Abstract:OBJECTIVE:To compare the differences in clinical therapeutic effect and safety between amphotericin B and its liposome form in treating invasive fungal infection (IFI) in hematological disorder with neutrocytopenia.METHODS:Of 111 patients with IFI, 82 were treated with amphotericin B and 29 with amphotericin B liposome. The mean cumulative dose of amphotericin B was 617 (60-1895) mg and the mean course was 18 (7-60) d, and those for amphotericin B liposome was 925 (140-3420) mg and 13 (7-50) d, respectively.RESULTS:The total effective rates of amphotericin B and its liposome groups were 69% and 58%, respectively (P>0.05). The adverse effect rates of chill and fever in amphotericin B and its liposome groups were 21% and 10% (P>0.05), hypopotassemia 34% and 14% (P=0.03), hepatic impairment 22% and 17% (P>0.05), and renal impairment 9% and 3%, respectively (P>0.05).CONCLUSION:The therapeutic effect for IFI of amphotericin B and its liposome was similar. The severe adverse reaction of amphotericin B liposome was slightly lower than that of amphotericin B.
In Vitro Interactions of Micafungin with Amphotericin B, Itraconazole or Fluconazole Against the Pathogenic Phase of Penicillium Marneffei.

Abstract:Penicillium marneffei infection is an important disease among human immunodeficiency virus patients in south-east Asia, including southern China. However, therapeutic strategies are limited. Combination regimens with synergistic drugs could provide additional options for treating penicilliosis. We evaluated the in vitro efficacy of combining micafungin with amphotericin B, itraconazole or fluconazole against the pathogenic yeast form of P. marneffei.Twenty isolates of P. marneffei were assayed. Drug interactions were assessed with the chequerboard technique using the CLSI (formerly the NCCLS) microdilution method (M27-A2) with minor modifications. The fractional inhibitory concentration index (FICI) was used to classify drug interactions. Results were interpreted as follows: synergy, FICI <= 0.5; no interaction, FICI > 0.5 and <= 4.0; or antagonism, FICI > 4.0.The in vitro interactions of micafungin combined with itraconazole showed the highest percentage of synergic interaction (65%); for the micafungin/amphotericin B combination, 50% of the isolates had synergic interaction. Micafungin significantly enhanced the antifungal activity of amphotericin B and itraconazole against P. marneffei. Micafungin, however, did not enhance the activity of fluconazole and no synergism was observed with this combination. Antagonism was not detected for any of the antifungal combinations assayed.The results of this study suggest that micafungin enhances the efficacy of itraconazole or amphotericin B in vitro and indicate that an echinocandin, such as micafungin, might have a potential role in combination therapy among patients infected with P. marneffei.
Short-Course Rather Than Low-Dose Amphotericin B May Exert Potential Influence on Mortality in Cryptococcal Meningitis Patients Treated with Amphotericin B Plus Flucytosine Alone or in Combination with Fluconazole

Abstract:Background: The influence of Amphotericin B (AmB) dose and the addition of fluconazole (Flu) on the AmB + 5-flucytosine (5FC) regimen for cryptococcal meningitis (CM) treatment remain debatable. Methods: A retrospective study was conducted to compare 44 CM patients treated with AmB + 5FC and 78 CM patients treated with AmB C 5FC C Flu using the propensity score matching method. The effects of AmB dosage, AmB course and Flu addition on the cerebrospinal fluid (CSF) chemical profile recovery, adverse effects, and 90-day mortality were compared between the groups. Results: No differences in adverse effects, the rate of the 14-day CSF chemical profile recovery and 90-day cumulative survival rate (91.2% vs. 87.5%, P = 0.637) were observed between the AmB + 5FC group and the AmB + 5FC + Flu group. However, the incidence rates of hypokalemia (33.9%) and creatinine elevation (7.1%) in patients treated with an AmB dosage of 0.4-0.5 mg/kg/d were less than those (53.0 and 22.7%, respectively) treated with an AmB dosage of 0.6-0.7 mg/kg/d (P = 0.034 and P = 0.018, respectively). The 90-day cumulative survival rate was 70.1% for patients treated with AmB for < 14 days and 96.4% for patients treated with AmB for >= 14 days (log-rank P < 0.001). Multivariate Cox proportional hazards models suggested the hazard ratio was 26.8 (95% CI: 3.9-183.2) for patients treated with AmB < 14 days than those treated with AmB >= 14 days (P = 0.001). Conclusion: Treatment with AmB less than 14 days was associated with a higher 90-day mortality in CM patients. A relative lower dosage but prolonged course of AmB in the + 5FC +/- Flu regimen led to favorable trends of fewer adverse effects and comparable clinical efficacy.

