Abstract:Purpose: We studied the prevalence of prostatitis-like symptoms and identified their associated risk factors in a population based Chinese sample. Materials and methods: A volunteer group of 15,000 eligible men residing in Beijing, Anhui, Xi'an, Guangzhou and Gansu cities or provinces were invited randomly to take part in the survey to complete a questionnaire that elicited information regarding sociodemographics, Eysenck personality questionnaire, current stress and health ratings, lifestyle, medical history, expressed prostatic secretion evaluation, score of the National Institutes of Health Chronic Prostatitis Symptom Index and International Index of Erectile Function-5. Results: Information on 12,743 (84.95%) men was collected. Of these men 1,071 (8.4%) reported prostatitis-like symptoms (mean National Institutes of Health Chronic Prostatitis Symptom Index pain score 7.55 +/- 3.22). The percent of chronic prostatitis was 4.5% (571) among the symptoms group according to past urological history and expressed prostatic secretion evaluation. Subjects with prostatitis-like symptoms (mean age 34.56 +/- 13.48 years) had higher mean pain and urinary symptoms scores (7.53 +/- 3.22 and 2.84 +/- 2.72, respectively) compared with subjects without prostatitis-like symptoms (1.18 +/- 2.32 and 0.72 +/- 1.66 for pain and urinary symptoms scores, respectively, mean age 30.7 +/- 10.17) (pain and symptoms scores, p <0.05). The quality of life score was 6.03 +/- 2.88 and 3.83 +/- 2.55 in groups with symptoms or nonsymptoms, respectively (p <0.05). Conclusions: Prostatitis-like symptoms are a multifactorial problem affecting men of all ages (15 to 60 years) and demographics, and the prevalence is high in China. The syndrome is closely related to alcohol consumption, cigarette smoking, frequent intercourse, as well as fatigue, pressure and too little sleep. These findings suggest that risk factors for this condition are largely modifiable and highlight potential targets for future prevention.

Abstract:The aim of this study was to describe the prevalence of and risk factors for prostatitis-like symptoms and its relation to erectile dysfunction (ED) among southern Chinese men. Data were collected from 2790 men attending the Fangchenggang Area Male Healthy and Examination Survey from September 2009 to December 2009. The prostatitis-like symptoms were assessed by the NIH Chronic Prostatitis Symptom Index and ED was assessed using the 5-item International Index of Erectile Function. Lifestyle and demographic characteristics were obtained through a questionnaire. Prevalence of prostatitis-like symptoms was 12.4% among 2790 Chinese men aged 20-84 years. In smokers who smoked ≥20 cigarettes per day (age-adjusted OR = 1.29; 95% CI = 1.00-1.66; p = 0.04), physical inactivity (age-adjusted OR = 1.31; 95% CI = 1.03-1.66; p = 0.02) was a significant risk factor for prostatitis-like symptoms. Alcohol consumption (daily drinking) also was a risk factor for prostatitis-like symptoms, although the differences were not statistically significant (age-adjusted OR = 1.36; 95% CI = 0.96-1.92; p = 0.07). Those with diabetes may also be at higher risk for prostatitis-like symptoms (age-adjusted OR = 1.37; 95% CI = 0.85-2.21; p = 0.19). In addition, men with ED were more likely to have had prostatitis-like symptoms (age-adjusted OR = 1.86; 95% CI = 0.47-2.36; p < 0.0001), and the ORs increased with increasing severity of ED status (mild ED, mild to moderate ED, and moderate to severe ED were 1.57, 2.62, and 3.24, respectively. Test for trend, p = 0.0001). Our results show that prostatitis-like symptoms are prevalent in Southern China affecting men of all ages. Smoking, drinking, lack of physical activity, and elevated plasma glucose level were associated with an increased risk of prostatitis-like symptoms. In addition, our results reveal that ED accounted for a large proportion (61.5%) among men with prostatitis-like symptoms; we also confirm the magnitude of ED associated with prostatitis-like symptoms. Thus, interventions to evaluate and improve ED might help ameliorate prostatitis-like symptoms and vice versa.
The Prevalence of Erectile Dysfunction and Its Relation to Chronic Prostatitis in Chinese Men.

