Clinical Analysis of Hirayama Disease:192 Cases Come from Literature

Abstract:Objective:The purpose of this study was to review the clinical manifestations,the neuroelectrophysiological characteristic,the neuroradiological characteristic of Hirayama disease and its possible pathogenesis.Methods:"Hirayama disease" or "upper limb amyotrophy" was taken as key words .One hundred and sixty-seven documents were retrieved from tow databases of the CNKI net and CMA digital periodicals.To remove the cases of incomplete information and duplicate reports,there were 184 patients in 22 documents who fulfilled the clinical criteria for Hirayama disease.192 patients' clinical material was used for analysis(8 patients came from our hospital).Results:One hundred and ninety-two patients,male to female ratio = 6.7∶1.The mean age at onset was 18.6 years,progressive muscle weakness and wasting were characteristic and occurred predominantly in the distal part of the right upper limb.Tremulous movement of fingers were recorded in 77.6% of patients,cold paresis occurred in 81.3% of patients,the increase of latency of motor unit potentials was the most prominent finding in EMG studies in all patients.Abnormal EMG findings were recorded most frequently in the opposite limb.With the neck in the neutral position,MRI showed flattening of the lower cervical cord in 44.3% of patients.With neck flexion,all patients had anterior shifting of the posterior dura and engorged venous plexus in the posterior dural space.Neurogenic symptoms ceased to progress within3-4 years in most patients.Application of a cervical collar relieved the symptoms in the part of patients.Conclusion:Hirayama disease is characterized by insidiously,initially progressive muscular weakness and wasting of the distal upper limb(s)in young people,followed by a spontaneous arrest within several years.Wearing early a neck collar to prevent deterioration is useful.
