Abstract:Objective To investigate the correlation between severe pre-eclampsia and the changes of factors such as GMP-140,D-dimer (D-D) in pro-coagulation. Methods 200 pregnant women of 20 to 36 gestational weeks were collected and classified into 4 groups according to the gestational weeks. PT,APTT,and the level of PLT,GMP-140,D-D,FIB in the peripheral blood were detected. The pregnant women who had two or more abnormal indexes were considered as high risk,and were divided into two groups randomly. The treatment group was dealt with intravenous drip of heparin and salvia,while the control group was given normal sodium instead. The indexes of blood coagulation system were measured dynamically and the disease incidence was collected. Results The levels of PLT,PT and APTT were significantly lower in heavy pre-eclampsia patients (P<0.05),with the concentrations of GMP-140 and D-D in plasma significantly higher (P<0.05),compared to the control group. As for predicting,when the level of GMP-140 greater than or equal to 68.0 lg/L and D-D greater than or equal to 0.28mg/L,the positive predictive value of the incidence of heavy pre-eclampsia was 55.5% while the negative predictive value reached 78.1%. The specificity and sensitivity were 51.3% and 81.2% in the first group,respectively. In the second group,these indexes were 82.1%,79.3%,68.2% and 84.4% respectively. In the third group ,they were 85.3%,83.1%,76.4% and 85.3% respectively. In the forth group ,they were 87.2%,89.1%,80.8% and 90.6% respectively. The incidence rate of severe pre-eclampsia was 7.5% in the treatment group and 17.76% in the control group. There was a significant difference between the two groups (P<0.05). Conclusion GMP-140 and D-D are two reliable indicators for pre-eclampsia in patients of pre-coagulation. Moreover,the incidence of pre-eclampsia can be degraded if heparin and salvia are used early.

Abstract:Pre-eclampsia is a complication of pregnancy characterized by hypertension and proteinuria that results in maternal and neonatal mortality and morbidity.Although the etiology remains unclear,it seems to be initiated by placental factors that enter the maternal circulation and may cause endothelial dysfunction.Recently,anti-angiogenic factors such as endoglin,the soluble form of vascular endothelial growth factor receptor(sFlt-1) as well as the placental growth factor have been found to be associated with the onset of pre-eclampsia.This review will summarize our current understanding of molecular structure of sFlt-1,signal pathways,with an emphasis on its role in the pathogenesis of preeclampsia and regulative factors.
Maternal alternations of haemostasis and thrombosis in preeclampsia

Abstract:Objective To investigate the maternal changes of haemostasis and thrombosis in preeclampsia,including vascular endothelial functions,platelet activation,coagulant activation,anticoagulant activation and fibrinolytic activation. Methods Maternal serum concentrations of vWF,TF,TFPI,sP-selectin,AT-Ⅲ,FPA and D-Dimer were measured by ELISA in forty-six women with preeclampsia and forty normal pregnant women before and after delivery. Results Compared with maternal serum in normal pregnancy,higher concentrations of maternal serum TF,vWF and sP-selectin as well as lower concentrations of maternal serum TFPI and D-Dimer in preeclampsia were observed before and after delivery. Maternal serum levels of AT-Ⅲ decreased at delivery and one day after delivery,but increased at five days after delivery. A lower trend of maternal serum concentrations of vWF,TF and sP-selectin were found after delivery,companied with a higher trend of maternal serum concentrations of TFPI and DDimer. Maternal serum concentrations of FPA were similar between two groups before and after delivery. Conclusions Pathological hypercoagulability and thrombophilias resulted from changes in the haemostatic system which included impaired endothelial functions,increased coagulation and platelet activation and decreased anticoagulation and fibrinolysis are the important pathophysiological alternations in preeclampsia.

Abstract:To study the dynamic changes in perinatal coagulation function in patients with preeclampsia (PE).The general data and coagulation indexes of 290 PE patients during the perinatal period (prenatal and 1 and 3 days postpartum) and 256 healthy pregnant women in the third trimester of pregnancy were investigated, and the data were analyzed.Compared with healthy pregnant women, prothrombin time (PT), fibrinogen (FIB), platelet count (PLT), mean platelet volume (MPV), thrombocytocrit (PCT), maximum amplitude (MA), and coagulation index (CI) of PE patients decreased, and activated partial thrombin time (APTT), thrombin time (TT), D-dimer (DD), platelet distribution width (PDW) and K values increased before delivery (P < .05). APTT and FIB in PE patients were lower in the day 1 postpartum group than in the prenatal and postpartum day 3 groups, and TT, DD, and fibrin degradation products (FDP) were higher (P < .05). PCT and MPV were highest in the prenatal group (P < .05).Compared with that of healthy pregnant women, the coagulation function of PE patients is in a relatively low-coagulation and high-fibrinolysis state on postpartum day 1, which increases the risk of postpartum hemorrhage and other adverse outcomes.