Abstract:This study was performed to evaluate the prevalence of erectile dysfunction (ED) and to explore its correlation to chronic prostatitis in China. A cross-sectional investigation from a large cohort study of Chinese men was used in this survey. A questionnaire consisting of general information regarding socio-demographics, chronic disease history, sexual function, the National Institutes of Health-Chronic Prostatitis Symptom Index, and the International Index of Erectile Function-5 (IIEF-5) was administered to 15 000 Chinese men aged from 15 to 60. The prevalence of ED was determined from the patient's self-evaluation and IIEF-5 score. The eligible individual both was married and had intercourse experience. In total, there were 12 743 respondents, giving a response rate of 84.95%. Among 7372 eligible men, ED prevalence as assessed by self-report and IIEF-5 score was 12.0% and 17.1%, respectively. Among 771 men with prostatitis-like symptoms, ED prevalence as assessed by self-report and IIEF-5 score was 39.3% and 30.1%, respectively. Among 370 men suffering from chronic prostatitis, ED prevalence as assessed by self-report and IIEF-5 score was 40.5% and 35.1%, respectively. The prevalence of self-reported and IIEF-5 score-assessed ED had high correlation with increasing age among all eligible men, men with prostatitis-like symptoms, and men with chronic prostatitis (P(t) < .05, P(s) < .05, P(cp) < .05). ED prevalence as assessed by both self-report and IIEF-5 score was higher in men with prostatitis-like symptoms and with chronic prostatitis than in the general group (P(s) < .05, P(cp) < .05). The prevalence of ED was higher in the prostatitis population than in the general population with either self-reported or IIEF-5 score assessment. The prevalence was higher with self-reported than with IIEF-5 assessment in men with prostatitis. Estimates of ED prevalence among men with prostatitis should not rely on self-reporting alone in that this is likely to overestimate the true prevalence.
Treatment of chronic prostatitis in Chinese men.

Abstract:The aim of this study is to assess the status of treatment of chronic prostatitis (CP) in Chinese men. A population-based cross-sectional survey was performed, in which 15 000 men aged between 15 and 60 years were randomly selected to receive a questionnaire designed to assess National Institutes of Health Chronic Prostatitis Symptoms Index (NIH-CPSI) status, therapeutic efficacy and 28 other items. A total of 12 743 men (84.95%) completed the questionnaire, of whom 1 071 (8.4%) were identified as having prostatitis-like symptoms and 517 (4.5%) were diagnosed with CP according to NIH-CPSI criteria and prostatitis-like symptomatology. Of the CP patients, 372 (65%) underwent long-term routine treatment 12 times per year. Additionally, 217 (72.8%) patients received antibiotic therapy and 215 (79.3%) men showed therapeutic effects. The treatment cost USD 1 151 (8 059 yuan) per person per year on average. Most CP patients received routine treatment, in most cases with antibiotics. Treatment was costly and most CP patients were not satisfied with its effectiveness. Antibacterial treatment might have been effective primarily in patients with bacterial disease.
A Preliminary Evaluation of the Psychometric Profiles in Chinese Men with Chronic Prostatitis/chronic Pelvic Pain Syndrome

Abstract:BACKGROUND:As one of the most commonly diagnosed diseases, chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is characterized by a variety of complex symptoms. Anxiety and depression are two of the most prevalent clinical manifestations of patients with CP/CPPS, and have adverse effects on the health of the subjects and prognosis of comorbidities. Such psychological disorders, however, have not been deeply and thoroughly studied in China. The aim of this study was to investigate the prevalence and severity of psychological disorders in Chinese adults with CP/CPPS.METHODS:From April 2008 to June 2009, 80 patients and 40 age-matched healthy men participating in a voluntary health examination were recruited. The majority of the subjects completed the questionnaires on the National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) as well as the hospital anxiety and depression scale (HADS).RESULTS:Of all the participants, 77 (96.3%) patients and 37 (92.5%) healthy controls completed the questionnaires. The average NIH-CPSI total score was 21.0 ± 9.5 for the patients and 2.2 ± 1.5 for the controls (P = 0.03). Of the 77 patients with CP/CPPS, 48 (62.3%), 5 (6.5%), and 1 (1.2%) had anxiety symptoms, depression symptoms, or both anxiety and depression symptoms, respectively. For the controls, the average HADS anxiety and depression scores in patients were 14.5 ± 6.8 and 5.2 ± 4.5, which were both significantly higher than in controls. Moreover, the prevalence and the symptom scores of both the HADS anxiety and depression were higher for the younger age group (< 35 years) than for the older age group (< 35 years).CONCLUSIONS:This preliminary study revealed that male patients with CP/CPPS had a higher prevalence of psychological disorders than in the control subjects. Moreover, the differences of the prevalence and severity of the psychological symptoms between the two different age groups may imply that psychological disorders related to CP/CPPS may be relieved with increasing age. The present study indicated that psychological evaluation is important in men with CP/CPPS, especially in younger men.