Abstract:Photodynamic therapy (PDT) is effective in the destruction of fungi. In order to evaluate the efficacy and safety of PDT for superficial mycoses, we performed an evidence-based review of published literature. Database of MEDLINE, EMBASE, and Cochrane Library was searched until March 2010. English-language articles evaluating the efficacy and safety of PDT for superficial mycoses were included. No randomized clinical trials were found. Seven reports described the antifungal effect of PDT against 63 superficial mycoses patients. Eight of 10 (80%) tinea cruris patients and 6 of 10 (60%) tinea pedis were led to mycological cure after 1–3 treatments. Unfortunately, only 4 (40%) tinea cruris patients and 3 (30%) tinea pedis had a persist healing at the 8-week follow-up. Six of the 9 (66.7%) foot-interdigital mycoses patients recovered clinically and microbiologically after 1 or 4 treatments. Only 2 patients (22.2%) had a persist healing at the 8-week follow-up. Eleven of 30 (36.6%) onychomycosis patients were cure for 18 months after treatment, and 3 onychomycosis patients were all cure in other 2 reports. The therapeutic effect of PDT for one pityriasis versicolor patients was well. Overall tolerability of PDT was good. Therefore, it is unclear what PDT’s place for superficial mycoses will be. Further clinical trials are needed to evaluate the efficacy of PDT to treat superficial mycoses. It is also important to optimize treatment protocols in order to cope with recurrence.
The use of amphotericin B in hematologic malignancy children with invasive fungal infections

Abstract:Objective To evaluate the clinical feathers and safety of Amphotericin B by analyzing the efficacy of Amphotericin B in the treatment of invasive fungal infections in children with hematologic malignancy.Methods Retrospetive review the clinical feathers and anti-fungi therapy of 19 children with fungal infections,to analyze the clinical findings and laboratory results by given Amphotericin B.Results In the 19 cases,four of them were definitely diagnosed to invasive fungal infections,ten of them were clinically diagnosed and five were suspectively diagnosed.Eight of them had fungal infections in respiratory system,one in subcutaneous node, one in conchae nasals,one in liver and spleen,two had septicemia,and one had abscess in pharyngeal wall.Three cases were fully recovered and nine cases got much better after the treatment of Amphotericin B.The overall clinical efficacy rate of Amphotericin B was 66.67%. The main adverse reaction was hepatic impairment and hypokalemia.Conclusions It may be well-tolerated in most children with hematologic malignancy if traditional Amphotericin B is administered properly,and the adverse reation are managed appropriately.
[Amphotericin B for Treatment of Fungal Infections in 40 Patients with Malignant Hematologic Diseases].

Abstract:OBJECTIVE:To observe the therapeutic effect and side effects of amphotericin B for fungal infections in patients with malignant hematologic diseases.METHODS:40 patients (male 27, female 13; average age 35.5 years) with malignant hematologic diseases were given amphotericin B, 5-50 mg/d per day for 5-85 days (average time 21 days).RESULTS:The clinical efficacy rate of amphotericin B was 52.5%, and the fungal elimination rate was 56.2%. Among the side effects, rigor and fever were present in 2.5% of the patients. Hypokalaemia was found in 12.5%, hepatotoxicity in 15.0% and nephrotoxicity in 15.0%.CONCLUSION:As amphotericin B has a broad anti-fungal spectrum and relatively good efficacy, it is still a high-efficiency drug in treatment of systematical fungal infections. However, the use of drug is limited because of its many side effects. Our study indicates that if it is used properly and hepatic and renal function tests are carried out regularly, amphotericin B is a relatively safe and effective drug.