Abstract:Objective: The aim of this study was to investigate the role of hemostatic factors in the pathogenesis of preeclampsia. Materials and Methods: Maternal and cord plasma concentrations of tissue factor (TF), tissue factor pathway inhibitor (TFPI), von willebrand factor (vWF), soluble P-selectin (sP-selectin), fibrinopeptide A (FPA), D-dimer, and antithrombin III (AT-III) were measured by enzyme-linked immunosorbent assay (ELISA) in 46 women with preeclampsia and 40 normotensive pregnant women before and after delivery. Results: The maternal plasma concentrations of TF, vWF, and sP-selectin were higher, but lower concentrations of TFPI, AT-III, and D-dimer were observed in women with preeclampsia compared to normotensive pregnant women before and after delivery. Compared with maternal plasma, fetal plasma concentrations of TF concentrations were increased significantly in both groups, whereas vWF, FPA, TFPI, AT-III, and D-dimer were decreased. Compared with normotensive pregnancy, fetal plasma concentrations of TF were markedly increased in preeclampsia, accompanied with a higher vWF and a lower sP-selectin and D-dimer levels. Furthermore, fetal plasma TF concentrations were more significantly increased in women with high blood pressure and severe proteinuria. Conclusions: Imbalance in the coagulation/fibrinolysis equilibrium, especially alterations in the extrinsic pathway of coagulation and anticoagulation, may play an important role in the pathogenesis of preeclampsia. In addition, fetal alteration of TF may be involved in the pathogenesis of fetal complications of preeclampsia.

Abstract:Preeclampsia (PE) is a severe pregnancy complication characterized by significant alterations in coagulation function. This study aims to analyze the correlation between coagulation function, platelet parameters, and pregnancy outcomes in PE patients. Clinical data, along with blood and urine samples, were collected from 168 PE patients and 128 healthy pregnant women. General demographic and laboratory testing data were recorded, and maternal and fetal outcomes were followed up. Data were analyzed using Kaplan–Meier and logistic regression analyses. In mild PE patients, thrombin time (p = .000), platelet distribution width (PDW) (p = .000), and clot formation time (p = .000) were increased, while prothrombin time (p = .000) and fibrinogen (p = .045) were reduced. With increasing PE severity, prothrombin time (p = .000), platelet count (PLT) (p = .000), mean platelet volume (MPV) (p = .000), plateletcrit (p = .000), maximum amplitude (MA) (p = .000), and coagulation index (p = .001) decreased, whereas activated partial thromboplastin time (APTT) (p = .000), thrombin time (p = .002), D‐dimer (p = .026), and PDW (p = .000) increased. Lower prothrombin time (p = .048), PLT (p = .004), and coagulation index (p = .026) or higher APTT (p = .032), thrombin time (p = .044), D‐dimer (p = .023), and PDW (p = .016) were associated with a higher risk of poor pregnancy outcomes. Thrombin time was identified as an independent risk factor (p = .025, OR = 2.918, 95% CI: 1.145–7.436), whereas gestational age was an independent protective factor (p = .000, OR = 0.244, 95% CI: 0.151–0.395). This study demonstrates that specific coagulation and platelet parameters are significantly associated with PE severity and adverse pregnancy outcomes. These findings highlight the importance of monitoring coagulation function in PE patients to improve clinical management and outcomes.