Sexual dysfunctions and psychological disorders associated with type IIIa chronic prostatitis: a clinical survey in China

Abstract:Introduction: Chronic prostatitis (CP) is a frequent prostate-related complaint, impacts negatively on quality of life and is mostly of unclear etiology. Increasing attention has been paid to the prevalence of sexual dysfunctions in CP patients; however, the impact of specific types of CP and the correlation of sexual dysfunctions with psychological disorders associated with CP are not well understood. Type IIIa CP is characterized by chronic pelvic pain, urination symptoms and white blood cells in expressed prostatic secretion, but free of bacterial infection. Methods: A population of 600 type IIIa CP patients were randomly selected and 40 normal man were included as the control group. Queries were conducted by urologists. The National Institute of Health Chronic Prostatitis Symptom Index (NIH-CPSI), the International Index of Erectile Function (IIEF-5) and the Symptom Checklist 90-R were used to evaluate the symptoms and severity of prostatitis, erectile dysfunctions and psychological problems, respectively. Scores of ejaculatory pain and premature ejaculation were also collected. Results: Our study revealed that sexual dysfunctions are frequently associated with this specific type of CP. The prevalence of erectile dysfunction, premature ejaculation and ejaculatory pain was 19, 30 and 30 %, respectively. A variety of psychological problems exist among type IIIa CP patients, including depression, anxiety, somatization, obsessive-compulsive and interpersonal sensitivity. In particular, the severity of erectile dysfunctions, but not premature ejaculation and ejaculatory pain, correlated significantly with depression and anxiety. Conclusion: Our data indicate that a moderate level of sexual dysfunctions exists among the type IIIa CP patients, and highlight the association of depression and anxiety with erectile dysfunction in CP patients, suggestting that special attention should be paid to these psychological issues in clinical treatments of the prostatitis symptoms and the associated erectile dysfunctions.
Social Determinants of Chronic Prostatitis/Chronic Pelvic Pain Syndrome Related Lifestyle and Behaviors among Urban Men in China: A Case-Control Study

Abstract:Purpose. In order to find key risk factors of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) among urban men in China, an age-matched case-control study was performed from September 2012 to May 2013 in Yichang, Hubei Province, China. Methodology. A total of 279 patients and 558 controls were recruited in this study. Data were collected by a self-administered questionnaire, including demographics, diet and lifestyle, psychological status, and a physical exam. Conditional logistic regression model was used to analyze collected data. Results. Chemical factors exposure, night shift, severity of mood, and poor self-health cognition were entered into the regression model, and result displayed that these four factors had odds ratios of 1.929 (95% CI, 1.321–2.819), 1.456 (95% CI, 1.087–1.949), 1.619 (95% CI, 1.280–2.046), and 1.304 (95% CI, 1.094–1.555), respectively, which suggested that these four factors could significantly affect CP/CPPS. Conclusion. These results suggest that many factors affect CP/CPPS, including biological, social, and psychological factors.

Chinese Urologists' Practice Patterns of Diagnosing and Treating Chronic Pelvic Pain Syndrome: a Questionnaire Survey.

