Treatment of Advanced Pancreatic Cancer by Transarterial Infusion of Gemcitabine

Abstract:Purpose:To evaluate the efficacy of transarterial infusion combined gemcitabine protocol and conventional protocol in the treatment of advanced pancreatic cancers.Materials and Methods:To retrospectively analyze the clinical effectiveness of transarterial infusion chemotherapy of combined gemcitabine group and conventional group in the treatment of 65 patients with advanced pancreatic can- cers.Results:The tumor response rate was 32.7% in 65 cases.The clinical benefit response rate(CBR) was 56.9%.Median survival time for all patients was 9.0 months,the cumulative 3,6,9,12 months sur- vival rates were 90.8%、72.6%、45.6% and 28.7%,respectively.Median survival time for gemcitabine group was 11.0 months,the cumulative 3,6,9,12 months survival rates were90.6%、80.7%、57.1%、47.1%.A decrease(over 50%)of CA199 was observed in 14 cases(51.9%).Median survival time for con- ventional group was 8.0 months,the cumulative 3,6,9,12 months survival rates were90.9%、63.9%、33.6%、10.7%.A decrease(over 50%)of CA199 was observed in 13 cases(44.8%).The CBR in cases with pancreatic cancers of the head(Median survival time was 10.0 months)was 68.3%,which was obviously higher than pancreatic cancers of the distal region(Median survival time was 7.0 months)37.5%(P<0.01).Median survival time for 42 patients combined with chemoembolization was 9.0 months,a little longer than the patients with pure transarterial infusion treatment.Conclusion:The transarterial in- fusion chemotherapy combined with gemcitabine can provide patients who had advanced pancreatic cancer with higher CBR and longer median survival time,especially in pancreatic cancers of the head.Treated combined with chemoembolization to the cases with liver metastasis can mildly improved median survival time compared with pure transarterial infusion chemotherapy.
