Abstract:Objective To investigate the current service status of general pediatric outpatient department in a third-grade class-A hospital in Beijing and to provide references for stratified health care and optimized mode of outpatient visit. Methods In this cross - sectional survey, uniform self - designed questionnaire was used to investigate children who paid outpatient visit to general pediatrics outpatient department of Peking University First Hospital,as well as their accompanying relatives and corresponding doctors from 8:00 to 21:00 on October 11,2013. The collected information covered general data, type of outpatient service, registration method, waiting length, reception length, reason of outpatient visit, times of visit, distance to the hospital,course of disease,diagnosis,treatment method and assessment of disease severity by accompanying relatives and doctors. ResuIts A total of 279 subjects completed the questionnaires. The children received expert outpatient service and general outpatient service took up 45. 5%(127/279) and 54. 5%(152/279). The children who made registration before treatment accounted for 96. 0% ( 268/279 ). The average waiting length was 30 ( 10,60 ) min. The first visit accounted for 46. 9%( 131/279 ), and the repeated visit accounted for 53. 1%( 148/279 ). The five leading reasons were fever(n=69,24. 7%),coughing(n=67,24. 0%),return visit(n=39,14. 0%),prescription only(n=25,9. 0%) and diarrhea(n=13,4. 6%),among which the ratio of prescription only in general outpatient service was higher than that in expert outpatient service(P﹤0. 05),and the ratios of fever and diarrhea in nighttime outpatient service were significantly higher than that in daytime service ( P ﹤0. 05 ). The five leading diseases were upper respiratory infection ( n =107,38. 4%), bronchitis(n=30,10. 8%),pneumonia(n=29,10. 4%),diarrhea/gastroenteritis(n=26,9. 3%)and epilepsy(n=12,4. 3%),among which ratios of upper respiratory infection and diarrhea/gastroenteritis in nighttime service were higher than that in daytime service(P﹤0. 05). There were 175(62. 7%)children who only took oral drugs for treatment,35(12. 5%) who needed further observation at home,32 ( 11. 5%) who received venous transfusion,17 ( 6. 1%) who only took aerosol inhalation,12(4. 3%)who were transferred to experts and 8(2. 9%)who were admitted into the hospital. The ratio of children who took oral drugs only in nighttime outpatient service was higher than that in daytime service ( P ﹤0. 05 ). The accompanying relatives who thought outpatient service ( very ) easy to access, barely easy to access and difficult to assess accounted for 49. 1% ( 137/279 ), 35. 8% ( 100/279 ) and 15. 1% ( 42/279 ). Difference existed in the assessment of disease severity between accompanying relatives and doctors(χ2 =7. 803,P﹤0. 05). Among 279 children,60. 2%(168/279) were suggested by doctors to receive treatment in eligible primary health care settings. Multivariate Logistic regression analysis showed that fever,return visit,course of disease(≥7 d),assessment of disease severity by accompanying relatives and doctors and treatment method were the factors influencing doctors'evaluation on whether patients should receive treatment in primary health care settings(P﹤0. 05). ConcIusion This third-grade class-A hospital undertakes plenty of primary health care service for children. The health care setting is chosen by sick childrenˊs families based on many factors. We suggest that the personnel training in local primary health care settings should be strengthened,parents' education on this should be promoted, relevant regulations should be formulated and safe and effective stratified health care should be put into practice.