Abstract:OBJECTIVE:To investigate the application of the Chinese Urological Association (CUA) guidelines on prostatitis and the effects on the clinical practice patterns of diagnosing and treating chronic pelvic pain syndrome (CPPS) among Chinese urologists.METHODS:We conducted a questionnaire investigation of the CUA guidelines on prostatitis among the urologists from 399 hospitals in 63 cities of China, and performed statistical analyses on all the eligible questionnaires collected.RESULTS:Of the 2 251 questionnaires distributed, 2 046 (90.9%) were eligible, of which 92.5% were from the urologists in tertiary or secondary hospitals, of whom 72.3% had senior or intermediate professional titles, and 90.2% had studied the CUA guidelines. Most respondents agreed that Type III prostatitis was a clinical syndrome, of which the diagnosis should be made after other conditions with similar symptoms had been ruled out and the aim was to relieve pain, alleviate urination symptoms and improve quality of life. Those who had and those who had not studied the CUA guidelines differed in their viewpoints on CPPS as illustrated in the guidelines. In clinical practice, the most common treatment options for CPPS were pharmaceutical therapy (95.0%), life style adjustment (88.9%), and psychotherapy (79.9%), and the most frequently prescribed drugs were phytotherapy (84.5%), α-blockers (79.0%) and antibiotics (64.0%).CONCLUSION:CUA guidelines on prostatitis has gained a nationwide application and promoted the standardization of the management of CPPS in China.
Prevalence of and risk factors for asymptomatic inﬂammatory (NIH-IV) prostatitis in Chinese men

Abstract:Background: While many investigators have studied symptomatic prostatitis, little research has been done with regard to asymptomatic (NIH-IV) prostatitis. Purpose: To describe the prevalence of and risk factors for NIH-IV prostatitis among a large male population. Methods: The study population was comprised of 1,868 men at the second phase recruitment of a population-based cohort in China. Asymptomatic and symptomatic men were defined by the National Institutes of Health Chronic Prostatitis (CP) Symptom Index. Meanwhile, EPS specimens and their leukocyte count were collected. Lifestyle and demographic characteristics were obtained through a questionnaire. Results: Prevalence of NIH-IV prostatitis was 21.1% among 1,868 asymptomatic men aged 19-78 years and increased with age. After adjusteing for potential confounding variables (age, smoking habits, alcohol drinking habits, education, physical activity, hypertension, dyslipidemia, obesity and diabetes), age remained a significant factor for NIH-IV prostatitis (OR = 1.35; 95% CI = 1.06-1.71; P = 0.01) and the risk of NIH-IV prostatitis was significantly higher in smokers≧15 pack/years than non-smokers (OR = 1.33; 95% CI = 1.01-1.75; P = 0.03). In addition, compared with non-drinkers, the OR of NIH-IV prostatitis in drinkers ≧1 drinks/week was 1.35 (95% CI = 1.03, 1.77, p = 0.02) after adjusting for the other variables above. In addition, having less than a college education may be a risk factor for NIH-IV prostatitis, although a statistically significant difference did not exist in our data (OR = 1.22; 95% CI = 0.97-1.52; P = 0.08). Conclusions: Our findings suggest that NIH-IV prostatitis is prevalent in China. Age, smoking, drinking and lower education levels were associated with an increased risk of NIH-IV prostatitis. The prevalence of NIH-IV prostatitis should be taken into account when estimating the total prevalence of CP in future studies.
Male's Reproductive Tract Infections in 9 Provinces of China:Self-reported Prevalence and Determinants

Abstract:Objective:To explore the prevalence and determinants of male's RTI in 9 provinces of China.Method:By using multi-stage cluster sampling method,a retrospective survey was conducted in 9 provinces of China.Male adults aged 20-49 years old were interviewed and the data on their living conditions,demography information,reproductive health and symptoms of RTI occurred in last 12 months were collected.Four symptoms including ‘Urethra itches',‘Have sores or burns when urinating',‘Have blebs or ulcers near penis',and ‘Abnormal discharge near penis',were used as indicators of RTI.The rate and determinants of RTIs were explored.Chi-square test and multinomial multilevel logistic regression were used for this analysis.Results:Among the 13 794 adult males,6.1% reported at least one symptom of RTIs during last year.The prevalence were 2.8% among unmarried males and 6.8% among married males.Only half of the males who had any symptom of RTIs went to see a doctor.Multilevel logistic regression analysis indicated that married men,farmers,men who smoke and/or drunk were more likely to report symptoms than their counterparts.Moreover,male's symptoms were significantly associated with poor living conditions,including no tap water and no chimney in the kitchen.It was also found that the incidence of male's RTI was significantly higher in some regions than others.Conclusion:Male's incidence of RTI is high but about half of infected men seek medical treatment.Men who live in rural areas with poor living condicions and who smoke and / or drinking alcohol are associated with higher risk of RTIs than their counterparts and more health education for RTI prevention is needed in rural areas.

