Study on the impairment of executive function in patients with single subcortical lesion stroke

Abstract:Objective To investigate the impairment of executive function in patients with single subcortical lesion stroke,and the rationality of using Montreal cognitive assessment (MoCA) (Beijing edition) to assess these patients. Methods Thirty one patients with single subcortical lesion stroke and 20 healthy controls were enrolled. Trail-making test (TMT),Stroop colour-words test (CWT),and similarities test (ST) were used to assess the executive function. Results ① Executive function performance of patients with single subcortical lesion stroke was inferior to that of the health controls in mental processing,response inhibition and set shifting. ② Patients with left side lesion had both the worsening in response inhibition and speed of mental processing,while patients with right side lesion had only the worsening in speed of mental processing. ③ Patients with thalamus stroke had worsening in response inhibition and speed of mental processing and patients with stroke in basal ganglia district had worsening in speed of mental processing and set-shifting. Patients with stroke in paraventricular white matter had worsening in speed of mental processing. There was no difference between left and right side lesion in these three groups. Conclusions ① Impairment of all executive function domains may be involved in single subcortical lesion stroke,especially the lowering of speed of mental processing. ② MoCA has its limit in assessing executive function impairment after single subcortical lesion stroke. Time assessing is recommended.