Abstract:Amphotericin B (AmB) has long stood as a cornerstone in the treatment of invasive fungal infections (IFIs), especially among immunocompromised patients. However, the landscape of antifungal therapy is evolving. New antifungal agents, boasting novel mechanisms of action and better safety profiles, are entering the scene, presenting alternatives to AmB’s traditional dominance. This shift, prompted by an increase in the incidence of IFIs, the growing demographic of immunocompromised individuals, and changing patterns of fungal resistance, underscores the continuous need for effective treatments. Despite these challenges, AmB’s broad efficacy and low resistance rates maintain its essential status in antifungal therapy. Innovations in AmB formulations, such as lipid complexes and liposomal delivery systems, have significantly mitigated its notorious nephrotoxicity and infusion-related reactions, thereby enhancing its clinical utility. Moreover, AmB’s efficacy in treating severe and rare fungal infections and its pivotal role as prophylaxis in high-risk settings highlight its value and ongoing relevance. This review examines AmB’s standing amidst the ever-changing antifungal landscape, focusing on its enduring significance in current clinical practice and exploring its potential future therapeutic adaptations.

Abstract:Objective To observe the therapeutic efficacy and adverse reactions of domestic amphotericin B in the treatment of invasive fungal infection (IFI) in hematological disorder patients. Methods A total of 45 cases of immunodepressive IFI patients receive domestic amphotericin B treatment from December 2004 to December 2008 in our hospital were retrospectively analyzed. Result All the cases patients received amphotericin B therapy for more than 2 weeks,in which the longest was 14 weeks,with 6 weeks as median. The total effective rate was 86. 7% . Adverse reaction included hypokalemia,fever,aminotransferase,kidney failure,and gastrointestinal reactions. Conclusions Domestic amphotericin B is affirmed in the preemptive therapy for IFI in hematological patients with tolerable adverse reactions.

Abstract:Background Amphotericin B (0.7 mg/kg) with flucytosine is the standard treatment for cryptococcal meningitis. However, the long treatment course can induce adverse reactions in patients; therefore, reducing the dose may decrease such reactions. We performed a retrospective analysis of treatment effects and adverse reactions when amphotericin B (0.4 mg/kg or 0.7 mg/kg per day) and flucytosine were used together to treat HIV-negative patients with cryptococcal meningitis. Methods Retrospective analysis was conducted on inpatients at the First Affiliated Hospital, College of Medicine, Zhejiang University (January 2005 to December 2009). Low- or high-dose amphotericin B (0.4 or 0.7 mg/kg per day, respectively) plus flucytosine was used. The negative conversion rate of Cryptococcus in the cerebrospinal fluid (CSF), patient mortality, and the incidence of side effects for the two groups (low- vs. high-dose) were compared immediately after treatment and 2 and 10 weeks later. Data were analyzed by the Student's t test, chi-square tests using SPSS 12.0 statistical software. Results Two weeks post-treatment, Cryptococcus negative CSF rates were 78% (18/23) in the low-dose group and 87% (13/15) in the high-dose group (P=0.28). Ten weeks post-treatment, both groups were negative. The mortality rate was 8% (2/25) in the low-dose group and 17% (3/18) in the high-dose group (P=0.25). There was a statistically significant difference in the incidence of adverse events between the groups, 48% (12/25) and 78% (14/18) in the low- and high-dose groups, respectively (P=0.04). Adverse events that required a change in treatment program in the low-dose group were 12% (3/25) compared to 39% (7/18) in the high-dose group (P=0.04). Abstract:Leishmaniasis is zoonotic disease caused byLeishmania with different clinical manifestations. It can be characterized into three clinical types: visceral leishmaniasis, mucocutaneous leishmaniasis and cutaneous leishmaniasis. Traditionally, antimony is the ifrst-line treatment for leishmaniasis, but its application is compromised due to the high adverse effects and the emergence of drug resistantLeishmania isolates. Clinical researches have shown that amphotericin B can be used in patients infected by antimony resistant isolates, and treatment of azole and propylene amine can also be effective and safe. Through the analysis of the components inLeishmania membrane, this article further illustrates the mechanism, usage and efifcacy of antifungal agents in the treatment of leishmaniasis.

Abstract:Invasive fungal infections (IFIs) represent a growing public concern for clinicians to manage in many medical settings, with substantial associated morbidities and mortalities. Among many current therapeutic options for the treatment of IFIs, amphotericin B (AmB) is the most frequently used drug. AmB is considered as a first-line drug in the clinic that has strong antifungal activity and less resistance. In this review, we summarized the most promising research efforts on nanocarriers for AmB delivery and highlighted their efficacy and safety for treating IFIs. We have also discussed the mechanism of actions of AmB, rationale for treating IFIs, and recent advances in formulating AmB for clinical use. Finally, this review discusses some practical considerations and provides recommendations for future studies in applying AmB for combating IFIs.

