Abstract:Objective To analyze the microsurgical techniques and outcomes of cervical spondylotic myelopathy. Methods The surgical experiences from 85 patients with cervical spondylotic myelopathy were analyzed retrospectively. All patients received anterior cervical microdiskectomy, 59 out of them underwent microsurgical decompression + auto iliac graft implant + titanium plate fixation, 2 underwent endoscopical decompression + auto iliac graft implanting + titanium plate fixation, and 24 underwent microsurgical decompression + Bryan cervical disk implanting. Results During a follow-up period of 23 months, ranged from 3 to 52 months, the spinal function was improved significantly in 82 patients (96.5%), and stable in 3 (3.5%). Twenty four patients with Bryan cervical disk implanting got a good cervical curvature on extension and flexion and good auto vertebral fusion by X-ray examination. Conclusions Anterior cervical microsurgical decompression is helpful to most patients with central cervical intervertebral disk herniation. Artificial cervical disk implanting could preserve spinal motor function and improve the therapeutic efficacy.
