Abstract:Mitochondrial diseases have traditionally been ascribed to defects of the respiratory chain,which helped researchers explain their genetic and clinical complexity.However,other mitochondrial functions are greatly important for the nervous system,including protein importation,organellar dynamics,and cell death.Defects in genes controlling these functions are attracting increasing attention as causes not only of neurological and psychiatric diseases but also of age-related neurodegenerative disorders.After discussing some pathogenic conundrums regarding the neurological diseases of the respiratory chain defects,we review altered mitochondrial dynamics in the etiology of specific neurological diseases and in the physiopathology of more common neurodegenerative disorders.

Abstract:Mitochondrial disease (MD) is a group of metabolic disorders,caused by mitochondrial DNA or nuclear DNA mutations.With the development of research in mitochondrial medicine,the phenotypic spectrum of MD has expanded significantly.The common phenotypes in Chinese population included mitochondrial encephalopathy with lactic acidosis and stroke like episodes,Leigh syndrome,Leber hereditary optic neuropathy and chronic progressive external ophthalmoplegia.The rare phenotype included myoclonic epilepsy with ragged-red fibers,Kearns-Sayre syndrome,sensory ataxic neuropathy,mitochondrial neurogastrointestinal encephalomyopathy,Alpers disease,limb girdle mitochondrial myopathy,neuropathy with ataxia and retinitis pigmentosa.However,considerable clinical variability exists and some individuals do not fit into the known disease category.The revolution in genetic technologies has dramatically improved the diagnosis strategy of MD.In this article the pathology,clinical symptoms,laboratory findings and therapy in MD are sumarrized so as to provide guidance for clinical practice.

Abstract:Mitochondrial disorders are clinical phenotypes associated with mitochondrial dysfunction, which can be caused by mutations in mitochondrial DNA (mtDNA) or nuclear genes. In this review, we summarized the pathogenic mutations of nuclear genes associated with mitochondrial disorders. These nuclear genes encode, components of mitochondrial translational machinery and structural subunits and assembly factors of the oxidative phosphorylation, that complex. The molecular mechanisms, that nuclear modifier genes modulate the phenotypic expression of mtDNA mutations, are discussed in detail.
[Diagnosis of mitochondrial disorders in children with next generation sequencing]

Abstract:Objective: To explore the application value of next generation sequencing (NGS) in the diagnosis of mitochondrial disorders. Method: According to mitochondrial disease criteria, genomic DNA was extracted using standard procedure from peripheral venous blood of patients with suspected mitochondrial disease collected from neurological department of Beijing Children's Hospital Affiliated to Capital Medical University between October 2012 and February 2014. Targeted NGS to capture and sequence the entire mtDNA and exons of the 1 000 nuclear genes related to mitochondrial structure and function. Clinical data were collected from patients diagnosed at a molecular level, then clinical features and the relationship between genotype and phenotype were analyzed. Result: Mutation was detected in 21 of 70 patients with suspected mitochondrial disease, in whom 10 harbored mtDNA mutation, while 11 nuclear DNA (nDNA) mutation. In 21 patients, 1 was diagnosed congenital myasthenic syndrome with episodic apnea due to CHAT gene p.I187T homozygous mutation, and 20 were diagnosed mitochondrial disease, in which 10 were Leigh syndrome, 4 were mitochondrial encephalomyopathy with lactic acidosis and stroke like episodes syndrome, 3 were Leber hereditary optic neuropathy (LHON) and LHON plus, 2 were mitochondrial DNA depletion syndrome and 1 was unknown. All the mtDNA mutations were point mutations, which contained A3243G, G3460A, G11778A, T14484C, T14502C and T14487C. Ten mitochondrial disease patients harbored homozygous or compound heterozygous mutations in 5 genes previously shown to cause disease: SURF1, PDHA1, NDUFV1, SUCLA2 and SUCLG1, which had 14 mutations, and 7 of the 14 mutations have not been reported. Conclusion: NGS has a certain application value in the diagnosis of mitochondrial diseases, especially in Leigh syndrome atypical mitochondrial syndrome and rare mitochondrial disorders.