Intraocular Penetration of Compound Amphotericin B Eye Drops after Topical Administration in Rabbit Eyes

Abstract:AIM To investigate the intraocular penetration of compound amphotericin B eye drops. METHODS The tear,cornea and aqueous humor of the experimental rabbits were collected at 30 min and 60 min after topical application of 50μL compound amphotericin B eye drops,and then the concentrations of amphotericin B and rifampicin were detected by HPLC.RESULTS The concentration of amphotericin B in cornea and tears at 30 min and 60 min after administration of 50μL compound amphotericin B eye drops were (1.9±s 0.4)μg·g-1 vs.(1.31±0.25)μg·g-1 and(430±70)μg·g-1 vs.(191±23)μg·g-1,respectively.The concentrations of rifampicin in cornea and tears were(9.9±1.0)μg·g-1 vs.(5.8±0.9)μg·g-1 and(96±10)μg· g-1 vs.(5.2±0.8)μg·g-1,respectively,with(0.42±0.11)mg·L-1 vs.(0.73±0.17)mg·L-1 in aqueous humor. CONCLUSION Rifampicin has the good intraocular penetration,and the concentrations of amphotericin B in tear and cornea are higher than minimum inhibitory concentration for treatingmost fungi,so compound amphotericin B eye drops can be used to treat fungal keratitis.
Hydrophobic binary mixtures containing amphotericin B as lipophilic solutions for the treatment of cutaneous leishmaniasis

Abstract:Cutaneous leishmaniasis, caused by Leishmania parasites, requires treatments with fewer side effects than those currently available. The development of a topical solution based on amphotericin B (AmB) was pursued. The considerable interest in deep eutectic solvents (DESs) and their remarkable advantages inspired the search for a suitable hydrophobic excipient. Various mixtures based on commonly used hydrogen bond donors (HBDs) and acceptors (HBAs) for DES preparations were explored. Initial physical and in-vitro screenings showed the potential of quaternary phosphonium salt-based mixtures. Through thermal analysis, it was determined that most of these mixtures did not exhibit eutectic behavior. X-ray scattering studies revealed a sponge-like nanoscale structure. The most promising formulation, based on a combination of trihexyl(tetradecyl)phosphonium chloride and 1-oleoyl-rac-glycerol, showed no deleterious effects through histological evaluation. AmB was fully solubilized at concentrations between 0.5 and 0.8 mg·mL-1, depending on the formulation. The monomeric state of AmB was observed by circular dichroism. In-vitro irritation tests demonstrated acceptable viability for AmB-based formulations up to 0.5 mg·mL-1. Additionally, an ex-vivo penetration study on pig ear skin revealed no transcutaneous passage, confirming AmB retention in healthy, unaffected skin.
Antifungal Effects of Domestic Butenafine and Its Topical Treatment on Experimental Dermatophytosis in Guinea Pigs

Abstract:Aim To investigate antifungal effects of domestic butenafine and its topical treatment on experimental dermatophytosis in guinea pigs.Methods The in vitro fungistatic concentrations were determined by a serial dilution method using sabrouraud dextrose agar and the MIC was estimated as the lowest concentration that completely inhibited the growth of dermatophytes.1% betenafine cream was applied on the infected site of guinea pig dermatophytisis caused by Trichophyton mentagrophytes in comparison with those of clotrimazole,naftifine,and bifonazole.Results MICs of butenafine for dermatophytes were in the range of 0.002 5～0.32μg·ml -1 and its antidermatophyte activity was greater than those of the reference drugs,showing that butenafine exhibits potent fungistatic activity.Topical butenafine cream showed excellent efficacy against dermatophytosis and the effect was superior to that of clotrimazole,there was a significant difference between two groups(P0.05).The effect of butenafine was higher than those of naftine and bifonazole,but there was no significant difference among them(P0.05).The incidence of relapse rate of butenafine cream after topical treatment was lower than those of three reference drugs.Conclusion Butenafine cream has potential for treatment of superficial fungal infections in humans.
Comparison of the Efficacies of Topical Liposomal Amphotericin B and Topical Clotrimazole in the Treatment of Pityriasis Versicolor