Abstract:Preeclampsia is a hypertensive disorder of pregnancy. It affects 2% to 8% of pregnancies worldwide and causes significant maternal and perinatal morbidity and mortality. Hypertension and proteinuria are the cornerstone of the disease, though systemic organ dysfunction may ensue. The clinical syndrome begins with abnormal placentation with subsequent release of antiangiogenic markers, mediated primarily by soluble fms-like tyrosine kinase-1 (sFlt-1) and soluble endoglin (sEng). High levels of sFlt-1 and sEng result in endothelial dysfunction, vasoconstriction, and immune dysregulation, which can negatively impact every maternal organ system and the fetus. This review comprehensively examines the pathogenesis of preeclampsia with a specific focus on the mechanisms underlying the clinical features. Delivery is the only definitive treatment. Low-dose aspirin is recommended for prophylaxis in high-risk populations. Other treatment options are limited. Additional research is needed to clarify the pathophysiology, and thus, identify potential therapeutic targets for improved treatment and, ultimately, outcomes of this prevalent disease.
Confusion in the monitoring of coagulation function in pregnant and neonate patients with severe disease: A case reports and brief literature review

Abstract:Rationale: Different populations have their own unique physiological and pathological characteristics. However, in specialized maternal and child hospitals, there is currently a lack of standardized methods for assessing coagulation dysfunction, both domestically and internationally. Patient concerns: A 19-day-old neonate was transferred to neonatal intensive care unit with cyanosis, nasal bleeding for 6 hours, and a consciousness disorder for 5 hours. A 33-year-old woman presented with hydramnios and a 39 + 3week intrauterine pregnancy. All indicators before delivery were normal, but postpartum hemorrhage occurred after delivery. Diagnoses: We retrospectively analyzed 1 neonate with pulmonary hemorrhage accompanied by thrombocytopenia and 1 pregnant patient with amniotic fluid embolism. Interventions: The new coagulation indicators, such as thrombin-antithrombin complex, plasmin-alpha 2 antiplasmin complex, thrombomodulin, and tissue plasminogen activator-plasminogen activator inhibitor-1 complex, have been indicated to be valuable. In neonates, it is necessary to continuously monitor special items combined with specific therapeutic agents, such as tranexamic acid. In cases where postpartum hemorrhage occurs with low fibrinogen levels, it is essential to effectively identify patients with severe amniotic fluid embolism from a high incidence of specimen clotting. Outcomes: The neonate's oxygen saturation stabilized, and after 5 days of treatment with low molecular weight heparin, thrombin-antithrombin complex and plasmin-alpha 2 antiplasmin complex returned to normal levels. The pregnant began to remove the remaining thrombus, the patient's condition recovered, and she had a good prognosis. Lessons: For pregnant and neonatal critical illnesses, it is necessary to develop personalized coagulation monitoring programs that provide realistic and reasonable treatment recommendations. Such programs should consider the unique physiological and pathological characteristics of different populations to ensure effective management of critically ill patients.

Abstract:Preeclampsia is a common pregnancy complication that causes maternal and fetal morbidity and mortality.Although the incidence is higher in nulliparous women than in multiparae,any multipara who has a history of PE bears a higher risk,and if the previous PE is of an early onset,or complicated with eclampsia,HELLP syndrome or FGR,the recurrence risk goes even higher,probably with an even severer degree.The study of the reccurent PE is very challenging for obstetricians. For the multiparous women, the obstetricians should focus on providing a better antenatal counselling,promoting the mothers' health, standardizing the prenatal examination,taking comprehensive preventive measures,monitoring the gestational process and delivery timing,so as to maximize the interests of both the mothers and the babies.
Association of a Reduction of G-protein Coupled Receptor 30 Expression and the Pathogenesis of Preeclampsia

Abstract:Preeclampsia is a pregnancy-specific disorder, which is a leading cause of maternal and perinatal mortality and morbidity. A lower increase of estrogen, compared with the increase in progesterone, is associated with pathogenesis of the disease during pregnancy. G-protein-coupled receptor 30 (GPR30) mediates the action of estrogen, however remains to be investigated in preeclampsia. The levels of GPR30 were measured in placentae from uncomplicated pregnancies and pregnancies complicated by preeclampsia using immunohistochemistry and western blotting. GPR30 expression was additionally measured in placental HTR8/SVneo cells following 17 beta-estrogen (E2) treatment in normal or hypoxia-reoxygenation conditions by western blotting. In addition, the outgrowth of HTR8/SVneo cells following E2 treatment in hypoxia-reoxygenation conditions was measured. Levels of GPR30 were significantly reduced in placentae from women with preeclampsia as compared with uncomplicated pregnancies. Treatment with E2 significantly increased the expression of GPR30 in HTR8/SVneo cells, in normal and hypoxia-reoxygenation conditions. Furthermore, treatment with E2 increased the outgrowth of HTR8/SVneo cells in hypoxia-reoxygenation conditions. The present study demonstrated lowered placental expression of GPR30 in preeclampsia. Estrogen treatment increases GPR30 expression in extravillous trophoblast and GPR30 may be involved in extravillous trophoblast invasion.