Abstract:Although mitochondrial disorders are increasingly being recognized, confirming a specific diagnosis remains a great challenge due to the genetic and clinical heterogeneity of the disease. The heteroplasmic nature of most pathogenic mitochondrial DNA mutations and the uncertainties of the clinical significance of novel mutations pose additional complications in making a diagnosis. Suspicion of mitochondrial disease among patients with multiple, seemingly unrelated neuromuscular and multi-system disorders should ideally be confirmed by the finding of deleterious mutations in genes involving mitochondrial biogenesis and functions. The genetics are complex, as the primary mutation can be either in the nuclear or the mitochondrial DNA (mtDNA). MtDNA mutations are often maternally inherited, but can also be sporadic or secondary to mutations in nuclear-encoded mitochondrial-targeted genes. Several well-defined clinical syndromes associated with specific mutations have been described, yet the genotype-phenotype correlation is often unclear and most patients do not fit within any defined syndrome and even within a family the expressivity of the disease can be extremely variable. This article describes examples representing diagnostic challenges of mitochondrial diseases that include the limitations of the mutation detection method, the occurrence of mitochondrial disease in families with another known neuromuscular disorder, atypical clinical presentation, the lack of correlation between the degree of mutant heteroplasmy and the severity of the disease, variable penetrance, and nuclear gene defects causing mtDNA depletion.

Abstract:MRI and 1H magnetic resonance spectroscopy (1HMRS) are the main neuroimaging methods to study mitochondrial diseases. MRI can demonstrate seven 'elementary' central nervous system (CNS) abnormalities in these disorders, including diffuse cerebellar atrophy, cerebral atrophy, symmetric signal changes in subcortical structures (basal ganglia, brainstem, cerebellum), asymmetric signal changes in the cerebral cortex and subcortical white matter, leukoencephalopathy, and symmetric signal changes in the optic nerve and the spinal cord. These elementary MRI abnormalities can be variably combined in the single patient, often beyond what can be expected based on the classically known clinical-pathological patterns. However, a normal brain MRI is also possible. 1HMRS has a diagnostic role in patients with suspected mitochondrial encephalopathy, especially in the acute phase, as it can detect within the lesions, but also in normal appearing nervous tissue or in the ventricular cerebrospinal fluid (CSF), an abnormally prominent lactate peak, reflecting failure of the respiratory chain with a shift from the Krebs cycle to anaerobic glycolysis. So far, studies correlating MRI findings with genotype in mitochondrial disease have been possible only in small samples and would greatly benefit from data pooling. MRI and 1HMRS have provided important information on the pathophysiology of CNS damage in mitochondrial diseases by enabling in vivo non-invasive assessment of tissue abnormalities, the associated changes of blood perfusion and cellular metabolic derangement. MRI and 1HMRS are expected to serve as surrogate biomarkers in trials investigating therapeutic options in mitochondrial disease.

Abstract:The anatomic complexity of the brain in combination with its high energy demands makes this organ specifically vulnerable to defects of mitochondrial oxidative phosphorylation. Therefore, neurodegeneration is a hallmark of mitochondrial diseases. The nervous system of affected individuals typically shows selective regional vulnerability leading to distinct patterns of tissue damage. A classic example is Leigh syndrome, which causes symmetric alterations of basal ganglia and brain stem. Leigh syndrome can be caused by different genetic defects (>75 known disease genes) with variable disease onset ranging from infancy to adulthood. Other mitochondrial diseases are characterized by focal brain lesions, which is a core feature of MELAS syndrome (mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes). Apart from gray matter, also white matter can be affected by mitochondrial dysfunction. White matter lesions vary depending on the underlying genetic defect and may progress into cystic cavities. In view of the recognizable patterns of brain damage in mitochondrial diseases, neuroimaging techniques play a key role in diagnostic work-up. In the clinical setting, magnetic resonance imaging (MRI) and MR spectroscopy (MRS) are the mainstay of diagnostic work-up. Apart from visualization of brain anatomy, MRS allows the detection of metabolites such as lactate, which is of specific interest in the context of mitochondrial dysfunction. However, it is important to note that findings like symmetric basal ganglia lesions on MRI or a lactate peak on MRS are not specific, and that there is a broad range of disorders that can mimic mitochondrial diseases on neuroimaging. In this chapter, we will review the spectrum of neuroimaging findings in mitochondrial diseases and discuss important differential diagnoses. Moreover, we will give an outlook on novel biomedical imaging tools that may provide interesting insights into mitochondrial disease pathophysiology.
The Pathological Features of Common Hereditary Mitochondrial Dynamics Neuropathy

