Clinical Features and Microsurgical Management of Saccular Middle Cerebral Artery Aneurysms

Abstract:ObjectiveTo investigate the clinical features and microsurgical management of the middle cerebral artery aneurysms (MCAAs). MethodsThe data of 37 cases of MCAAs were analyzed retrospectively, who were managed microsurgically from January to December 2005. 38 MCAAs were detected in 37 patients including 11 M1As, 25 MbifAs and 2 MdistAs. 32 MCAAs ruptured before the operation, of which 37.5% incurred the intracerebral hematoma. Based on Hunt-Hess Grade, 3 cases were grade 0, 3 cases grade Ⅰ, 9 cases grade Ⅱ, 15 cases grade Ⅲ, 5 cases grade IV, and 2 cases grade V on admission. All patients were managed microsurgically via pterional approach under the electrophysiological monitoring. After the lateral fissure was dissected under microscope antegradely or retrogressively along the MCA, the aneurysms were exposed and clipped. To make sure the clips were inserted on exactly proper place, a microvascular Doppler was applied to detect the blood stream in the MCA and related branches. ResultsAll 38 MCAAs were clipped with one or several clips. 25 patients recovered satisfactorily when discharged. Among 21 cases who were followed up for 6 months postoperatively, 16 cases scored GOS 4~5, 5 cases scored GOS 2~3, 1 case scored GOS 1. ConclusionWith microvascular Doppler and under electrophysiological monitoring, microsurgical management of MCAAs can gain satisfactory results, as long as adequate operative plan is made preoperatively based on the combined neuroradiological methods.