Abstract:Aims To investigate the clinical characteristics of health care-seeking men presenting with lower urinary tract symptoms (LUTS) in China and to reveal risk factors for symptom severity. Methods This multicenter, hospital-based, cross-sectional study recruited 1477 eligible male subjects, who were at least 45 years, seeking health care at 9 participating hospitals across the mainland China. The general medical information and subjective symptoms were recorded, followed by the measurement of prostate volume, urodynamic indices, and laboratory tests for kidney function, plus glucose/lipid metabolism. Univariate and multivariate linear regression were employed for the detection of risk factors for symptom severity. Results The proportion of mild, moderate, and severe LUTS was 14.6%, 32.6%, and 52.8%, respectively, with 62.2% reporting the triple combination of storage, voiding, and postmicturition symptoms. Median prostate volume was 44.6 ml, and 71.1% were experiencing comorbidities. Thirteen independent risk factors for LUTS severity were identified, namely, nocturnal voiding episodes and the presence of straining and weak steam; the triple combination of symptom subtypes; general and nocturia quality of life; Q(max) and bladder outlet obstruction index; and numbers of comorbidities, hypertension, estimated glomerular filtration rate, and cholesterol and glycosylated hemoglobin. Conclusions The majority of health care-seeking LUTS men present with moderate-to-severe and overlapping symptoms, with a high prevalence of both lower urinary tract dysfunction and systemic comorbidities. The evidence from both urological and nonurological independent risk factors demonstrate the multifactorial nature of LUTS, for which a multidisciplinary management is essential.
Preliminary study of the effects of pathogens on chronic prostatitis

Abstract:Objective To investigate the effects of pathogenic organisms on chronic prostatitis.Methods A total of 101 patients with chronic prostatitis(symptomatic and/or WBC count >10/HP in prostatic secretion) and 68 healthy controls underwent tests of midstream urine culture,WBC counting of expression of prostate secretion(EPS),EPS/post-prostatic massage urine(PPMU) culture,and sexually transmitted disease(STD) test of EPS/PPMU.The positive rates of pathogens were compared between the 2 groups using χ~2 test.Results The positive rates of bacterial culture with midstream urine in patient group and controls were 5.0%(5/101) and 4.4%(3/68),respectively;and the rates with EPS/PPMU were(37.6)%(38/101) and 39.7%(27/68),respectively.There was no significant difference in the positive rates between the 2 groups(P>0.05).The positive rate of STD bacterial culture with EPS/PPMU in patient group was 35.6%(36/101);and that in controls was 35.3%(24/68).Similarly,no significant difference was found between the 2 groups(P>0.05).In addition,no significant correlation was found between the positive EPS/ PPMU cultures and WBC counts in EPS in patient group.Conclusions There is no significant difference in species and prevalence of pathogenic microorganisms in urine and EPS/PPMU between chronic prostatitis patients and healthy controls.It is suggested that microorganisms may not be the cause of chronic prostatitis and that the prostate is often colonized by commensal urethral bacteria in men with or without chronic prostatitis.

Abstract:Study design, materials and methods The population-based, cross-sectional field survey was conducted between July 2011 and February 2012 in five cities of China using questionnaires regarding demographics, the prevalence and the HRQoL. A random sample of men and women aged≥18 years residing in the five cities who were representative of the general populations was selected for demographic questionnaires, Overactive Bladder Symptom Score (OABSS) and International prostate symptom score(IPSS) first. The individuals meeting the diagnostic criteria of LUTS were further interviewed through King’s health questionnaire (KHQ) and ICIQ-MLUTS/ICIQ -FLUTS to estimate the impact of LUTS on HRQoL. Using 2002 ICS definitions, the prevalence estimates of storage, voiding, and postmicturition LUTS were calculated. Data were weighted by city, age cohort, and gender.