Abstract:ObjectivesMitofusin 2 and ganglioside-induced differentiation-associated protein 1 are two main mitochondrial dynamics-related proteins. Dysfunction of these two proteins leads to different subtypes of Charcot–Marie–Tooth disease type 2A (CMT2A) and CMT2K. This study aims to report the pathological difference between CMT2A and CMT2K in a large cohort.MethodsThirty patients with molecularly confirmed CMT2A and nine with CMT2K were identified by next-generation sequencing. Sural nerve biopsies were performed in 29 patients.ResultsThe patients with both diseases showed length-dependent neuropathy with distal weakness, sensory loss, and no deep tendon reflex. Optic neuropathy appeared in 3/30 (10%) patients with CMT2A. Tendon contracture appeared in 4/9 (50.0%) patients with CMT2K. Sural biopsy revealed the loss of both myelinated and unmyelinated nerve fibers. Closely packed, irregularly oriented neurofilaments were observed in axons of unmyelinated nerve fibers in both diseases. Another important finding was the ubiquitous presence of smaller, rounded, and fragmented mitochondria in CMT2A and elongated mitochondria in CMT2K in the myelinated and unmyelinated axons.ConclusionThis study confirmed large diversity in phenotypes between CMT2A and CMT2K. Mitochondrial dynamics-related variations can induce different mitochondrial morphological changes and neurofilament accumulation in axons.
Neuroimaging characteristics and genetic correlations in pediatric mitochondrial disorders: a cohort study from China

Abstract:Abstract Background Mitochondrial disorders present with high clinical, genetic and neuroimaging heterogeneity. The spectrum of neuroimaging characteristics expanded fast based on the expanding of molecular etiologies. Methods In this study, we retrospectively recruited a cohort of primary pediatric mitochondrial disorder patients with confirmed genetic diagnosis. The clinical presentation, brain CT/MRI data, genetic results and biological dysfunction were collected and reviewed.Results Seventy-seven patients were enrolled, including 54 patients with mtDNA (70.1%) and 23 patients with nDNA mutations (29.9%). The most common MRI findings were deep nuclei lesion (65.1%), brain atrophy (50.0%), cortical and subcortical white matter lesion (36.4%), deep white matter lesion (30.3%), and diffused cerebral atrophy (10.6%). Overall lesion in diffused deep white matter was more common in nDNA-mutated patients (p=0.0365). Deep nuclei lesion was observed most frequently in patients with mutations in MT-ATP6, MT-ND6, and NDUFAF5 genes, while cortical/subcortical white matter lesion was common in patients with MT-TL1 gene, mtDNA large-scale deletion, or NDUFAF5 gene defects. The most common MRI finding in patients with m.3243A>G mutation was simultaneous cortical/subcortical white matter and deep nuclei involvement (9/24, 37.5%), followed by cortical grey/subcortical white matter lesion (7/24, 29.2%), unilateral or bilateral lesion in deep nuclei (5/24, 20.8%), whereas some patients showed normal MRI (3/24, 12.5%). The most common MRI change in POLG-mutated patients was brain atrophy (4/4,100%), lesion in deep nuclei (3/4, 75%), deep white matter (2/4, 50%) and cortical grey/subcortical white matter lesion (1/4,25%). Heatmap showed multi deep nuclei lesion was more common in patients with OXPHOS subunits defects; cortical grey and subcortical white matter lesion was more common in dysfunction of mtDNA maintenance, expression and translation; and lesion in deep white matter was more common in patients with dysfunction of metabolism of cofactors. Discussion Neuroimaging findings in pediatric mitochondrial disorder patients are variable. The most common patterns are deep nuclei lesion, brain atrophy, cortical grey/subcortical white matter lesion, deep white matter lesion, and cerebellar atrophy. There are some correlations between brain MRI results of patients and their genetic defects, as well as biological dysfunction.

