Clinical manifestation of syphilitic posterior uveitis

Abstract:Objective To review the clinical manifestations of syphilitic posterior uveitis.Methods A retrospective study was conducted in 28 patients(37 eyes) diagnosed with syphilitic posterior uveitis.Results In our 28 cases of syphilitic posterior uveitis,21 were males and 7 were females.Ages ranged from 30 to 74 years with an average of 51.Uveitis was bilateral in 9 patients.Initial symptoms were nonspecific and included decreased visual acuity and floaters.Treponema pallidum particle agglutination(TPPA) and rapid plasma regain(RPR) were ordered and returned positive,which verified syphilis.Tests for human immunodeficiency virus(HIV) were negative.Ocular manifestations varied across these cases,and included optic neuritis in 5 eyes,retinal vasculitis in 7 eyes,neuroretinitis in 8 eyes,and chorioretinitis in 17 eyes.Visual acuity improved markedly and the fundus lesions subsided following proper treatment.RPR titer decreased or became undetectable in all patients during the follow-up.Conclusion Syphilitic posterior uveitis often present with atypical manifestations.All suspected patients with posterior uveitis should be examined with TPPA and RPR.Early diagnosis and prompt treatment are important for the prognosis.