Abstract:Hypertensive disorders complicating pregnancy remains a leading cause of maternal and perinatal mortality and morbidity,and affects 5% to 7% of all pregnancies.The pathophysiologic changes associated with HDCP,such as small artery spasm,a reduction in colloid osmotic pressure while an increase in intravascular hydrostatic pressure,an increase in capillary permeability,can have a serious impact on the maternal respiratory function and lead to pulmonary edema,even if without left heart failure in the mother.Correct hypoxemia and remove the primary disease is the key to treatment.

The Interaction Between Lipid Metabolism and Blood Coagulative-Fibrinolytic Activities in Preeclampsia and Normal Pregnancy

Abstract:Objective To investigate the changes of lipid metabolism and blood coagulative-fibrinolytic activities in normal pregnancy and their roles in pathogenesis of preeclampsia (PE). Methods Prospective study was carried out in this study. Sera TG, TC, LDL, HDL, FIB, D-Dimer and AT-Ⅲ was measured consecutively in 114 normal pregnant women during 10-14, 20-24 and 30-34 gestational weeks. Four of them developed late-onset PE mildly. Data were statistically analyzed by t-test, repeated measure ANOVA and correlation analysis. Results Sera TG, TCHO and LDL elevated as gestational age increased in normal pregnant women (P0.01). FIB and D-Dimer were increased and AT-Ⅲ decreased as gestational age increased in normal pregnancy (P0.01). Sera TG and TG/HDL ratio in PE were higher than those in control group but without statistically significant difference (P0.05). Sera HDL, TCHO and LDL were less in preeclampsia than control group without statistically significant difference (P0.05). FIB was higher in PE than that in control group without statistically significant difference (P0.05). The correlation analysis indicated that TCHO, TG and LDL was positively associated with the levels of FIB and D-Dimer, and inversely associated with the levels of AT-Ⅲ significantly. The ratio of AT-Ⅲ/TG elevated as gestational age increased in normal pregnancy (P0.01). The ratio of AT-Ⅲ/TG was lower and FIB/HDL was higher in PE than those in control group without significant difference (P0.05). Conclusions The lipid metabolism and blood coagulative-fibrinolytic activities is related to gestational age in normal pregnancy. Abnormal changes of lipid metabolism and blood coagulative-fibrinolytic activities play roles in preeclampsia.
Blood coagulation parameters and platelet indices: changes in normal and preeclamptic pregnancies and predictive values for preeclampsia

Abstract:Background: Preeclampsia (PE) is an obstetric disorder with high morbidity and mortality rates but without clear pathogeny. The dysfunction of the blood coagulation-fibrinolysis system is a salient characteristic of PE that varies in severity, and necessitates different treatments. Therefore, it is necessary to find suitable predictors for the onset and severity of PE. Objectives: We aimed to evaluate blood coagulation parameters and platelet indices as potential predictors for the onset and severity of PE. Methods: Blood samples from 3 groups of subjects, normal pregnant women (n = 79), mild preeclampsia (mPE) (n = 53) and severe preeclampsia (sPE) (n = 42), were collected during early and late pregnancy. The levels of coagulative parameters and platelet indices were measured and compared among the groups. The receiver-operating characteristic (ROC) curves of these indices were generated, and the area under the curve (AUC) was calculated. The predictive values of the selected potential parameters were examined in binary regression analysis. Results: During late pregnancy in the normal pregnancy group, the activated partial thromboplastin time (APTT), prothrombin time (PT), thrombin time (TT) and platelet count decreased, while the fibrinogen level and mean platelet volume (MPV) increased compared to early pregnancy (p<0.05). However, the PE patients presented with increased APTT, TT, MPV and D-dimer (DD) during the third trimester. In the analysis of subjects with and without PE, TT showed the largest AUC (0.743) and high predictive value. In PE patients with different severities, MPV showed the largest AUC (0.671) and ideal predictive efficiency. Conclusion: Normal pregnancy causes a maternal physiological hypercoagulable state in late pregnancy. PE may trigger complex disorders in the endogenous coagulative pathways and consume platelets and FIB, subsequently activating thrombopoiesis and fibrinolysis. Thrombin time and MPV may serve as early monitoring markers for the onset and severity of PE, respectively.

