Abstract:Objective To investigate the clinical and electroencephalographic(EEG)characteristics,therapeutic response and prognosis of epilepsy with continuous spikes and waves during slow sleep(CSWS).Methods The clinical and EEG data of 16 children with CSWS were analyzed,and therapeutic response and prognosis were followed up.Results The onset ages were from 3 years and 8 months to 8 years and 4 months.All children had epileptic seizures,and partial motor seizures during sleep occurred in all children.The seizures during daytime included atypical absence seizure(8/16 cases),partial motor seizure(8/16 cases),atonic seizure(4/16 cases),myoclonic seizure(4/16 cases) and generalized tonic-clonic seizure(1/16 cases).Psychological abnormalities and motor regression occurred in all 16 children.Five children were preterm,2 children had family history of epilepsy,and 1 child was diagnosed as varient type of phenylketonuria.Electrical status epileptics during slow sleep(ESES)was observed on EEG monitoring in all the children.MRI abnormalities appeared in 8 children,including cerebral cortical dysplasia(2/16 cases),cerebral atrophy(2/16 cases),congenital corpus callosum hypoplasia(1/16 cases),and abnormal signal in bilateral posterior putamen(1/16 cases).Antiepileptic dugs(AEDs) could reduce some patients(4/16 cases) seizure,but had no effects on psychological abnormalities and motor regression.All children received high-dose of methylprednisolone intravenously.After treatment,13 children were seizure-free,and the condition of psychological abnormalities and motor regression was improved in 12 children.ESES got improved in 14 children while re-testing EEG.All children were followed up,and except 3 children who were 4 to 6 years old,other children were all seizure-free.Eight children got ESES and clinical recurrent when retesting EEG,and 6 cases of them received intravenous high-dose methylprednisolone therapy again,and 4 children got improvement apparently.Unfortunately,all children were below the average level intellectually.Conclusions CSWS is a rare age-dependent epileptic syndrome in children.Children with this disease all have epileptic seizures,mental and motor regression clinically.ESES of EEG is the specific feature of this disease.AEDs can reduce the frequency of the seizure in part of the patients,but had no effect on psychological and motor regression.Intravenous high-dose methylprednisolone therapy can significantly improve psychological and motor regression and ESES in most of the patients.The outcomes in terms of EEG and epileptic seizures are good,but psychological abnormalities and motor regression often been remain.