Abstract:OBJECTIVES:To establish the chronic prostatitis symptom index which more suitable for Chinese to refine and standardize evaluation of current symptoms in men with "chronic prostatitis".METHODS:The literature of previous work was reviewed to develop a symptom index instrument. There were 18 questions representing three broad categories: pain symptoms, urinary symptoms and other symptoms. To evalute the clinical utility and applicability for Chinese people of this tool, we used it in 100 patients with chronic prostatitis and 100 control patients (40 with benign prostatic hyperplasia, BPH; 30 with infertility; 30 with erectile dysfunction).RESULTS:Of the three aspects symptoms, the primary component was pain. But pain in lower back and lower abdomen had less specificity. Pain in the rectal area was not very common but it could well distinguish patients with prostatitis from those BPH, infertility and ED. Urinary symptoms were also common. We used 5 symptom questions in urinary aspect in the last version of this index. In other symptoms, we dropped the question of uncomfortable of waist and back at last. It could not distinguish patients with prostatitis from those infertility and erectile dysfunction. Finally we analyzed the result and modified the new version of chronic prostatitis symptom index. It included 5 pain symptom questions, 5 urinary symptom questions and 2 other symptom questions. There are 12 questions in total.CONCLUSIONS:The chronic prostatitis symptom index we developed was validated and useful in clinical practice as well as research protocols. Moreover, it was more suitable for Chinese people.

Abstract:OBJECTIVE:To investigate the application of the Chinese Urological Association (CUA) Guidelines on Prostatitis and its effects on the clinical practice patterns of diagnosing and treating chronic pelvic pain syndrome (CPPS) among Chinese urologists and andrologists.METHODS:We conducted a questionnaire investigation on the application of the CUA Guidelines on Prostatitis among the urologists and andrologists of 173 hospitals in 21 cities of China, and performed statistical analyses on all the eligible questionnaires collected.RESULTS:Of the 1 056 questionnaires distributed, 851 (80.6%) were eligible, of which 71.6% were from the urologists or andrologists in grade 3 hospitals, 80.7% of them with senior or intermediate professional titles and 97.5% had studied the CUA Guidelines. Most of the subjects agreed that Type III prostatitis is a clinical syndrome, whose diagnosis should exclude other conditions with similar symptoms, and whose treatment should aim at relieving pain, alleviating urination symptoms and improving the quality of life. Those who had and those who had not studied the CUA Guidelines differed in their viewpoints on CPPS as illustrated in the book. In clinical practice, the most common treatment options for CPPS were psychological therapy (80.7%), medication (80.4%) and life style adjustment (79.6%), and the most frequently used drugs were phytotherapy (80.0%), alpha-blockers (68.9%) and antibiotics (61.0%).CONCLUSION:CUA Guidelines on Prostatitis has gained a nationwide application and promoted the standardization of the management of CPPS in China.

Abstract:Chronic prostatitis is one of the most common diseases in andrology,and the incidence is rising.Through the acquisition of the focal information to reveal syndrome type of traditional Chinese medicine on chronic prostatitis,it may provide some quantified indexes for the normalization and standardization of syndrome differentiation.Since the 1980s,prostate local syndrome differentiation has been paid a lot of attention by many scholars;different scholars put forward different theories and methods.In this paper,the concept of Chinese medicine prostate,the evolution process of prostate local syndrome differentiation,as well as the research progress of local syndrome differentiation of chronic prostatitis was summarized.

Abstract:Objective Rapid population ageing in China is increasing the prevalence of lower urinary tract symptoms suggestive of benign prostatic hyperplasia (LUTS/BPH) among older people. The associated economic burden is increasing as well. Relevant data from China are currently insufficient. Design Secondary analysis of a cohort sample. Setting A nationally representative, cross-sectional survey-the China Health and Retirement Longitudinal Study (CHARLS)-was conducted in 2011 in mainland China. Participants The study included individuals in the community selected from CHARLS by multistage probability sampling. A total of 5888 participants aged 50 years and above were included. Outcome measures Self-reported morbid state was derived from a structured questionnaire. The weighted prevalence of LUTS/BPH was estimated and stratified by age group, marital status, education level, economic level, residential area and geographical region. Multivariable weighted logistic regression was used to examine the association of socioeconomic status with the odds of BPH. Results The weighted overall prevalence of LUTS/BPH was 10.66% (95% CI 9.36 to 12.12). Among individuals aged over 70 years, the prevalence was 14.67% (95% CI 11.80 to 18.09) and it increased with ageing (p<0.05). The prevalence of LUTS/BPH among subjects residing in urban areas was higher (13.55%, 95% CI 10.95 to 16.64) than those living in rural areas (8.38%, 95% CI 6.90 to 10.15). The prevalence of LUTS/BPH was lowest in the South-Central and South-West regions and highest in the North-West region. Conclusions We found an increasing trend of prevalence of LUTS/BPH with ageing. It varied according to marital status, socioeconomic status and geographical region.