Because of the unique features of the mitochondria, these diseases display characteristic modes of inheritance and a large degree of phenotypic variability. Recent studies have suggested that mitochondrial dysfunction plays a central role in a wide range of age-related disorders and various forms of cancer.

Abstract:Objective To investigate the neuroimaging features in mitochondrial encephalomyopatbies with lactic acidosis and stroke-like episodes(MELAS).Methods Twenty-two clinically diagnosed patients who came from department of neurology,Huashan hospital in October 2003 to July 2006 were analyzed for CT,MRI,MRI contrast,MRA and MRS.Results In all 22 patients,the neuroimaging results of 21 were positive.There were 9 patients lying in hemisphere,12 in both cerebral hemispheres,including occipital,parietal,temporal and frontal lobe.The abnormal areas showed low signal intensity on T_1-weighted MRI,high signal intensity on T_2-weighted MRI and fluid attenuated inversion recovery(FLAIR)images.The lesions of 12/16 patients on MR contrasted images were enhanced.The lesions of one patient showed malacoma-like changes,one showed Fahr syndrome' s change and another showed high signal intensity on MR contrasted images.Conclusion Although the neuroimaging features of MELAS are complicated,the specific ones could help to make the diagnosis.
The clinical and genetic spectrum of mitochondrial diseases in China: A multicenter retrospective cross-sectional study

Abstract:Mitochondrial diseases (MtDs) present diverse clinical phenotypes, yet large-scale studies are hindered by their rarity. This retrospective, multicenter study, conducted across five Chinese hospitals' neurology departments from 2009 to 2019, aimed to address this gap. Nationwide, 1351 patients were enrolled, with a median onset age of 14.0 (18.5) years. The predominant phenotype was mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) (45.0%). Mitochondrial DNA (mtDNA) mutations were prevalent (87.4%), with m.3243A>G being the most common locus (48.7%). Meanwhile, POLG mutations in nuclear DNA (nDNA) accounted for 16.5%. Comparative analysis based on age groups (with a cut-off at 14 years) revealed the highest prevalence of MELAS, with Leigh syndrome (LS) and chronic progressive external ophthalmoplegia (CPEO) being the second most common phenotypes in junior and senior groups, respectively. Notably, the most commonly mutated nuclear genes varied across age groups. In conclusion, MELAS predominated in this Chinese MtD cohort, underscored by m.3243A>G and POLG as principal mtDNA mutations and pathogenic nuclear genes. The phenotypic and genotypic disparities observed among different age cohorts highlight the complex nature of MtDs.

Abstract:Mitochondrial disorders (MIDs) occasionally manifest as polyneuropathy either as the dominant feature or as one of many other manifestations (inherited mitochondrial neuropathy). MIDs in which polyneuropathy is the dominant feature, include NARP syndrome due to the transition m.8993T>, CMT2A due to MFN2 mutations, CMT2K and CMT4A due to GDAP1 mutations, and axonal/demyelinating neuropathy with external ophthalmoplegia due to POLG1 mutations. MIDs in which polyneuropathy is an inconstant feature among others is the MELAS syndrome, MERRF syndrome, LHON, Mendelian PEO, KSS, Leigh syndrome, MNGIE, SANDO; MIRAS, MEMSA, AHS, MDS (hepato-cerebral form), IOSCA, and ADOA syndrome. In the majority of the cases polyneuropathy presents in a multiplex neuropathy distribution. Nerve conduction studies may reveal either axonal or demyelinated or mixed types of neuropathies. If a hereditary neuropathy is due to mitochondrial dysfunction, the management of these patients is at variance from non-mitochondrial hereditary neuropathies. Patients with mitochondrial hereditary neuropathy need to be carefully investigated for clinical or subclinical involvement of other organs or systems. Supportive treatment with co-factors, antioxidants, alternative energy sources, or lactate lowering agents can be tried. Involvement of other organs may require specific treatment. Mitochondrial neuropathies should be included in the differential diagnosis of hereditary neuropathies.

