The Clinical Significance of the Expression of E - Cd and EGFR Proteins in Supraglottic Carcinoma

Abstract:Objective:To study the expression and clinical significance of E-cadherin (E-cd) and epidermal growth factor receptor (EGFR) proteins in supraglottic carcinoma. Methods: Using LSAB-immunohistochemical technique, the expression of E-cd and EGFR proteins were investigated in 54 cases of human supraglottic squamous cell carcinoma. Results: The rate of expression was 44.4% in the E-cd(E-cd~+)and 53.7% in the EGFR(EGFR~+),they significantly correlated with TNM stages, invasive extent, lymph node metastasis and degree of differentiation(P0.01 or P0.05). The 5-year cumulative survival rate was 94.29% in the E-cd~+ cases, 61.53% in the E-cd~-negative cases(E-cd~-),94.74% in the EGFR~-negative cases(EGFR~-), 59.06% in the EGFR~+ cases, with significant difference(P0.01). The prognosis was significantly poor in E-cd~-EGFR~+ subjects: the 5-year cumulative survival rate was 100.00% in the E-cd~+EGFR~- cases, while it was 55.03% in the E-cd~-EGFR~+ cases(P0.01). Four of five occult lymph node metastasis(cN~-pN~+)were E-cd~-EGFR~+. Conclusion: These findings suggest that the immunohistochemical determination of expression of E-cd and EGFR proteins in supraglottic carcinoma can be used as the predictors of tumor invasion, lymphatic metastasis and prognoses. If combining them, a more precise prognosis can be obtained. Such a combined technique may be very useful as an indicator for grading the biological malignancy of laryngeal cancer.