Abstract:Introduction: Pityriasis versicolor (PV) often exhibits recurrence even despite treatment, and a standardized therapy with a complete cure rate remains elusive. Given the fungicidal property of amphotericin B, its potential for PV treatment warrants investigation. Objectives: This study aimed to compare the efficacy of topical liposomal amphotericin B and clotrimazole cream in treating PV. Methods: A randomized controlled trial was conducted with 44 PV patients, aged 15 to 70 years, who were equally assigned to either the amphotericin or clotrimazole group. The trial was registered at the Iranian Registry of Clinical Trials on March 7 th , 2022. PV diagnosis was confirmed in all patients through positive microscopic results. The amphotericin group received topical gel containing liposomal amphotericin B 0.4%, while the other group received topical cream containing clotrimazole 1%. Both treatments were applied twice daily to the affected skin lesions for 14 days. The primary outcomes assessed were mycologic cure, clinical cure, and complete cure at day 14 post-treatment. Results: Both drugs demonstrated successful treatment outcomes, with comparable rates of mycologic cure (77.3%), clinical cure (63.6%), and complete combined cure (63.6%). There was no significant difference between the groups in terms of the degree of cure (P=0.75). Adjustments for age, sex, lesion extent, and lesion site yielded insignificant risk ratio and risk difference estimates for the complete cure rate (P>0.05). Conclusions: The topical application of liposomal amphotericin B 0.4% is equally effective as topical clotrimazole 1% in treating pityriasis versicolor. These findings suggest that liposomal amphotericin B could be considered as a viable alternative treatment for PV. Treatment of Superficial Mycosis with Compound Bifonazole Solution: A Double-blind Controlled Clinical Trial

Abstract:Objective To investigate the effects of compound bifonazole solution for the treatment of superficial mycosis. Methods The study groups were treated with compound bifonazole solution and the control group with clotrimazole solution in a double-blind controlled clinical trial.The solutions were applied to skin lesions once a day.The course of treatment was two weeks for tinea corporis and tinea cruris and four weeks for tinea manus and tinea pedis.The patients were followed up weekly for two weeks after cessation of treatment and evaluated with regard to erythema,papule,blister,scale,keratinization and pruritus.Mycologic examinations were performed before,during and right after treatment and two weeks after treatment. Results A total of 434 patients participated into the study.The clinical cure rates of study group were 82.25% in tinea corporis and tinea cruris,and68.75%tinea manus and tinea pedis,with a total response rates of 95.85% and 92.5% in tinea corporis and tinea cruris,and 92.5% in tinea manus and tinea pedis,respectively.The clinical cure rates of control group were 58.6% in tinea corporis and tinea cruris,and 44.7% in tinea manus and tinea pedis,with a total response rates of 83.0% and 87.2% in tinea corporis and tinea cruris,and in tinea manus and tinea pedis,respectively.The MICs to350clinical isolates of pathogenic fungi were 1.6~2.5mg/L for compound bifonazole solution,and 3.125~25mg/L for clotrimazole solution. Conclusion Compound bifonazole solution is a high-effective,broad-spectrum antifungal agent.It is keratolytic,well permeable and safe for relatively long term application. Abstract:Objective: To investigate the efficacy and safety of liposomal amphotericin B (L-AmB) for the salvage treatment of invasive fungal disease (IFD) in patients with hematological diseases. Methods: Data were retrospectively collected from 80 patients with hematological issues treated with L-AmB between June 2023 and December 2023 after failure of previous antifungal therapy. Baseline patient information, clinical efficacy, and factors affecting the efficacy of L-AmB were analyzed by logistic regression. Moreover, adverse effects associated with L-AmB were evaluated. Results: Among the 80 patients, 9 (11.2%) had proven IFD, 43 (53.8%) had probable IFD, and 28 (35.0%) had possible IFD. The efficacy rate of L-AmB salvage therapy for IFD was 77.5%, with a median daily dose of 3 (range: 1-5) mg·kg(-1)·d(-1) and a median dosing course of 14 (range: 8-25) days. Multivariate logistic regression analysis showed that the disease remission status (OR=4.337, 95% CI 1.167-16.122, P=0.029) and duration of medication (OR=1.127, 95% CI 1.029-1.234, P=0.010) were independent factors affecting the efficacy of L-AmB. The incidence of infusion reactions associated with L-AmB, including fever and chills, was 5.0%. The incidence of hypokalemia was 28.8% (predominantly grades 1-2), and the incidence of nephrotoxicity was 11.3% (predominantly grades 1-2) . Conclusion: L-AmB is safe and effective in the treatment of patients with IFD who are intolerant to or who have experienced no effect of previous antifungal therapy, with a low rate of adverse reactions.