Abstract:Background: Primary mitochondrial diseases (PMDs) are the most common inborn errors of energy metabolism, with a combined prevalence of 1 in 4300. They can result from mutations in either nuclear DNA (nDNA) or mitochondrial DNA (mtDNA). These disorders are multisystemic and mainly affect high energy-demanding tissues, such as muscle and the central nervous system (CNS). Among many clinical features of CNS involvement, parkinsonism is one of the most common movement disorders in PMDs. Methods: This review provides a pragmatic educational overview of the most recent advances in the field of mitochondrial parkinsonism, from pathophysiology and genetic etiologies to phenotype and diagnosis. Results: mtDNA maintenance and mitochondrial dynamics alterations represent the principal mechanisms underlying mitochondrial parkinsonism. It can be present in isolation, alongside other movement disorders or, more commonly, as part of a multisystemic phenotype. Mutations in several nuclear-encoded genes (ie, POLG, TWNK, SPG7, and OPA1) and, more rarely, mtDNA mutations, are responsible for mitochondrial parkinsonism. Progressive external opthalmoplegia and optic atrophy may guide genetic etiology identification. Conclusion: A comprehensive deep-phenotyping approach is needed to reach a diagnosis of mitochondrial parkinsonism, which lacks distinctive clinical features and exemplifies the intricate genotype-phenotype interplay of PMDs.

Abstract:Objective To study the clinic, image, pathology and genetic change of the mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke (MELAS Syndrome). Methods There were 21 MELAS cases from Navy General Hospital and First Hospital of Peking University, 5 of which were family patients and 16 sporadic cases. The age of onset was between 2 and 45 years old. All of them had head imageological exam, 18 cases were performed muscle biopsy and 3 brain biopsy. 17 cases had the mtDNA A3243G point mutation screening. Results All the 21 cases had stroke history. Maternal inheritance diabetes mellitus and (or) deafness appeared in the maternal line members of the family patients. Cognition disturbance and allolalia were the chief clinical manifestations. The imageology showed that the impairment involved occipital lobe and temporal lobe most. 17 cases presented red-ragged-fibres (RRF) and parking lot -like ultrastructure in 3 cases. Brain biopsy showed layerage structure. There were 15 of them carried mtDNA A3243G mutation. Conclusions MELAS Syndrome usually involved central nerve system and skeletal muscle. Cognition functional disturbance, headach and allolalia were the chief clinical manifestations. RRF appears as the muscle pathological feature. Most of the patient carry mtDNA A3243G mutation.

Abstract:Mitochondrial disorders are a diverse group of diseases caused by mutations in genes encoded by either nuclear or mitochondrial DNA. In a group of patients with nuclear mitochondriopathies, the authors analysed the clinico-radiological and genotypic spectrum. The study included 25 patients with a genetic diagnosis of nuclear mitochondrial cytopathy who were seen over a 5 y period. There were 25 patients in the study cohort (Mean age of presentation- 14 mo). Biallelic mutations involving nuclear mitochondrial genes were identified in all 25 of them. In 13 and 9 patients, respectively, respiratory chain defects (complex I and complex IV) and mitochondrial DNA depletion syndromes were identified. Twelve novel variants were identified. Interestingly, NDUFV1 with a south Indian founder variant c.1156 C > T (p.Arg386Cys) was the commonest variant. Accurate phenotyping combined with next generation sequencing aids in the precise diagnosis of mitochondrial nuclear gene defects and provides the opportunity for appropriate counseling.

Adult-onset mitochondrial encephalopathy in association with the MT-ND3 T10158C mutation exhibits unique characteristics: A case report

