Clinical comparison of posterior lumbar interbody fusion with or without posterolateral lumbar fusion in treatment of degenerative lumbar diseases

Abstract:Objective To study the clinical outcomes of posterior lumbar interbody fusion(PLIF) with and without posterolateral lumbar fusion(PLF) in treatment of degenerative lumbar disease.Methods A total of 78 patients who underwent lumbar fusion surgery because of degenerative lumbar disease from March 2007 to February 2008 were studied.All cases were blindly randomized into group PLIF+PLF and group PLIF.The follow-up period was 24 weeks.Oswestry disability indix,lumbar lordosis angle,segmental lordosis angle,disc space height,rate of spondylolisthesis and union rates were observed.Blood loss in the surgery,drainage within 48 hours after operation,incision pain and postoperative complications were noted in addition.Comparison of the follow-up results between the two groups was made.Results In 78 cases of follow-up,there were no significant differences in statistics between the two groups in the improvements of clinical symptoms,lumbar lordosis angle,segmental lordosis angle,disc space height,rates of spondylolisthesis,fusion rates and blood loss.PLIF with PLF led to more drainage within 48 hours after operation and more severe incision pain at the 3rd day after operation.Conclusion There is no significant difference in fusion rate and short-term clinical effects between PLIF with and without PLF.Combination of PLIF and PLF increases trauma of operation.
