Abstract:According to the literature, there have only been 42 reported cases of cerebral artery embolization secondary to shotgun pellet missiles. The most common site of embolization is the middle cerebral artery followed by the internal carotid artery, the anterior cerebral artery, the posterior cerebral artery, and the angular artery, respectively. Management options range from surgical intervention to conservative medical management. Outcomes included death, neurological deficit, and complete symptom resolution. Neurological deficits reported include hemiparesis, quadrantanopsia, and others.1 We present the case of a 27-year-old man who sustained multiple shotgun wounds with embolization of a pellet to his right middle cerebral artery (Fig. 1). A 27-year-old man was brought in by ambulance after being found down with multiple shotgun wounds to his right neck, right upper extremity, right chest, and right lower extremity. Vital signs were stable on initial presentation except for desaturation into the low 90s on room air. His initial Glasgow Coma Scale was 15. A chest tube was placed for decreased breath sounds over his right thorax with a gush of air and a small amount of blood returned. His multiple shotgun wounds were hemostatic. He had no neurologic or vascular deficits. A computed tomography (CT) scan of the chest/ abdomen/pelvis showed a right hemopneumothorax with lung contusions, multiple pellets around his heart, and multiple pellets in his liver without contrast extravasation. CT scan of the head showed a pellet in the right middle cerebral artery territory without cranial injury (Fig. 2). The patient was admitted to the surgical intensive care unit for hemodynamic monitoring. Neurosurgery was consulted and conservative management with aspirin was recommended. His chest tube was removed on hospital Day 5 and he was discharged with aspirin on hospital Day 6. He was seen in the surgery clinic 2 weeks after discharge and had only minor pain at the shotgun wounds. He has since been lost to follow-up. Shotgun pellet embolization to the cerebral circulation is rare. An asymptomatic presentation of cerebral embolization of a metallic missile is extremely rare. Shotgun injuries to the neck and chest are the most common sources for cerebral embolization. Sequelae secondary to embolization range from hemiparesis to death. In this case presentation, this 27-year-old male sustained multiple shotgun injuries to his right neck, right torso, and right upper and lower extremities with pellet embolization through a pulmonary vein to his right middle cerebral artery. He was asymptomatic and was treated conservatively with aspirin.
