The Effects of Posterior Fusion Cage on Stabilization in Different Segmental Lumbar Spine

Abstract:Objective:Toprobetheeffectsofposteriorfusioncageon stabilization in differentsegmental lumbar spine. Methods: 6 fresh adult cadaver specimens of lumbosacral segments (L1~5) were used for biomechanical evaluation of posterior fusion cage on stabilization in different segmental lumbar spine. A three-dimensional movement device with the maximum load of 10N.M were applied to produce 6 kinds of pure moment of force and to create 6 kinds of movement of the lumbar spine in flexion, extension, left and right lateral bendings as well bilateral axial rotations. Measurements were done in turn on each specimen for five conditions: intact condition, unstable condition, bilateral medial 1/3 facetectomy with cages, bilateral medial 1/2 facetectomy with cages and bilateral complete facetectomy with cages. Analysis of the changes of ROM in different conditions and segments was performed. Results: Under bilateral medial 1/3 facetectomy, bilateral medial 1/2 facetectomy and bilateral complete facetectomy, posterior fusion cage could always significantly decrease the ROM of L4~5 segment in flexion, left and right lateral bendings, with the ROM of 4.50±2.43, 2.61± 0.82, 3.21±1.41,4.70±1.47, 3.43±0.75, 4.13±0.81,5.98±2.67, 3.89±0.70, 4.53±1.33 respectively(P0.01). For L5~S1 segments, under the bilateral medial 1/3 facetectomy, posterior fusion cage could always significantly decrease the ROM in flexion (4.70±2.50,P0.01)and right lateral bendings (2.32±0.99,P0.01); under the bilateral medial 1/2 facetectomy, posterior fusion cage decreased the ROM in flexion (6.02±2.15,P0.05)and under the bilateral complete facetectomy, decreased the ROM in flexion, extension, left and right lateral bendings (2.59±0.27、2.69±1.10、1.08±0.76、1.02±0.69, P0.01)and left right axial rotations (1.15±0.30,P 0.05). Conclusions: Posterior fusion cage has the affection on stability of L4~5, L5~S1 segment, especially for the latter. When posterior fusion cage need to be applied in L5~S1 segment, the bilateral facetectomy should be not more than 1/3.
