Treatment of proximal humeral fractures with percutaneous pinning and retrograde intramedullary nailing

Abstract:Objective To investigate clinical results of percutaneous pinning and retrograde intramedullary nailing in the treatment of proximal humeral fractures. Methods We treated 29 cases of proximal humeral fractures with percutaneous pinning and internal fixation (21 cases), retrograde intramedullary nailing (5 cases), or percutaneous pinning and retrograde intramedullary nailing (3 cases) between June 1999 and December 2003. Results Fracture union was achieved in all the 29 cases in 8~12 weeks, without delayed union. There were no complications as pinpoint infection, injury of blood vessel, iatrogenic radial nerve injury, brisement of streak needle, or osteonecrosis of the humeral head. Symptoms of axillary nerve injury were seen in 1 case receiving percutaneous pinning, and disappeared 2 months later after treatment. According to the Neer Shoulder Joint Score Standard, there were 10 cases of excellent results, 15 cases of good, 3 cases of fair, and 1 case of poor, the rate of excellent-or-good results being 86.2% (25/29). Conclusions Both percutaneous pinning and retrograde intramedullary nailing are good methods for treating proximal humeral fractures, with satisfactory therapeutic efficacy and low complication incidence.