Abstract:Background Hypertensive complications during pregnancy play a significant role in the increased rates of maternal and perinatal morbidity and mortality on a global scale. Preeclampsia is characterized by elevated blood pressure levels and the presence of protein in the urine and is associated with diverse hematological alterations, particularly impacting the coagulation cascade. The primary objective of this research was to conduct a comparative analysis of the coagulation profiles and pregnancy outcomes in women with preeclampsia versus those with normal blood pressure during pregnancy. Methods This was a prospective case-control study with 74 participants across two groups, conducted from September 2022 to May 2024. The participants were enrolled and divided into two groups, with 37 in the clinically diagnosed preeclampsia group and 37 in the normotensive group. Coagulation parameters including platelet count, bleeding time, clotting time, international normalized ratio (INR), activated partial thromboplastin time (APTT), prothrombin time (PT), fibrinogen levels, alkaline phosphatase (ALP) levels, D-dimer levels, and fibrin degradation products (FDP) levels were assessed. Maternal and neonatal outcomes were also compared. In our study, we comprehensively examined both maternal and neonatal outcomes in preeclampsia and normotensive groups. Maternal complications analyzed included mode of delivery, incidence of eclampsia, placental abruption, hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome, postpartum hemorrhage (PPH), and peripartum cardiomyopathy (PPCM). For neonatal outcomes, we assessed birth weight, appearance, pulse, grimace, activity, and respiration (APGAR) scores, and the duration of neonatal intensive care unit (NICU) stays. Results The results showed that mean platelet count was significantly lower in the preeclampsia group (151,503 ± 59,875/µL) compared to the normotensive group (245,405 ± 69,021/µL) (p < 0.0001). Bleeding time, INR, APTT, and PT showed significant elevation in the preeclampsia group, indicating a slower coagulation process. Fibrinogen levels, ALP levels, and D-dimer levels were significantly higher in the preeclampsia group (p < 0.0001). The preeclampsia group had a higher rate of cesarean sections (65% vs. 24%) and lower neonatal birth weights (mean 2.3 kg vs. 2.5 kg). APGAR scores were comparable between groups, but a higher number of neonates went to the NICU in the preeclampsia group (64.9% vs. 10.8%). The preeclampsia group also showed higher rates of low birth weight (27%), intrauterine growth restriction (27%), respiratory distress syndrome (10.8%), and asphyxia (5.4%). Conclusion Preeclampsia is associated with significant hematological changes, particularly in coagulation parameters, and adverse fetomaternal outcomes. Early identification and monitoring of these changes are crucial for timely intervention and improving maternal and neonatal health outcomes.
Association of Imbalanced Sex Hormone Production with Excessive Procoagulation Factor SerpinF2 in Preeclampsia

Abstract:Background: Preeclampsia, a serious pregnancy-associated syndrome, is the leading cause of maternal and perinatal morbidity and mortality. Significant exacerbation of the hypercoagulation status as well as imbalanced steroid hormones have been reported in developed preeclampsia. However, it remains unclear whether the two pathological changes are directly associated. Method and results: Our proteomic analysis revealed a significantly elevated SerpinF2/alpha 2-antiplasmin level in preeclampsia plasma. Measurement of the longitudinally gestational change of plasmin-alpha 2-antiplasmin (PAP) complex, testosterone, estradiol in preeclampsia patients and normal pregnant women demonstrated that the circulating PAP and testosterone levels in the early-onset preeclampsia (E-PE) patients were substantially higher, whereas estradiol concentration was significantly lower than that in normal pregnant controls from early pregnancy throughout gestation. Correlation analysis revealed that circulating PAP is in positive correlation with the concentration of testosterone, and in negative correlation with estradiol in E-PE patients. In E-PE placenta, the productions and activities of 17 beta-hydroxysteroid dehydrogenases 3 and aromatase, the essential enzymes for testosterone and estradiol synthesis, were compromised. In human renal and trophoblastic cells, testosterone and estradiol could regulate SerpinF2 expression in opposite ways. In addition, obvious fibrin deposition was colocalized with SerpinF2 in intervillous spaces and the area surrounding syncytiotrophoblasts in E-PE placenta. Conclusion: The findings reveal a tight correlation between the imbalanced steroid hormone production and the procoagulation factor in E-PE patients, which provide potential biomarkers to predict preeclampsia, and bring new insight into the pathogenesis of preeclampsia.
