Contrast-enhanced Ultrasonography with Different Enhanced Intensity in the Diagnosis of Kidney Neoplasm

Abstract:Purpose To evaluate the contrast enhanced ultrasonography in the diagnosis of kidney neoplasm, and to explore its application in pathologic classification. Material and Methods 112 patients with kidney neoplasms underwent pre-operative contrast enhanced ultrasound examination, and divided into hypoenhancing group (n=26) and iso/hyper-enhancing group (n=86) based on contrast enhancement pattern. The imaging features were compared to pathology. Results There was a statistical difference in pathologic types between hypoenhancing group and iso/hyperenhancing group. In 26 cases with hypoenhancing kidney tumors, 17 were low grade or benign, 8 were clear cell tumors and 1 was metastasis. In 86 cases with iso- or hyperenhancing tumors, 75 were clear cell tumors, 2 were polycystic kidney cancer, 1 metastasis, 1 low grade angiomyolipoma and 7 benign tumors. In differentiation of benign and malignant kidney tumors, the sensitivity, specificity, positive predictive value and negative predictive value of hypoenhancement were 62.96%, 89.41%, 65.38% and 88.37%, respectively. In diagnosis of clear cell carcinoma, the sensitivity, specificity, positive predictive value and negative predictive value of iso/hyperenhancement were 90.36%, 62.07%, 87.21% and 69.23%, respectively. Conclusion Different ultrasonographic enhancement patterns suggest different renal pathologies. Contrast enhanced ultrasound can help with kidney tumor diagnosis and predict prognosis.