Abstract:Background: Mitochondrial diseases are a heterogenous group of multisystemic disorders caused by genetic mutations affecting mitochondrial oxidation function. Brain involvement is commonly found in most cases but rarely as the unique clinical manifestation. Since the knowledge of its clinical manifestation combined with genetic testing is important for preventing misdiagnosis and delay in treatment, we report here how we diagnosed and managed a very unusual case of mitochondrial encephalopathy. Case summary: We report a 52-year-old woman with recurrent stroke-like episodes carrying the m.10158T>C mutation in the MT-ND3 gene, which is also responsible for fatal infant-onset Leigh syndrome. Despite the common mutation, the present case featured a distinct clinical and neuroimaging manifestation from Leigh syndrome. This patient presented with sudden onset of right-sided hemiparesis and hemilateral sensory disturbance accompanied by a left temporal cluster-like headache and later developed epilepsy during hospitalization, with no other signs suggestive of myopathy, lactate acidosis, or other systemic symptoms. Brain magnetic resonance imaging revealed variable lesions involving multiple cortical and subcortical regions. Furthermore, a negative genetic test obtained from peripheral blood delayed the diagnosis of mitochondrial disease, which was eventually established through second-generation DNA sequencing using biopsied muscle. Conclusion: Based on this report, we suggest that clinicians pursue proper genetic testing for patients when the clinical phenotype is suggestive of mitochondrial diseases.
Clinical Analysis of Mitochondrial Respiratory Chain Enzyme Deficiency in Children: Report of 7 Cases

Abstract:Objective To investigate the clinical features of the non-syndromic mitochondrial disease in children which were identified by mitochondrial respiratory chain enzyme complex activity assay. Methods From March 2010 to April 2011, seven children who were diagnosed as non-syndromic mitochondrial disease were included into this study. The enzyme activity of respiratory chain enzyme complexes (Ⅰ-Ⅳ) and ATP synthase (complex Ⅴ) in peripheral blood leukocytes of 7 children were determined. All the children whose routine screening of mitochondrial genes were negative. Children's clinical data were retrospectively analyzed. Results ①There were intelligence backwards after infection in 5 cases, combined seizures in 4 cases, paralysis in 6 cases; deafness, ataxia in 2 cases, combining anterior pituitary hypofunction/episodes of hypoglycemia in 1 case; myodynamia and hypomyotonia in 7 cases. The findings of brain MRI were abnormal in 6 cases, the findings of electroencephalogram (EEG) were abnormal in 5 cases and abnormal findings of electromyography (EMG) in 3 cases. Blood lactate increased slightly in 3 cases, the level of myocardial enzyme were abnormal in 2 cases. ②Conventional mitochondrial genes screening was negative. ③ 4 childen had isolated complex defects: complex Ⅱ deficiency (2 cases), complex deficiency of Ⅰ or Ⅳ (1 case, respectively); 3 cases were found to have combined deficiencies: 2 cases had combined deficiencies of complex Ⅰ and Ⅲ, 1 case had combined deficiencies of complex Ⅱ and V. Conclusions Mitochondrial respiratory chain enzyme deficiency in children may present non-specific neurological symptoms, the clinical features were extensive involvement of multi-system, and regular screening of mitochondrial gene was negative, the mitochondrial respiratory chain enzyme activity assay of peripheral leukocyte may help to identify the non-syndromic mitochondrial disease. Genetics of mitochondrial diseases: Current approaches for the molecular diagnosis

Abstract:Mitochondrial diseases are a genetically and phenotypically variable set of monogenic disorders. The main characteristic of mitochondrial diseases is a defective oxidative phosphorylation. Both nuclear and mitochondrial DNA encode the approximately 1500 mitochondrial proteins. Since identification of the first mitochondrial disease gene in 1988 a total of 425 genes have been associated with mitochondrial diseases. Mitochondrial dysfunctions can be caused both by pathogenic variants in the mitochondrial DNA or the nuclear DNA. Hence, besides maternal inheritance, mitochondrial diseases can follow all modes of Mendelian inheritance. The maternal inheritance and tissue specificity distinguish molecular diagnostics of mitochondrial disorders from other rare disorders. With the advances made in the next-generation sequencing technology, whole exome sequencing and even whole-genome sequencing are now the established methods of choice for molecular diagnostics of mitochondrial diseases. They reach a diagnostic rate of more than 50% in clinically suspected mitochondrial disease patients. Moreover, next-generation sequencing is delivering a constantly growing number of novel mitochondrial disease genes. This chapter reviews mitochondrial and nuclear causes of mitochondrial diseases, molecular diagnostic methodologies, and their current challenges and perspectives.
