreak-up time (FBUT). Results: Dry eye group had higher OSDI score than healthy control group, but its TMH, NIBUT-Fir and NIBUT-Ave were lower than those in healthy control group (all P<0.01). Total meiboscore in dry eye group was higher than that in healthy control group (P<0.01), and it showed a significant correlation with NIBUT-Fir and NIBUT-Ave (r=-0.449 and -0.398, P<0.01), but no correlation with ages was observed (r=0.031, P>0.05). The NIBUT-Fir and NIBUT-Ave showed a significant correlation with FBUT (r=0.833 and 0.727, P<0.01). Conclusion: Keratograph 5M is a convenient, accurate and non-invasive method to assess the function of tear film and meibomian gland, and the new meibography scoring system can evaluate the function of meibomian gland objectively and succinctly.
Evaluation of meibomian gland dysfunction in type 2 diabetes with dry eye disease: a non-randomized controlled trial

Abstract:Background: The purpose of this investigation was to evaluate the morphology and physiological function of the meibomian glands between type 2 diabetics with dry eye disease (DED) and control subjects. Doing so will help to better reveal the pathologic mechanisms of meibomian gland dysfunction (MGD) and DED in type 2 diabetes mellitus (T2DM). Methods: Ninety subjects were divided into the following four groups: DM-DED group: T2DM patients with DED (n = 30); DM control group: DM patients without DED (n = 18); DED group: DED patients without DM (n = 26); and normal control group: normal subjects (n = 16). All participants administered the ocular surface disease index (OSDI) questionnaire, tear meniscus height (TMH), noninvasive Keratograph tear film break-up time (NIKBUT), Schirmer I test (SIT), corneal fluorescein staining (CFS), eyelid margin abnormality examinations, meibum quality and meibomian gland (MG) dropout evaluations. Results: The percentage of MG dropout in the upper and lower lids was significantly higher in the DM-DED group than the DED group (P < 0.05 or P < 0.01). However, there was no significant difference in other MG parameters between these two groups. Oppositely, Significant difference was observed in all of MG parameters except MG dropout in the lower lids comparing DM group with normal controls (P < 0.05 or P < 0.01). While the SIT values decreased in the DM-DED group compared to the DED group (P < 0.05), no significant differences were found in the values of other tear parameters. Conclusions: The higher prevalence and increased severity of MGD was found in patients with both T2DM and DED compared to those only with DED. Abstract:Purpose. To investigate meibomian gland and tear film function in patients with type 2 diabetes.Methods. This prospective study compared changes in meibomian gland and tear film function in type 2 diabetic patients with nondiabetic patients. Meibomian gland function was evaluated by measuring lipid layer thickness (LLT), grading of meibomian gland loss, lid margin abnormalities, and expression of meibum. Tear film function was assessed by measuring tear breakup time (TBUT), the Schirmer I test, noninvasive breakup time (NIBUT), tear meniscus height (TMH), and corneal fluorescein staining.Results. Meibography scores were significantly higher in the diabetic group compared with the nondiabetic group (p=0.004). The number of expressible glands was significantly lower in the diabetic group in temporal, central, and nasal third of the lower eyelid (nasal:p=0.002; central:p=0.040; and temporal:p=0.039). The lid margin abnormality score was significantly higher in the diabetic group than in the nondiabetic group (p=0.04). There was no statistically significant difference in the tear film function parameters between the two groups.Conclusions. Meibomian gland dysfunction (MGD) in type 2 diabetic patients is more severe compared with nondiabetic patients. Overall, most of the diabetic patients manifest as having asymptomatic MGD.
