Abstract:Objective To investigate the clinical features of cardiac involvement in Takayasu' s arteritis(TA).Methods This study is a retrospective analysis of all hospitalized patients at Peking Union Medical College Hospital between 1987 and 2010 and diagnosed with TA and associated cardiac diseases.Results 45(8.6%,45/524) patients with TA complicated by cardiac involvement were seen over 13 years.Cardiac diseases included hypertensive cardiomyopathy(16 patients),cardiomyopathy(12 patients),valvular disease(8 patients),coronary artery disease(3 patients),and pulmonary hypertension-associated right heart failure(2 patients).Four patients had more than one form of cardiac involvement.The mean age at onset of TA was 21.3 years,and the male to female ratio was 1:2.75.Cardiac symptoms developed in 75.6%(34/45) patients.Cardiac involvement was present at the onset of TA in 17.8% of patients(8/45),and the remainder(82.2%) had cardiac involvement within six years or so for the diagnosis of TA.There was no statistically significant difference between the groups in terms of gender,clinical type,cardiac symptoms,severity,activity,and prognosis,except onset age,which was significantly higher in cardiac valve group and pulmonary hypertension-associated right heart failure group than in other groups(P0.05).Conclusion Cardiac manifestations are not rare in TA.Clinicians should evaluate cardiac involvement even in asymptomatic patients.In the contrast patients,when encountering patients with cardiomyositis,heart valve disease,coronary artery disease,and heart failure,cardiac involvement associated with systemic diseases like TA should be considered in the differential diagnosis,especially in young patients with pulselessness or vascular bruits.
