Abstract:Objective To observe the effect of intrathecal(IT) administration of morphine and ketamine on the contents of spinal excitatory amino acids(EAAs) and expression of glutamate transporter subtype-1(GLT-1) mRNA in rat model of incision pain.Methods A total of 125 adult male Spragur-Dawley rats were randomly divided into five groups,and each group contained five time points,with each time point having 5 rats.The incision pain model was established according to Brennan's method.Group Ⅰwas sham operation group,Group Ⅱ received IT normal saline(25 μL),Group Ⅲ received IT morphine(10 μg),Group Ⅳ received IT 100 μg ketamine,and Group Ⅴ received IT 5 μg morphine plus 50 μg ketamine.In groups Ⅲ-Ⅴ the drugs were diluted in normal saline to a final volue of 20 μL;5 μL normal saline was used to flush the catherter after administration.Paw-withdrawal threshold(PWT) was measured to assess the pain behavior 1,2,6,12 and 24 h(T1-T5) after injection.Then lumbar spinal cord of rats was collected to measure the contents of EAAs(GLU,ASP)by amino acid analysator.The expression of GLT-1 mRNA was measured by real-time PCR at T5 point in the spinal cord.Results Compared with groupⅠ,the PWT of group Ⅱ-Ⅴ decreased at all time points after incision(all P<0.05,excluding T1 point in group Ⅲ),and the contents of spinal EAAs increased at T1 and T2 points in group Ⅲ and Ⅴ,and at all time points in groupⅡ and Ⅳ(all P<0.05).Compared to group Ⅱ,the PWT increased at all time points in group Ⅲ-Ⅴ(all P<0.05),the content of EAAs decreased at T1 and T2 in group Ⅳ,and at all points in group Ⅲ and Ⅴ(all P<0.05).Compared to group Ⅲ,the PWT in group Ⅴ increased at all point(all P<0.05,excluding T1 point of group Ⅴ),the EAAs contents increased at T3-T5 point in group Ⅳ and decreased at T1 and T2 in group Ⅴ(all P<0.05).Compared to group Ⅳ,the PWT of group Ⅴ increased at all points,and the EAAs contents decreased at all points in group Ⅴ(all P<0.05).Compared with groupⅠ,the expression of spinal GLT-1 mRNA decreased at T5(all P<0.05) in all groups.Compared with groupⅡ,the expression of spinal GLT-1 mRNA increased(all P<0.05) in group Ⅲ-Ⅴ,and compared with group Ⅲ and Ⅳ,the expression of spinal GLT-1 mRNA was significantly increased(all P<0.05) in groupⅤ.Conclusion Combined intrathecal injection of morphine and katamine can reduce acute pain in rat model of incision pain,which might be related to the decrease of excitatory amino acids and the change of GLT-1 mRNA expression in spinal cord.(Shanghai Med J,2010,33: 536-540)