Abstract:Background: Mycoplasma genitalium (M. genitalium) has been shown to be involved in chronic non-gonococcal urethritis (NGU). However, the prevalence and determinants of this emerging sexually transmissible infection among prostatitis patients remain obscure. Methods: Two hundred and thirty-five patients diagnosed with prostatitis and 152 health controls from sexually transmitted diseases (STD) clinics in Shanghai, China, were selected. M. genitalium was detected in the initial voided urine (VB1), midstream of urine (VB2), expressed prostatic secretion (EPS) and the opening urine after massage (VB3) by quantitative polymerase chain reaction (Q-PCR) targeting the Mycoplasma genitalium adhesion protein (MgPa). An infection of the prostate was considered positive if a uropathogen was found only in the EPS sample or VB3, or if it was at least four-fold greater in EPS or VB3 than in VB1 or VB2. The prostatitis patients with M. genitalium infection were treated with azithromycin. Results: The prevalence of M. genitalium infection was significantly higher among the prostatitis group than the control group (10 vs 3%, P = 0.005). Among the prostatitis group, M. genitalium infection was significantly associated with those patients who received treatment for genitourinary infection previously than those patients who did not (17 vs 6%; adjusted OR, 4.011; 95% CI, 1.562-10.300). The symptoms were totally or partially improved in 83% per cent (19/23) of prostatitis patients with M. genitalium, positive in EPS and M. genitalium turned negative after azithromycin treatment. Conclusions: M. genitalium was prevalent in the patients with prostatitis, particularly in those who received ineffective antibiotic treatment for the bacterium, and was identified as having a significant association of prostatitis.

Abstract:Objective\r\nThis multi-center study aims to investigate the PSA level, prostate volume, biopsy scheme and biopsy method of patients receiving prostate biopsy in Chinese Prostate Cancer Consortium (CPCC) hospitals and identify the detection rate of prostate cancer (PCa) in different PSA ranges in Chinese.\r\n\r\n\r\nMethods\r\nDetailed clinical information of consecutive patients underwent prostate biopsy in 33 CPCC hospitals were recorded with EpiData Entry 3.1. Those information included PSA level, prostate volume, biopsy scheme and biopsy method. All statistics analysis was performed with SPSS 17.0. T test was applied to compare age between different groups. Mann-Whitney U test was applied to compare PSA, prostate volume, and percent free PSA.\r\n\r\n\r\nResults\r\nA total of 19292 cases were collected. After excluding cases with factors that would influence PSA level, 13904 cases from 22 hospitals were involved in further investigation. The medium volume of prostate was 44.0ml (range 31.1~65.5 ml). The distribution of patients in different PSA level included 701 cases (5%) in PSA 50.0 μg/L. There were 10808 cases who underwent transrectal ultrasound-guided prostate biopsy and 3087 cases who underwent transperineal biopsy. There were 312(2%), 1436(10%), 2640(19%), 3920(28%), 4989(36%) and 607(4%) cases who underwent non-systematic biopsy, 6-, 8-, 10-, 12- core and saturation biopsy, respectively. PCa detection rates were 44%, 26% and 35% in all patients, patients with PSA 4.1-10.0 μg/L and 10.1-20.0 μg/L, respectively. The distribution of Gleason scores in those patients with positive biopsy included the 1295 cases (21%) with less than 7 scores, 1164 cases (19%) with 3+ 4 scores, 1155 cases (19%) with 4+ 3 scores, 2371 cases with more than 8 scores and 138 cases (2%) with uncertain scores.\r\n\r\n\r\nConclusion\r\nThe most widely applied biopsy scheme in CPCC hospitals is transrectal ultrasound-guided biopsy. The PSA level of patients in CPCC hospitals is relatively high.
